Alternatively, you can complete a hardcopy MLC Insurance Product Disclosure Statement Application Form available at mlc.com.au When selecting Payment Authorities on the application form simply select to have payments of both/either initial and ongoing fees deducted from your account. You ll need to provide investor account number(s) to tell us which account the fees will be debited from. Once completed, you ll be able to view the client s insurance details.
