5 Service Description ID Watchdog Plus ID Watchdog Plus is our core identity monitoring, identity theft detection and resolution product. Plus monitors thousands of public and private databases searching for new and updated information associated with an employee s personal, identifiable and financial information. Plus includes monitoring of an employee s social security number, criminal records, address history, TransUnion credit report and more. ID Watchdog Platinum In addition to the advanced identity monitoring services offered with ID Watchdog Plus, ID Watchdog Platinum also includes credit report monitoring for all three credit reporting agencies (Equifax, Experian and TransUnion) as well as access to an employee s credit reports and credit scores from all three bureaus.* Credit reports and scores are automatically refreshed annually and any time suspicious activity is detected. Employees enrolled in both ID Watchdog Plus and ID Watchdog Platinum products are notified of changes to their records via alerts. Employees can also access their identity profile information, credit report information (Platinum only) and respond to alerts through our secure employee dashboard. Eligibility ID Watchdog services are available to employees, their spouses, and dependents. Household pricing includes the employee, a spouse, and up to 5 dependents through the age of 24 whose primary address is the same as the primary account holder.** Pre-Existing Conditions If a pre-existing threat of identity theft is present at, or prior to activation, additional charges may apply toward the investigation and resolution of such incidents. Prices to resolve existing cases range from $79.95 to $ per incident. Portability An employee has the option to continue ID Watchdog protection by paying the full amount of the discounted, monthly group rate directly to ID Watchdog within 30 days after the effective termination date. The employee will then have the option to continue paying the discounted group rate directly to ID Watchdog for up to twelve months following termination. After the twelve month period, regular retail pricing will apply for continued service to the employee. * The ID Watchdog Platinum, Employee + Spouse and Employee + Family tiers include credit reports and scores for up to two adult family members (e.g. employee and spouse). The ID Watchdog Platinum, Employee + Child(ren) tier includes a credit report and score for the qualified employee only. **Credit reports and scores for minors under age 18 are not available with any ID Watchdog product

6 Integration ENROLLMENT OPTIONS INCLUDE: Benefits Portal The Benefits Portal is an online web site available to both the broker and their employers. Employers can enroll, edit, change and cancel employees and their family members, view reports, and download documents. In addition to the employer features, brokers can add new employers and view roll up reports by employer. Enrollment data can be uploaded in bulk and processed offline or individually data entered into the system. Web Service API ID Watchdog also offers a light weight Web Services API allowing the Employer or Broker to directly integrate their Benefits Management System with ID Watchdog. ID Watchdog Hosted Enrollment ID Watchdog can support the open enrollment process on behalf of the employer. ID Watchdog will host a branded benefit registration page which the employer can direct their employees in order to register for their identity protection benefit. When enrollment has closed ID Watchdog will securely provide the enrollment list to the employer for verification and initiation of payroll deduction
