16 INVESTIGATIVE REPORT ASSIGNMENT On Wednesday, April 9, 2014, at 3:44 P.M., I was notified by the Florida Highway Patrol Orlando Regional Communication Center of a possible fatal traffic crash that occurred on State Road 551 (Goldenrod Road) northbound, north of University Blvd., at the address of 4035 Goldenrod Road, Orlando, FL in Orange County, Florida. The address is the business of Kindercare Learning Center (daycare). I arrived on scene at 4:11 P.M. This crash involved two vehicles. One of the vehicles entered the building of Kindercare Learning Center and the other vehicle left the scene of the crash. There were multiple children pedestrians from the building transported to hospitals. As a result of the crash, one child pedestrian suffered fatal injuries, two serious bodily injuries, 10 non-incapacitating injuries and adult pedestrian non-incapacitating injury. One occupant from the vehicle who entered the building was treated for injuries on Thursday, April 10, ROADWAY The initial crash occurred on northbound outside lane of State Road 551 (Goldenrod Road) at the business entrance to 4035 Goldenrod Road, Orlando, FL (Kindercare Learning Center). State Road 551 is a designated north-south roadway and the roadway travels in a north-south direction in the area of the crash. State Road 551 consists of two northbound through lanes and two southbound through lanes. There are two southbound inside left turn lanes south of the area of the crash. There is one southbound outside right turn lane south of the area of the crash. There is one Case Number FHP Page 4 : sisee
17 INVESTIGATIVE REPORT northbound inside left turn lane north of the area of the crash. The northbound lanes were divided from the southbound lanes by a raised concrete median. The median is 6 feet 3 inches in width. The width of the northbound outside lane is 13 feet 8 inches. The width of the northbound inside lane is 11 feet 9 inches. The width of the northbound inside left turn lane is 11 feet 2 inches. The width of the southbound inside left turn lane is 11 feet 2 inches. The width of the southbound outside left turn fane is 11 feet 2 inches. The width southbound inside through lane is 11 feet. The width of the southbound outside through lane is 13 feet 6 inches. The width of the southbound outside right turn lane is 12 feet 6 inches. The outer edges of the northbound and southbound outside lanes are abutted by a raised concrete curbing. The outside edges of north and southbound State Road 551 is marked by solid white painted line. The through lanes for northbound traffic are separated from each other by intermittent white pained lines. The through lanes for southbound traffic are separated from each other by solid white lines for the first 200 feet north of the intersection of University Blvd. than change to intermittent white painted lines. The inside edge of northbound and southbound of State Road 551 are marked by solid yellow painted line. State Road 551 is composed of asphalt that is in a traffic polished condition. The coefficient of friction of eastbound State Road 551 is.65. The grade at the area of the crash is 0%. The superelevation of the roadway at the area of the crash is 0%; however, the roadway is crowned for water drainage. The posted speed limit for eastbound State Road 551 is 45 miles per hour. 18 INVESTIGATIVE REPORT north of the area of collision. There were no adverse highway characteristics or defects found that would have contributed to this crash. The most southern portion of the driveway entrance to Kindercare Learning Center is 275 feet north of the intersection for University Blvd. The entrance to Kindercare Learning Center has a 7% upgrade for the first 8 feet east of State Road 551 and then turns into a gradual downgrade of 5%. The driveway for Kindercare Learning Center is 23 feet 6 inches in width. The raised concrete curbing directly in front of the Kindercare Learning Center that abuts the northbound lanes has a grass shoulder that is 3 feet in width to the east. There is a paved concrete sidewalk near State Road 551 that abuts the east side of the grass that is 5 feet 2 inches in width. There is a grassy area that separates the paved concrete sidewalk from another concrete paved sidewalk. The grassy area is 17 feet 8 inches in width. The paved concrete sidewalk near the parking lot east of the grassy area is 5 feet 2 inches in width. There is another strip of grassy area east of the second sidewalk shaped in an upside down L" from a northerly direction. The thin portion of this grassy area is 5 feet in width and the north portion widest section is 23 feet in width. The parking lot for Kindercare Learning Center is east of the grassy area shaped in an L form. The entrance to the parking lot east of the grassy area is 23 feet in width. The parking lot is 59 feet 9 inches in width from the west outer edge to the east outer edge. There are 11 parking spaces on the west side of the parking lot, closest to the State Road 551 and there are 11 parking spaces on the east side of the parking lot, closest to the building. The concrete ramp leads to the raised 6 inch concrete sidewalk east of the parking lot. The concrete ramp is 3 feet 8 inches in width. The paved concrete sidewalk that abuts the east side of the parking lot is 5 feet 2 inches in width. There is a landscaped area that abuts the east side of the sidewalk directly in front of the Kindercare Learning Center building. The landscaped area is covered with mulch and had shrubbery bushes in a line formation in front of the building. The Kindercare Learning Center building was built in the year of The length of the front of the building is 70 feet and the width of the building is 70 feet. There are two rooms on the north side of the building 24 feet wide. The room to the front of the building is 30 feet 8 inches in length. The room to the rear of the building is 37 feet 8 inches in length. The surrounding outside walls was made out of a wood frame installed with cooling/heating installation. The lower portion of the outer wall was lined with cement bricks. The flooring for the two rooms was installed with vinyl tile. The two rooms were separated by a dividing wall that was constructed out of drywall held up by wooden studs and a door. VEHICLE: V-1: V-1 was a silver 2005 Dodge Durango. V-1 was equipped with an automatic transmission, power-assisted steering and power-assisted front and rear disc brakes. V-1 was equipped with V liter engine and rear wheel drive chassis. V-1 was equipped with standard occupant restraint devices which consisted of lap belts and Case Number FHP Page 7 * S3 J.
20 INVESTIGATIVE REPORT shoulder harnesses. V-1 was also equipped with driver and front seat passenger air bags which did not deploy during the crash. V-1 had the assigned Florida tag number and V.I.N. number. OCCUPANT: V-1 DRIVER: (D-1): D-1 was Robert Alex Corchado, a maie who was 28 years old at the time of the crash. D-1 resided D-1 had a valid Florida class E operator s license with no restrictions and endorsement. Physical evidence in the form of (fingerprints and D.N.A. samples) from D-1 and statements made by an independent witness located at the address of 32808, was used to identify Robert Alex Corchado as the driver of V-1 and sole occupant. It is unknown if D-1 was wearing his occupant restraint device at the time of the collision. This investigation was unable to determine where D-1 was coming from or going to. Just prior to the area of the crash, V-1 was traveling northbound on State Road 551 in the outside lane approaching the business entrance to Kindercare Learning Center. D-1 should have been familiar with the area of the crash, due to traveling this route on previous occasions from his phone records. D- 1 also should have been familiar with V-1, due to the amount of time he spent driving this V-1, which was documented by different persons on different occasions. VEHICLE: V-2: V-2 was a black 2006 Toyota Solara convertible. V-1 was equipped with an automatic transmission, power-assisted steering and power-assisted front and rear disc brakes. V-2 was equipped with V liter engine and front wheel drive chassis. V-2 was Case Number FHP Page 8 : SiSS-4
21 INVESTIGATIVE REPORT equipped with standard occupant restraint devices which consisted of tap belts and shoulder harnesses. V-2 was also originally equipped with driver and front seat passenger air bags which did not deploy during the crash. V-2 had the assigned Florida tag number and V.i.N. number. OCCUPANT: V-2 DRIVER: (D-21: D-2 was Albert Dean Campbell, a male who was 61 years old at the time of the crash. D-2 resided D-1 had a valid Florida class E operator s license with no restrictions or endorsement. D-2 had presented his Florida Driver s License to the Traffic Crash Investigator, (Trooper Ricardo Reyes) and advised him he was the driver of V-2. There was one other occupant in V-2. D-2 was wearing his occupant restraint device at the time of the collision which consisted of a shoulder and lap belt. D-2 reported no injuries at the scene. D-2 was coming from Logan s Steakhouse restaurant on University Blvd., west of State Road 434 (Alafaya Trail) and was heading to his wife s work, which is located in the Goldenrod Plaza at Just prior to the area of the crash, V-2 was traveling northbound on State Road 551 in the outside lane making a right turn into the dual business entrance of Goldenrod Plaza and Kindercare Learning Center. D-2 was familiar with the route he was traveling and familiar with driving V-2. OCCUPANT: V-2 FRONT RIGHT (0-1): 0-1 was Kimberly Bailey Campbell, a female who was 53 years old at the time of the crash. 0-1 resided at She was seated in the front right seat of V-2. She was wearing Case Number FHP Page 9 :: Si
22 INVESTIGATIVE REPORT her occupant restraint device at the time of the collision which consisted of a lap belt and shoulder harness. She was not transported for her injuries; however, she received medical treatment on the following day from her physician, Doctor Son Nguyen. She was then referred by her physician to the Stand-Up MRI of Orlando injuries sustained in the crash. Her injuries consisted of pain to her lower back, neck pain, left shoulder, upper left arm and numbness in the left hand. PEDESTRIAN: P-1 P-1 was Lily Quintus, a female who was 4 years old at the time of the crash. P-1 had the address of Prior to the collision, she was occupying the room located in the northwest side of the building of Kindercare Learning Center. She was transported to Arnold Palmer Regional Medical Center, due to receiving serious bodily injuries. She was pronounced deceased by Dr. Mark Levy at 6:15 P.M. at Arnold Palmer Regional Medical Center. PEDESTRIAN: P-2 P-2 was June Decalzada, a female who was 3 years old at the time of the crash. P-2 had the address of Prior to the collision, she was occupying the room located in the northwest side of the building of Kindercare Learning Center.
