Please write a descriptive paragraph that details your qualifications for a scholarship and indicate why you believe you should be selected as a scholarship recipient. Please describe your career goals and why you chose that career.

4 Please provide a descriptive list of your extracurricular activities. I understand that the information on this form as well as my transcripts may be given to the scholarship selection committee. If I am awarded a scholarship, I hereby give permission for the release of my academic progress to the donor and for the release of any publicity pertaining to the scholarship. I understand that if I am awarded a scholarship, I will be asked to attend the Annual Awards Day Program and to thank the donor of the scholarship in writing. I also understand that I may be called on to represent the college at special events and/or other college functions.
