7 Benefits of using Telemedicine for Care Conferencing Patient at the centre and if providers at a distance can still connect Saves travel time and cost for all Able to fit CC into busy schedules because don t have to travel Better than connecting over the phone put a face to a name
8 For Example (care conferencing) 62 year old woman, low income, with multiple chronic diseases, chronic pain and significant wounds on her feet and legs Being cared for by multiple providers including high risk chiropodist Challenge is communication between CCAC/St. Elizabeth nurses and chiropodist as well as diabetes educator team and other providers involved in care Care conference organized in Smiths Falls as that is where patient lives high risk chiropodist joined via OTN from Brockville ++ benefits from both patient and provider perspective
9 For Example (access) 26 year old woman in Perth area Type 1 Diabetes on an insulin pump Frequent admissions to Perth hospital for DKA Health Link Care Coordinator met with patient Able to facilitate access to Diabetes nurse educator at HDH for insulin pump management by collaborating with Telemedicine team who then collaborated with Kingston Telemedicine Team Looking at creating an opportunity to have endocrinology appointments via TMS from Perth or Smiths Falls ++ benefits to patient and we hope the health care system
10 Personal Computer Video Conferencing (PCVC) OTN s web based Personal Videoconferencing service enables you to use videoconferencing for initial and/or follow up patient consultations and to do so from the convenience of your personal computer or laptop. Use OTN s Personal Videoconferencing in Ontario to see patients, case conference, participate in distance learning, and attend meetings.

14 Telemedicine Services in 5 Easy Steps 1. Fill out the Telemedicine referral 2. Fax referral form with relevant information to appropriate Telemedicine services regional contact 3. Telemedicine Coordinator will arrange appointment with required health care provider or specialist 4. Telemedicine Coordinator will contact the client and those involved with the appointment date time and location 5. Telemedicine nurse will support client during the appointment with technical and nursing expertise
