1 La Fe Preparatory School Social Worker Application Requirements Eligibility Minimum of Bachelor s degree in Social Work from a School or Program accredited by the Council on Social Work Education. Two years experience as Social Worker in school, community, resource or mental health setting. Current license or eligible for licensure in Texas as a Licensed Social Worker (LSW) as required by Chapter 50 of the Human Resources Code. Dedicated to the vision of the parent organization, El Centro de Salud Familiar La Fe, Inc. and understands the history and culture of the Segundo Barrio. Able to work cooperatively with supervisors, colleagues, parents, and students. Evaluation Process All written applications will be screened and evaluated by the Superintendent, Principal, and Administrator of Social Services. The finalist will be selected by a panel of judges comprised of educators, administrators and community leaders. The panel of judges will conduct personal interviews with each finalist. 2 JOB DESCRIPTION Job Title: Social Worker for La Fe Preparatory School I. JOB SUMMARY Responsible for providing comprehensive social services to students and their parents/guardians in an academic setting to include the adequate and timely care planning processes as an integral component of multidisciplinary team and through the use of a student-centered approach to casework-using skills to provide a strengths-based perspective in maximizing service care plans. Social Worker performs duties in a proactive manner to help students and families overcome barriers to services by exploring the use of community and La Fe program resources. Services include inquiry/intake, assessment, crisis intervention, case management; follow up, home visits, advocacy and information and referral to community resources. An essential part of social services include accurate documentation of all services provided. II. ESSENTIAL JOB FUNCTIONS 1. Ensures that any and all services provided follow Department of Social Services Policies and Procedures, the Texas State Board of Social Work Examiners Licensing Standards and the National Association of Social Workers Professional Code of Ethics and La Fe s standards. 2. Performs social work casework roles; acting as a liaison, advocate, educator, facilitator with student, family members, teachers, administrators and community resources to maximize assistance with service care plan needs. 3. Provides case management services to include; intake, needs assessment, individual service care plans, supportive guidance, linking to community resources, home visits, follow up services and helping students and families access therapy or counseling services. 4. Assists educators and administrators with challenging behaviors of students through the implementation of in-classroom support, interventions and strategies. 5. Utilizes the strength-based prospective and problem solving approach with students, families and staff in the best interest of the students. 6. Provide support to staff with urgent or crisis situations to include proper reporting procedures for abuse, neglect, safety concerns, or threats to harm oneself or others. 3 7. Facilitate, plan and organize presentations/trainings for teachers and parents on relevant topics. 8. To participate in planning sessions with school staff and administrators regarding events, schedules and curriculums to be used throughout the school year to incorporate social work component. 9. To participate in the planning and program of monthly parent nights and meetings. 10. Oversee parent coordinator to help organize and involve parents in school activities, to volunteer and take an active role in their children s education. 11. Participate in ARD review meetings for special education students at the parent/guardian or administrators request. 12. Monitors student and family progress towards care plans through communication with teachers, administrators and family feedback and input. 13. Identifies and maintains working knowledge of community resources to maximize student and family access to opportunities. 14. Documents direct services and all communication with, on behalf of and for students and their families in student record. 15. Performs other duties as assigned. III. MINIMUM JOB REQUIREMENTS A. Work Experience Two years experience as a social worker in a school, community resource or mental health setting. B. License/Registration/Certification 1. Current license or eligible for licensure in Texas as a Licensed Social Worker (LSW) as required by Chapter 50 of the Human Resources Code. 2. Possession of an appropriate Texas Driver s License, vehicle liability insurance, and good driving record. C. Education and Training 1. Ability to establish and maintain effective working relationships with clients, agency staff and other organizations. 3. Ability to provide crisis intervention, casework and case management, assessment, planning, implementation, monitoring, and evaluation of psychosocial and financial needs of clients. 4. Ability to exercise good judgment in unusual or stressful situations. 5. Ability to communicate effectively, both orally and in writing. 6. Ability to represent La Fe Preparatory School in public functions, presentations, organizations, etc. 7. Ability to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the students and families served. 8. Knowledge of the principles of growth and development over the life span. 9. Ability to identify and categorize each student s age-specific grouping of needs, such as those for infant, adolescent or aging patients. 10. Bilingual English/Spanish. 11. Knowledgeable of Social Work principles and standard practices. Skilled in areas of communication with various student, family, academic, and community levels, and the use of skills in rapport establishment, engagement of the student and family members, focusing, negotiation, conflict mediation, facilitator and advocacy within a student, family, and team centered approach to care in an school care setting a psychosocial perspective to student care. 12.
