3 Dear Family Members, We are appreciative of the opportunity to take care of your loved one at this difficult time. Please know that our objective is to provide quality inpatient care for children, adolescents and adults in need of crisis stabilization, and to deliver services that are personalized, comprehensive and cost-effective for patients. We believe each person s individual needs must be identified and treated in order to facilitate attainment of his/her optimal level of functioning and good health. Our goal is to ensure a safe and secure therapeutic environment where rapid stabilization of symptoms can be achieved with the help of our compassionate, highly-skilled and respectful professionals. ADMISSIONS. To be admitted, individuals may come to us on either a voluntary or involuntary basis. Involuntary admissions typically happen when one is a danger to self or others; or when one becomes gravely disabled and unable to execute normal, routine activities in their daily life. TREATMENT. Our treatment programs offer an intensive range of modalities including medication management, medication education, individual therapy, family therapy, psychosocial rehabilitation education and case management services. Our licensed therapists and doctors utilize Cognitive Behavioral Therapy (CBT), with a strong focus on family involvement and re-integration into the community with appropriate support and post-discharge services. Individuals participate in various groups based on their diagnosis, level of functioning and specific treatment needs, as identified in a treatment plan developed by the patient s treatment team. Staff members are trained in cultural competency and services are available without discrimination. While we recognize many people who have a psychiatric disorder may also have a substance abuse disorder, we are not a substance abuse treatment organization. We do provide education and referrals for specific substance abuse treatment. Information on state laws and statutes regarding therapy is available in our administration office. FAMILY PARTICIPATION. Families and significant others are highly encouraged to participate in their loved ones treatment AND IT IS REQUIRED FOR FAMILIES OF CHILDREN AND ADOLESCENTS. Family involvement is imperative to a minor s treatment and ensures better outcomes for the patient. All efforts will be made to help the patient s support system understand the illness from a multidimensional perspective as this understanding promotes healing and stability. Mental health workers, case managers, social workers, therapists and/or nurses assist the patient and their support systems, working with all on family dynamics, social and medication issues, discharge and post-discharge planning. 6 Visitation & Contact Guide While your loved one is a patient in our hospital we encourage you to call and visit them. Having visitors helps our patients feel supported and cared for by those who are important in their lives. Because of extremely limited areas for visitation, we ask that you call ahead and SCHEDULE your visits so we can put you on the visiting schedule each day and be sure we have enough room for everyone to have visitors. VISITOR HOURS West Springs North 6:00 7:10p Daily West Springs South/Children & Adolescents 6:00-7:10p Daily West Springs South/Adults 7:15-8:25p Daily Due to limited space, visitation time is limited to 10 minutes. As a courtesy to all our patients, please make every effort to arrive at the time you are scheduled and be ready to leave at the end of 10 minutes so the next visitors can begin their visitation. Only immediate family members are allowed visitation with all patients under the age of 18. All visitors must be aged 12 or older. Only two visitors per patient at any time. Please leave all valuables in your car. Purses, backpacks, keys, cell phones, pens, pencils, illegal items and outside food & drink are not allowed in the hospital. Please refer to the list of items considered contraband in the hospital and leave them home or in your car. Additionally All visitors must be on the patient s approved contact list. Visits by the patient s clergy, attorney or physician will not be restricted or denied at any time. All visitors must sign in/out and wear a visible visitors badge. All visitors must read and sign a confidentiality statement, indicating understanding of patient confidentiality rights. Visitors will be required to have a wand scan to ensure no dangerous items are being brought into the hospital. Visitors in possession of unapproved items will be denied visitation until the visitor exits the building and returns without the item(s). Items such as keys will be inventoried by staff, kept locked during visit and returned to visitor upon departure. In the event that a visitor is suspected of passing drugs to a patient, the visitor will be asked to leave the premises immediately. Should the visitor resist, law enforcement shall be called for assistance.
7 Visitors who smell of alcohol and/or drugs, or exhibit any symptoms of alcohol or drug intoxication, will be asked to leave the premises immediately. Should the visitor resist, law enforcement shall be called for assistance. Visits may be ended prematurely at the discretion of staff if the visit becomes stressful or unsafe. Circumstances such as this will also be documented in the patient s medical record. Visitors who have a communicable disease, such as mumps, measles, influenza, chicken pox or lice, or who have experienced a fever, cough, runny nose, sore throat, skin rash, vomiting or diarrhea in the last 48 hours should not visit patients until they are symptom-free and not a risk for transmitting disease to our patients. Staff may refuse visitor entry or request a visitor to leave if they are displaying symptoms of a communicable disease. Patients who are discharged from our hospital will not be allowed to visit remaining patients for 90 days after discharge. MAIL Patients are able to send mail and to receive mail. If you would like to send mail to one of our patients please address to: West Springs Hospital /4 Road Grand Junction, CO Visitation, phone calls and the ability to send and/ or receive mail may change or be limited during hospitalization based on provider discretion. 8 Contraband Items considered hazardous to our patients, staff or visitors are called Contraband. Safety is always our first concern. Our current list of items not allowed on our patient care units includes, but is not limited to: Plastic bags Drawstrings or strings in pants, shirts or jackets and shoelaces Belts, ties and scarves Jewelry necklaces, earrings or other body piercing jewelry (may have watches without metal bands), unless authorized by physician Drugs or unidentified substances or powder Knives, guns or metal objects designed as weapons Metal or wire hangers, letter openers Sewing needles, hooks, safety pins, straight pins, scissors, metal nail clippers or files of any kind Clothing with drug, alcohol, sexual themes or potential addicting themes i.e., gambling, tobacco, casinos Any outside food or beverages Electrical devices with a cord i.e. hair dryers/curling irons, razors, radios, earphones etc Audio/visual recording devices Hard plastic objects Books and magazines not approved by the treatment team. Exceptions are materials pertinent to a patient s religion Nail polish and nail polish remover Cigarettes, lighters, matches and any other tobacco products. CWPH is a NO SMOKING/ NO TOBACCO facility Underwire bras Glass or metal of any kind, including bottles; cans; frames; make-up in glass bottles, with mirrors, or metal in the handles of the make-up brushes Perfumes, cologne or aftershave Steel-toed boots and high-heeled shoes Hairsprays and any liquids containing alcohol Aerosols of any kind Cameras, mp3 players, internet-capable electronics, pagers or cell phones. Cell phones may be used to extract contact information under staff supervision, then immediately returned to facility safe. Electric razors Any other item considered potentially harmful, non-therapeutic or altered from original use
9 On admission, the patient will have his/her belongings checked in and explanation of the contraband policy provided. Illegal substances or contraband found in a patient s possession will be turned over to law enforcement and a detailed acceptance of the item attached to the patient s personal check-in list. These include but are not limited to: Unregistered guns Knives Illegal drugs or unidentified substances or powder The people working here are amazing and have helped me change my life in a positive way. K.W., PATIENT 2013 Not allowed in the hospital Medical Marijuana Use Patients admitted to the hospital possessing medical marijuana, associated paraphernalia and a legal Colorado medical marijuana card will have those items recorded, bagged and secured separately from any other items. Mind Springs Health is a smoke-free campus. This policy will be reviewed with the patient as well as the federal statute banning marijuana use. The patient s primary care provider will be notified within 24 hours of admission that the patient has a medical marijuana card, marijuana and paraphernalia. The patient s medical marijuana use will be addressed during team treatment planning. Items not returned to the patient at discharge will be turned over to law enforcement for disposal. A patient s identity, privacy and confidentiality will be protected at all times when a request for assistance is made of law enforcement.

Up to 3 complete changes of clothing, including socks and underwear (underwire bras are not permitted) Shirts must cover midriff Pajamas or shorts and t-shirts to wear at bedtime Winter coats, hats, gloves and a sweatshirt or light jacket without string ties. (during cold weather months) Shoes, including gym shoes for recreation time ITEMS TO LEAVE AT HOME Clothing with obscenities, drug, alcohol, gambling, tobacco or casino themes Pants with excessively frayed hems or chains or metal studs, straps or clips Drawstrings or strings in pants, shirts or jackets Belts Shoelaces Cell phones, pagers Video games CDs or MP3 players Jewelry Cash Anything valuable SUGGESTED PERSONAL CARE ITEMS Hair brushes Combs Toothbrushes Toothpaste Shampoo, conditioner Non-alcohol body or perfume sprays in unbreakable containers Make-up in soft plastic containers Contact solutions in plastic containers Non-alcohol mouthwash Notebooks without wire binders Envelopes, stamps Kleenex The following are checked out and used with staff supervision: Curling irons and hair dryers Electric razors OTHER PERSONAL ITEMS PATIENTS KEEP IN THEIR ROOMS Bibles or other religious books Books no hardback, only paperback Pillows and comforters, bedspreads or blankets What a patient should have in the hospital

16 Family Family Services Services Family services Family support services are available to all our patients family members. Our family support staff is knowledgeable because they ve walked in your shoes, or a similar pair. We know how important it can be to connect with others who have experienced the kinds of issues your family is dealing with now. We also remember how lost and overwhelmed we felt at times, searching for information and answers. And while we may not have all the answers you need, we do have a lot of information and the ability to make your path just a little bit easier. With Family Services, you can receive Support Encouragement Education of many kinds Experience Understanding from those personally experienced A listening ear in a safe place to be heard We may be able to help with Getting some of your many questions answered Gaining knowledge about mental health recovery and how it can happen Connecting to resources Reclaiming your own life Family Support Groups At 5pm on Mondays, feel free to meet with our peer staff in the North lobby of Mind Springs Health, the two story building on the Mind Springs Health Grand Junction campus. At 6pm on the 1st Monday of each month there is a support and education group geared primarily for families of adults. It features a variety of educational information important to family members on various diagnoses. If there is subject matter you would like to see addressed, please let us know and we will do our best to meet your requests. For families of children and youth we provide parenting support and attendance at Individualized Education Plans or other community meetings. We know the challenges of raising a child or youth with a mental health diagnosis. It s not easy and you deserve to have all the support you need. 17 PATIENT ADVOCACY A very important person in the hospital is our Patient Representative, Jenni Stansberry, who serves as an advocate for our patients. Jenni is available to patients and/or loved ones to help in clarifying needs, linking inquiries to the right resources and assisting understanding of information, both given and received. She can help you with both grievances and compliments regarding patient rights and the quality of care & service received at the hospital during a patient s stay.
