1 Key Performance Indicators (KPI) ServicePoint Guide Common terms Reporting Period (RP) The time period for which the report is pulled between and including the start date and the effective date. Active Clients: On the Entry Exit tab the Client has a Program Entry date and the Program Exit date is null Housed Clients: Active Client with a Date of Move In on the Intake Assessment Schedule A target number: The number of individuals or families that can be case managed at any given point in the year as outlined in the Schedule A of all CHF-funded programs. Occupancy Definition: Percentage of clients on the last day of the RP with a Program Entry and no Program Exit divided by CHF Schedule A target number Active clients (housed + unhoused). CHF Schedule A target number Entry/Exit Tab Limitations: This is a point in time calculation on the last day of the month. This is not the average of total active clients over the RP. If you have a client exit 1 day before the last day of the month they will not be considered Active (ex. RP = between and including October 1 st October 31 st. If a client exits on October 30 they are not considered an active client for the reporting period) Updated February

2 Percentage of Clients Housed Definition: Percentage of clients as of last day of the RP with a move-in date in their intake assessment divided by the number of active clients. Housed clients (move-in date). Active clients (housed + unhoused) HS Housing First Intake Interview (see below) and Entry/Exit Tab (see above) Limitations: If the program entry time on the Entry/Exit tab is after Intake date and time the client will be indicated as un-housed. Positive Reasons for Leaving Definition: The number of housed clients who upon exit have a Positive Reason for Leaving divided by the total number of housed exits within the reporting period. Housed clients with positive reasons for leaving. Total # of housed exits Entry/ Exit Tab Possible Reasons for inconsistencies: Choosing Other as it is classified as neutral. See Exit Categorization Updated February

3 Exit Destinations of Those with Positive Reasons for Leaving Definition: Percentage of clients who upon Program Exit have a positive reason for leaving and a positive exit destination Positive exit destinations of the Housed clients with positive reasons for leaving. Housed clients with positive reasons for leaving Same as above (Positive Reasons for Leaving) Limitations: Clients with positive reason for leaving and negative or neutral exit destinations. See Exit Categorization Proactive Interactions with Mainstream Services Definition: Percentage of housed clients who have received at least 1 referral within the first 6 months of program. Service referrals include all referrals completed on behalf of the client regardless of referral outcome. Referrals can be internal (within same program/agency) or external. # of housed clients with a referral within the first 6 months. # of housed clients who have a 6 month assessment completed HS Housing First Follow-up Assessment (3-60 month) Limitations: If the 3 and 6 month assessment has: None, Don't know and/or Declined to Answer chosen, they are not considered a referral. Reduction of Public System Utilization Definition: Percentage of clients with a reduction in public systems use at 12 months (measured by ER visit, EMS use, police interactions). Compare the Interactions at intake to the interactions at the 3, 6, 9 & 12 month follow-up assessments. Updated February

4 Calculate how many clients have had a decrease in System Use: Intake Assessment Results: ER visit, EMS use and police interactions questions are added together Follow-up Assessment Results: Results from same questions for the 3, 6, 9 & 12 month assessment are added together Compare intake results to the sum of the same questions for the 3, 6, 9 & 12 month follow-up results for each client Rule Intake # > Follow-up # Intake # < Follow-up # Intake # = Follow-up # Classification of System Use Decrease Increase Unchanged # of housed clients with a decrease in System Use. # of housed clients who have a 12 month assessment completed HS Housing First Intake Follow-up Assessment 3,6, 9 &12 Month Updated February

5 Limitations: In order for a client to be counted for this KPI, the above questions must have been answered at Intake as well as at 3 month and/or 6, 9, 12 month follow-up assessment. Please note: Effective April 1, 2015 this KPI will calculate System Use at 12 months compared to Intake. Income at Exit for those with Positive Reasons for Leaving Definition: Percentage of exited clients with a positive reason for leaving of Completed Program and income at exit. Calculate how many housed clients had a positive reason for leaving of Completed Program within the RP and have income at exit # of housed clients at exit with a positive reasons for leaving of Completed Program & Income # of housed clients at exit with a positive reasons for leaving of Completed Program 1. Entry Exit Tab See Positive Reasons for Leaving for visual 2. HS Housing First Exit Interview Limitations: None Program Retention and Positive Reason for Leaving Definition: Percentage of clients remaining in program at the end of the RP OR exited with positive reasons for leaving within the RP Entry Exit Tab See Proactive Interactions with Mainstream Services & Positive Reasons for Leaving for visual Limitations: None.
