All information pertaining to the applicant i.e. business plan, supporting evidence, etc. is on file at the Board and is available upon request. Any interested person who has an economic interest in the outcome of these applications may serve and file an objection within 29 days of this publication. The objector shall: 1. complete a Notice of Objection Form, 2. serve the applicant with the objection, 3. file a copy of the objection and provide proof of service of the objection on the applicant with the Board, 4. pay the appropriate fee. (1) Despite section 4, one place of worship and one dwelling used in conjunction with the place of worship, together with accessory buildings and structures, may be erected, located and used on the land described in subsection (3) if the following requirements are met: Requirement Place of Worship Residence Maximum building 10 metres 10 metres height Minimum side yard 14 metres 14 metres Minimum rear yard 21 metres 21 metres Maximum gross floor 860 square 605 square metres area metres (2) In addition to the requirements set out in subsection (1), a minimum of 49 parking spaces is required on the land described in subsection (3). (3) Subsections (1) and (2) apply to the land in the Town of Markham in the Region of York, being part of Lots 28 and 29 in Concession 6 and further identified as Property Identifier Number (LT) and registered in the Land Registry Office for the Land Titles Division of York ( 65). The definition of schedule of benefits in subsection 1 (1) of Regulation 552 of the Revised Regulations of Ontario, 1990 is amended by adding the following paragraph: 26. Amendments dated January 15, 2015 (effective April 1, 2015); 2. The Regulation is amended by adding the following section: 37.9 (1) Despite subsection 37.1 (2), the amount payable for an insured service rendered by a physician in Ontario, as set out in the schedule of benefits, shall be reduced by 2.65 per cent if the service is rendered on or after February 1, (2) For greater certainty, the reduction under subsection (1) is in addition to, and not in place of, any other reduction provided for under this Regulation. Commencement 7/15 3. (1) Subject to subsections (2) and (3), this Regulation comes into force on the day it is filed. The member has the knowledge, skill and judgment to perform the procedure safely, effectively and ethically. 2. Before performing the procedure, the member determines that the newborn s condition warrants performance of the procedure, having considered, i. whether the procedure is necessary to preserve or restore the health of the newborn, ii. the known risks and benefits to the newborn of performing the procedure, iii. the predictability of the outcome of performing the procedure, iv. the safeguards and resources available in all the circumstances to safely manage the outcome of performing the procedure, and v. other relevant factors specific to the situation. 3. The member has successfully completed an intubation training program approved by the Council. Commencement 2. This Regulation comes into force on the day it is filed.
