9 Multiple sclerosis (MS) is a chronic disease that affects the central nervous system, which includes the brain and spinal cord. Multiple sclerosis is an autoimmune disease where the immune system reacts against components of the nervous system and the main target of this immune attack is the myelin covering the axons. The axons are a part of a neuron (nerve cell) used to transmit and communicate information to other nerve cells and is analogous to rubber insulation over an electrical wire. Successful communication between the nerve cells requires intact myelin. Typically the neurological signs and symptoms occur when there has been myelin damage. 7
10 8 There are different patterns for the disease course, the treatment for different forms of MS differs, and patients can be categorized within one of these groups: Relapsing-Remitting MS This is the most common form of MS. In relapsing-remitting MS, patients have attacks or relapses in which new neurological symptoms or deficits such as numbness, weakness, visual changes or loss of balance appear for at least 1 day. The typical age of onset of this form of MS is the 20s-30s. Symptoms generally improve or remit to varying degrees in different patients. Intravenous steroids are often administered to speed up recovery. A relapse is a new neurological deficit that lasts for more than 24 hours. Attack or exacerbation is another name used for a relapse. Secondary Progressive MS This is a form of MS that generally occurs in relapsing-remitting patients years into the course of disease. In this form of MS, neurological deficits do not remit, but persist and cause chronic symptoms. Primary Progressive MS Approximately 10% of patients have primary progressive MS, in which progression occurs from the onset of disease. Primary progressive MS often begins after age 40 years. An increased proportion of men experience primary progressive MS compared to other forms of MS. Progressive Relapsing MS A minority of patients experience a course of disease in which relapses and progression are superimposed from the onset of disease. Neuromyelitis Optica (Devic s Disease) This is a form of MS that affects the optic nerves and spinal cord almost exclusively. An antibody marker in the serum is present in approximately 75% of patients.
