3 Meaningful Use The set of standards defined by CMS Incentive programs that governs the use of electronic health records and allows eligible providers and hospitals to earn incentive payments by meeting specific criteria. 3 Goals of Meaningful Use: Complete and accurate information Better access to information Patient empowerment

4 Engaging Patients Better access to information EHR s facilitate better access to information providers need to make healthcare decisions and also allow information to be shared more easily between doctors, hospitals, and other care settings Patient empowerment Empower patients to take a more active role with immediate access to their health information or their families health information. Patients can receive an electronic copy of their health information or access this information securely over the internet.

9 Importance of the 3 E s Vital to improving patients health outcomes Well informed patient leads to greater compliance Improved communication between provider and patient Aids in decision making Patients are able to more effectively manage their own care Active involvement of patients and families can lower underuse or over use of medical services and reduce health care costs Patients seek care from multiple providers, patients who have copies or access to their medical records can share those with providers and avoid unnecessary or duplicative laboratory tests or procedures.

11 Eligible Professional Core Set Objective: Provide clinical summaries for patients for each office visit Measure: Clinical summaries provided to patients for more than 50% of all office visits within 3 business days 11

12 Eligible Professional and Hospital Core Set Objective: Provide patients with an electronic copy of their health information upon request Professional- including diagnostic test results, problem list, medication lists, and medication allergies Hospital-including diagnostic test results, problem list, medication lists, medication allergies, discharge summary and procedures Measure: More than 50% of all patients who request an electronic copy of their health information are provided it within 3 business days 12

13 Eligible Hospital Core Set Objective: Provide patients with an electronic copy of their discharge instructions at time of discharge, upon request Measure: More than 50% of all patients who are discharged from an eligible hospital or emergency department and who request an electronic copy of their discharge instructions are provided it at discharge 13

14 Eligible Professional Menu Set Objective: Provide patients with timely electronic access to their health information (including lab results, problem list, medication lists, medication allergies) within four business days of the information being available to the eligible professional Measure: More than 10% of all unique patients seen by the eligible professional are provided timely electronic access to their health information subject to the eligible professional's discretion to withhold certain information 14

15 Eligible Professional and Hospital Menu Set Objective: Use certified EHR technology to identify patientspecific education resources and provide those resources to the patient if appropriate Measure: More than 10% of all unique patients seen by the eligible professional or admitted to the eligible hospital's inpatient or emergency department during the EHR reporting period are provided patient-specific education resources 15

19 Eligible Professional Core Set Objective: Provide clinical summaries for each office visit Measure: Clinical summary provided within 1 business day for more than 50% of office visits Summary must include: Patient name, Provider s name and contact info, Date and location of visit, Reason for office visit, Current problem list and any updates, Current medication list and any updates, Current medication allergy list and any updates, Procedures preformed during the visit, Immunizations or medications administered during the visit, Vital signs and any updates, Laboratory test results, List of diagnostic tests pending, Clinical instructions, Future appointments, Referrals to other providers, Future scheduled tests, Demographics maintained by EP, Smoking status, Care plan field including goals and instructions, Recommended patient decision aids 19

20 Eligible Professional Core Set Objective: Use secure electronic messaging to communicate with patients on relevant health information Measure: Send a secure message using electronic messaging function of certified EHR technology to more than 5% of unique patients 20

21 Eligible Professional and Hospital Objective: Core Set Provide patients the ability to view online, download, and transmit their health information Measure: More than 50% of all unique patients have timely online access and more than 5% of unique patients view, download, or transmit to a 3 rd party their health info 21
