2 Meaningful Use Overview We are at the forefront of the patient engagement era. The American Recovery and Reinvestment Act of 2009 included the Health Information Technology for Economic and Clinical Health (HITECH) Act, which gives the Department of Health & Human Services (HHS) the authority to establish programs to improve healthcare quality, safety, and efficiency through the promotion of health IT, including electronic health records and private and secure electronic health information exchange (HIE). In order to accomplish these goals, the Centers for Medicare & Medicaid Services (CMS) and the Office of the National Coordinator for Health Information Technology (ONC) established Meaningful Use (MU), which uses a combination of financial incentives and penalties to encourage healthcare providers to comply with recommended standards. MU Stage 1 laid the foundation for EHR technology, and in Stage 2, we are seeing more of a push to focus on improving clinical outcomes. MU Stage 2 has led to an increased focus on consumer engagement, including requirements that providers meet consumer uptake percentages. InstantPHR TM was developed to enable healthcare providers to deliver high quality patient care. The product s ONC 2014 Edition Modular EHR Certification designates that the software is capable of supporting healthcare providers with Stage 1 and Stage 2 Meaningful Use objectives and measures. Meaningful Use requires the utilization of ONC Certified Health IT products. InstantPHR TM MU Summary Get Real Health s InstantPHR TM is an ONC 2014 Edition Modular Certified EHR Technology (CEHRT) that meets both ambulatory and inpatient criteria for: (a) Clinical (a)(15) Patient-specific education resources (d) Privacy and Security (d)(1) Authentication, access control, and authorization (d)(5) Automatic log-off (d)(8) Integrity (e) Patient Engagement (e)(1) View, download and transmit to 3rd party (e)(3) Secure messaging (Ambulatory only) (g) Utilization (g)(1) Automated numerator recording (g)(4) Quality management system InstantPHR TM provides EPs and hospitals with an ONC 2014 Edition CEHRT to fulfill core patient-centric objectives and measures. Additionally, InstantPHR TM is a certified technology for automated numerator recording. This feature enables EPs and hospitals to accurately track and report percentage-based objectives. The last year to begin participation in the Medicare EHR Incentive Program is 2014 with the last payments disbursed in 2016, and the last year to begin participation in the Medicaid EHR Incentive Program is 2016 with the last payments disbursed in With $27 billion available for incentive payments from CMS, eligible professionals (EPs) can receive up to $44,000 in Medicare payments and $63,750 in Medicaid payments. Hospitals can potentially qualify for incentive payments of $2 million or more. Page 2 of 6
3 The chart below depicts the correlation between Stage 2 objectives and measures that are required criteria for 2014 Edition EHR certification. Eligible Professionals Core Measure 7 Provide patients the ability to view, download, and transmit their health information within 4 business days of the information being available to the EP. Measure 1: More than 50 percent of all unique patients seen by the EP during the EHR reporting period are provided timely (available to the patient within 4 business days after the information is available to the EP) online access to their health information. Measure 2: More than 5 percent of all unique patients seen by the EP during the EHR reporting period (or their authorized representatives) view, download, or transmit to a third party their health information (e)(1) View, download and transmit to 3rd party. Core Measure 13 Use clinically relevant information from Certified EHR Technology to identify patient-specific education resources and provide those resources to the patient. Patient-specific education resources identified by Certified EHR Technology are provided to more than 10 percent of all unique patients with office visits seen by the EP during the EHR reporting period (a)(15) Patient-specific education resources Core Measure 17 Use secure electronic messaging to communicate with patients on relevant health information. A secure message was sent using the electronic messaging function of CEHRT by more than 5 percent of unique patients (or their authorized representatives) seen by the EP during the EHR reporting period (e)(3) Secure messaging (Ambulatory only) Page 3 of 6
4 Eligible Hospitals/Critical Access Hospitals Core Measure 6 Provide patients the ability to view online, download, and transmit information about a hospital admission. Measure 1: More than 50 percent of all unique patients discharged from the inpatient or emergency departments of the eligible hospital or CAH (POS 21 or 23) during the EHR reporting period have their information available online within 36 hours of discharge. Measure 2: More than 5 percent of all unique patients (or their authorized representatives) who are discharged from the inpatient or emergency department (POS 21 or 23) of an eligible hospital or CAH view, download or transmit to a third party their information during the EHR reporting period (e)(1) View, download and transmit to 3rd party. Core Measure 10 Use clinically relevant information from Certified EHR Technology to identify patient-specific education resources and provide those resources to the patient. More than 10 percent of all unique patients admitted to the eligible hospital s or CAH s inpatient or emergency departments (POS 21 or 23) are provided patient-specific education resources identified by Certified EHR Technology (a)(15) Patient-specific education resources. CMS Definitions of Unique Patient for EH and CAH s Reporting Purposes If a patient is admitted to an eligible hospital s or CAH s inpatient or emergency department (POS 21 or 23) more than once during the EHR reporting period, then for purposes of measurement that patient is only counted once in the denominator for the measure. All the measures relying on the term unique patient relate to what is contained in the patient s medical record. Not all of this information will need to be updated or even be needed by the provider at every patient encounter. This is especially true for patients whose encounter frequency is such that they would see the same provider multiple times in the same EHR reporting period. Note: EPs, EHs, and CAHs will be required to provide the denominator when attesting for Meaningful Use.
