1 2014 MSW Supplementary Information Application (FOR ALL PROGRAM OPTIONS) I. Applicants to the Jane Addams College of Social Work MSW Program are required to submit two online applications. UIC Online Application This application should be submitted first, as early as October 1, 2013 but no later than January 15, There is a two to five day delay from the time applicants submit their UIC Online Application and the time they can begin uploading the documents that are part of the MSW Supplementary Information Application. Persons submitting the UIC Online Application after January 15 may risk not having sufficient time to complete and upload MSW Supplementary Information Application documents or for referees to upload their reference letter. MSW Supplementary Information Application While applicants must wait two to five days to upload documents required by the MSW Supplementary Information Application they should be prepared to act quickly. For instance, applicants should upload the document stating who their referees are as soon as possible so that those individuals can upload their reference statement long before the 11:59 p.m. January 31, 2014 deadline.

All applicants should respond to each of the information requests. If a request does not apply, please write N/A. Not responding to an information request may either delay the evaluation of an application or results in an application being judged to be incomplete. 2

Persons graduating within the last six years from a CSWE accredited undergraduate social work program must apply to the Advanced Standing Program Option. Those individuals are not eligible for the Full-Time or the PM-Extended Study Program Option. 2. Persons transferring from another CSWE accredited MSW Program within the last four years must apply to the Transfer Program Option. Those applicants are ineligible for the Full-Time and PM-Extended Study Program Options. Those applicants may indicate, however, their interest in either the day schedule or the evening schedule. The approved schedule will depend upon the schedule for which applicants are found to be eligible. 3. Persons who graduated from an undergraduate social welfare or social work program not accredited by the Council on Social Work Education (CSWE) must apply to the Full-Time or PM-Extended Study Program Option. Those applicants are not eligible for the Advanced Standing Program Option. 4. International applicants or American citizens/ permanent resident applicants with foreign credentials must consult the international credential section of the College s website and respond to all appropriate requests. Go to for more information. 5. Please note that the minimum requirements must be met either at the time of application or by May 31 prior to enrollment to the College. 6. Applicants can satisfy minimum requirements with AP courses only if those courses are listed on the applicants official college transcripts or other documentation provided by undergraduate colleges. 7. Applicants whose undergraduate colleges do not require a specific writing course and use other ways to insure students meet minimum writing standards should submit with their application a written statement from the college or university that explains how the applicant has satisfied the writing requirements. 3

5 4. Social and Behavioral Science Courses a. Have you taken 20 or more semester hours or 30 quarter hours of social and behavioral science courses in which you receive the grade of C or higher? Was this accomplished by virtue of your major in Anthropology, Economics, Political Science, Psychology, or Sociology? If yes, no further information is required. If No, please list the specific social and behavioral science course work below in section c. b. If you have taken less than 20 semester hours or 30 quarter hours of social and behavioral science courses in which you earned the grade of C or higher, list those courses in section c below. c. Listing of specific social and behavioral sciences courses taken by persons not majoring in Anthropology, Economics, Political Science, Psychology, or Sociology: Course/School Academic Term Grade Credit Hours If No, will you have taken at least 20 semester hours or 30 quarter hours by this coming May 31, prior to enrollment in the Fall? 5. Grade Point Average Requirement Is your undergraduate GPA for the last 60 semester hours or 90 quarter hours 2.75 or higher? Your calculation: Please note: The review of an application will stop if the calculated GPA is less than 2.75 (on a 4 point scale) unless you do three things: Action #1: Prepare a written statement that explains why your GPA for the last 60 semester or 90 quarter hours is less than 2.75 and what steps you will take to insure that your graduate GPA will be 3:0 or higher. Statement should be one page or less. Action #2: Action #3: State your overall GPA, the GPA in your major, and the GPA in the social and behavioral science courses listed on page 4 if you are not an Anthropology, Economics, Political Science, Psychology, or Sociology major. Please do one of the following: 3a. Obtain a statement from a professor or academic advisor that addresses specifically your ability to do graduate level work. 3b. Prior to applying to the College of Social Work take at least two graduate level courses and demonstrate that you can earn an A or B in graduate level courses. That performance should be verified by an official transcript. 3c. After receiving your undergraduate degree and prior to applying to College of Social Work, take at least 12 hours of undergraduate courses in Anthropology, Economics, Political Science, Psychology, and/or Sociology to demonstrate you can earn A s and B s. 3d. Complete a certificate in an area relevant to social work to demonstrate your ability to do well academically in social work related courses. The submitted documentation will be reviewed at a meeting of the Admissions Committee. 5

6 Section B: FOUNDATION EQUIVALENT COURSES TAKEN BY ADVANCED STANDING APPLICANTS Introduction Persons who have graduated from a CSWE (Council on Social Work Education) accredited undergraduate social work program within the last six years are eligible for the Advanced Standing Program Option. Those individuals cannot apply to the Full-Time or PM Extended Study Program Options. Persons who graduate from a non-cswe accredited undergraduate social work program or from a non-cswe accredited track within an accredited social work program are not eligible for Advanced Standing. Applicants who are enrolled in an undergraduate social work program that is in candidacy for CSWE accreditation should advise the Director of Admissions and Financial Aid to learn to which program option they must apply. Advance Standing applicants must match correctly undergraduate courses roughly equivalent to each of the courses taken in the foundation year of the MSW Program at the Jane Addams College of Social Work. The grade of B or higher must be received for each of those equivalent courses. Advance Standing applicants who do not satisfy this requirement by the May 31 prior to the academic year for which they have applied will not be admitted into the College. In addition applicants who received the equivalent of a baccalaureate degree in Social Work from a foreign school must have their degree evaluated by CSWE s International social work Degree Recognition and Evaluation Services (SWDRES). Information about the service can be obtained at An application from that office can be obtained by ing iswdres@cswe.org. Finally, Advanced Standing applicants interested in working in elementary, middle, or secondary schools must submit with their application a copy of their test results indicating passing scores on the Test of Academic Proficiency (TAP). Applicants may substitute ACT Plus Writing test results if they have test scores of 22 or higher. 6

Research methodology basics applied to social work: problem formulation, design, measurement, sampling data analysis, computerization, ethics, and qualitative and quantitative methodologies. Students are assigned to asocial agencies where, under the supervision of an agency field instructor, selected micro and macrosystem cases are carried for social for social work services. 9 Section C: MSW COURSES TAKEN BY TRANSFER APPLICANTS Introduction Individuals who are currently attending or who have attended another CSWE (Council On Social Work Education) accredited MSW program within the last four years are eligible for the Transfer Program Option. Those individuals are not eligible for the Full-Time or PM Extended Program Options. In Parts One and Two, transfer applicants must list foundation year and other MSW courses they have taken at their initial MSW Program. They must match up correctly the foundation year courses taken with the foundation year courses that are taken at the Jane Addams College of Social Work. Working definitions of the College s foundation year courses may be found on page 8. The information on this sheet helps the Admissions Committee determine if an applicant will be able to satisfy minimum requirements in a timely manner. 13 Part Two: College s Policy and Practice 1. Students have the primary responsibility for financing their graduate education and should be prepared to do so if they do not receive financial support from the College or an external source. 2. Applicants should read the statement of financing graduate education found on the College Website 3. Persons admitted to the MSW Program should understand that the majority of financial support provided by the College is need based. Therefore, all admitted students will be asked to provide specific information that will enable the College to distribute need based awards as objectively as possible. 4. American citizens or permanent residents who are admitted to the MSW Program should submit their FAFSA (Free Application for Federal Student Aid) by the UIC priority deadline of March 1, if at all possible. International students, in their first year of study are expected to submit to the Office of International Student Services, a statement of guarantee that they have sufficient funds to cover all expenses related to their first year of graduate study 5. Applicants to the PM-Extended Study Program Option should note that much of the financial support provided by the College requires students to be registered in 12 or more semester hours. Consequently, there is limited financial support available through the College for PM Extended Study students during their first two years of study. 6. Applicants interested in financial support from the College must submit between April 15 and June 15 an application for financial support for MSW students. 14 Section F: References Please see application procedures on the College website: Section G: Resumé Please see application procedures on the College website: Section H: Test of Academic Proficiency (TAP) To practice as a school social worker in an elementary, middle, or secondary school in Illinois MSW students must obtain a Type 73 certification from the Illinois State Board of Education. The Type 73 certification requires MSW students to take and pass two sets of tests: Test of Academic Proficiency (TAP) Passing TAP test scores should be submitted by applicants themselves to the MSW Program as part of their MSW Supplementary Information Application. They should not request the Illinois Certification Testing System to do this for them. Individuals who are not able to submit their TAP test results prior to the January 31 application deadline may mail a hard copy of their test results. All test results must be received by March 15 by the College. Applicants who took the ACT Plus Writing test within the last ten years may submit that test in in lieu of the TAP if they have a composite test score of at least 22. In no case should applicants ask the Illinois Certification Testing System test scores on their behalf. Individuals must submit the test scores themselves. Persons applying to the Full-Time Program Option, PM-Extended Study Program Option, and Transfer Program Option applicants transferring in as foundation year students may be admitted conditionally into the School Concentration if they do not have passing TAP test results. They have until the end of the December of their foundation year to submit passing TAP or substitute composite ACT-Plus writing test scores of 22 or higher. Persons applying to the Advanced Standing Program Option must submit to the College Passing TAP or substitute ACT Plus writing test scores by March 15 at the latest in order to be admitted into the School Concentration. Transfer Program Option applicants must also submit passing TAP or substitute acceptable ACT Plus Writing test scores by March 15 if they wish to be enrolled in the School Concentration. Type 73 Certification Examination Once enrolled in the School Concentration and prior to receipt of their MSW students must take the Type 73 certification content examination. Upon passing that examination they can work in an elementary, middle, or secondary school as a school social worker.
