1 Pinpoint Registration A database to map services for disabled children and young people in Buckinghamshire The Disabled Children Programme Buckinghamshire County Council has completed its Aiming High for Disabled Children (AHDC) Programme in line with the funding it received up to 31st March The Council is now continuing with its Short Breaks programme developed through AHDC. This is funded through the Early Intervention Grant (EIG). It continues to be a key priority to improve communication with parents of disabled children and we aim to do this through our AHDC Pinpoint Database. It is also a statutory requirement of the Children Act 1989 that all Social Services departments keep a database of disabled children to help with planning and monitoring services. What is the AHDC Pinpoint Database? AHDC Pinpoint is a voluntary database which will enable Buckinghamshire County Council to keep you informed about the AHDC Programme and services provided to disabled children and their families. This form is intended for completion by parents or carers of disabled children and young people who are: 0 19 years old AND Live in Buckinghamshire OR are the responsibility of the Local Authority. It is important that we include as many disabled children as possible on the database as this will help us to plan services for disabled children and their families in local areas based on accurate information. It will also be a vital source of information for Adult Services to support disabled young people in their transition to adulthood. By completing this form you are influencing where services are developed in Buckinghamshire for disabled children. On receipt of your completed form, your details will be entered on to the database and you will be sent a copy of Starting Point a directory of services for parents/carers with disabled children. 1

2 How will this data be used? The information that you provide on this form will be used to: Present you with useful information* about services for disabled children. * If you wish to receive this information please provide your address on the registration form. Provide statistical information to the Buckinghamshire Family Information Service, Social Care, Education, Health and voluntary organisations to help plan appropriate services for disabled children and their families in the future. All information held on the AHDC Pinpoint Database will be treated in the strictest confidence and is protected in accordance with the Data Protection Act Your signature on this form confirms your consent for the County Council to process data within the County Council systems. Some information may be shared with authorised personnel within the County Council and other appropriate organisations for the purposes stated above. You will have full access to the information that is held about your child and have the right to remove it at any time by contacting us on the number/ address provided below. Registration on the database is not a requirement to obtain any services in Buckinghamshire. Keeping the information up to date: It is important that the database is kept up to date. Every months you will be sent a copy of the information held about your child, and are asked to let us know about any changes that may have taken place. It would also help us if you could telephone or pinpoint@buckscc.gov.uk if there are any changes we need to know about e.g. address, school. 6 Examples for descriptions of disability on page 3 (Please note that these examples were developed through consultation with parents for the original Disability Register in 2000). Moderate physical disability: Child or young person may have mobility problems such as being unable to manage stairs; difficulty in getting up off the floor; needs aids or assistance in walking and may have a wheelchair. and/or The child or young person may have problems with hand function - as used in eating, washing, dressing, playing, writing/ drawing, holding objects etc - and find these activities considerably difficult or require aids or assistance. Severe physical disability: Child or young person may be unable to walk and be dependent on carer for mobility. and/or The child or young person may be unable to use hands other than to operate simple switch systems such as toys, wheelchair etc. and is dependent upon the carer. Moderate visual impairment: Child or young person may require special aids for reading or the impairment restricts mobility. This does not include children who only require spectacles to correct vision. Severe visual impairment: Child or young person is blind, is unable to read print and may be dependent on the carer for mobility. Moderate hearing impairment: Child or young person needs substantial aids (two hearing aids, phonic ear etc.). Also, if a child needs good acoustics and needs to face the speaker for lip-reading etc. Severe hearing impairment: Child or young person is deaf and may need non-verbal communication, e.g. signing, writing, computer keyboard. Moderate learning disability: Child or young person needs extra help and support in some areas of his/her learning difficulties. May be in mainstream school and require extra educational input. Will require modified curriculum and some adult support in daily living. Severe learning disability: Child or young person is very dependent on others for care because of his/her learning difficulties. Will require significantly modified curriculum at school, be unaware of common dangers and require considerable personal support. Moderate speech or language difficulties: Child or young person has a speech/language impairment but is able to make themselves understood. Severe speech or language difficulties: Child or young person has very little or no recognisable speech for their age and has to communicate need in other ways e.g. signing, eye movement etc. Moderate social communication disorder: Child or young person is on the autistic continuum and may require extra help and support. May be in mainstream school and require extra additional input. Will require modified curriculum and some adult support in daily living. Severe social communication disorder: Child or young person is on the autistic continuum and is very dependent on support because of his/her learning difficulties. May require significantly modified curriculum at school and considerable adult support in daily living. Severe behavioural / emotional difficulties: If behaviour patterns are sometimes well beyond those expected and behaviour is liable to cause self injury or injury to others, in need of constant supervision and often beyond the control of a single carer. Psychiatric illness: Child or young person has a diagnosed psychiatric condition, e.g. schizophrenia, severe depressive illness or other psychosis that makes him/her dependent on others in some areas. Chronic ill health: Child or young person has a permanent or long-term health condition that has a marked adverse impact on daily life e.g. cystic fibrosis, severe epilepsy, sickle cell anaemia.
