1 A GUIDE TO REHABILITATION SERVICES FOR THE PAN BIRMINGHAM CANCER NETWORK 1

2 Development of this directory complies with the requirements of both the NICE Guidelines for Supportive and Palliative Care, and the Improving Outcomes Guidance for Brain and CNS Tumours, and will aid in planning discharge and ongoing treatment as close to the patients home as possible. We aim to provide details of rehabilitation services appropriate particularly for patients with Brain and CNS Tumours, patients at all stages of the cancer pathway, and those with palliative care needs. The contents are therefore a mixture of generalist services which provide domiciliary or day patient intervention and more specialist treatment with in an in-patient facility. The guide is written to facilitate referral to appropriate rehabilitation following acute care, or from within the community at any stage of the patient s illness, in accordance with agreed cancer rehabilitation pathways. Clients may be working on managing their behaviour, community integration, independent living skills, vocational exploration or returning to work. 46
