2 Dear Acosta-Rua Center Guest, Welcome to Community Hospice of Northeast Florida s Dr. Gaston J. Acosta-Rua Center for Caring, where we are committed to providing comprehensive, compassionate care to you and your loved ones. The Acosta-Rua Center opened in 2007 and is the namesake of Dr. Gaston J. Acosta- Rua, who was a Jacksonville neurosurgeon well known for his humanitarian work in the Jacksonville community, as well as for medical missionary service to the needy in Central America and West Africa for nearly 30 years. He died May 19, 2003 at age 65 of complications from pancreatic cancer. The Acosta-Rua family generously chose to memorialize Dr. Acosta-Rua through a combined family gift, believing that the significance of the gift is in Dr. Acosta-Rua s legacy of caring and community support. Today, the Acosta-Rua Center is fully staffed with a team of medical professionals 24 hours a day. It is one of four Community Hospice inpatient care centers in Jacksonville, along with the Earl B. Hadlow Center for Caring in Mandarin, the George and Margaret Morris Center for Caring at the Shands Jacksonville Pavilion, and the Anne and Donald McGraw Center for Caring at the Beaches. As you continue on your hospice journey, the Acosta-Rua Center staff is always available to provide you support and guidance. We are honored to accept the responsibility of caring for you and your loved ones at this time, and we take this responsibility very seriously. We will work with you in coordinating your plan of care, to answer questions or to just spend time together and discuss how we may best support you. 4 DR. GASTON J. ACOSTA-RUA CENTER FOR CARING PATIENT INFORMATION Community Hospice's Dr. Gaston J. Acosta-Rua Center for Caring is a 24-hour inpatient care center for the shortterm needs of patients and their families. The goal of hospice inpatient care is to return the patient to the place they call "home." VISITORS We encourage the families and friends of our residents to visit anytime. This is your loved one's home, and we want you to be comfortable here. If you wish to spend the night in your loved one's room, foldout chairs are in all the rooms, and foldaway beds are available on a first-come, first-served basis. All visitors must enter and exit the Acosta-Rua Center through the main lobby. Each time you visit, you will be asked to sign in at the reception desk and obtain a visitor badge. The visitor badge must be placed on your clothing to identify you to our staff as a guest and allow you access to all of the residential areas of the Acosta-Rua Center. Visitor badges should be returned to the reception desk as you sign out. During your visit, please be respectful of other patients, families and visitors. FLAGS FOR VETERANS The military flags on display represent branches of the military and are hung to recognize and honor individual veterans who are currently receiving care in our inpatient care centers. Thanks to our founding sponsors of the "Flags for Veterans" program, Rolling Thunder Florida Chapter 4, and Allied Veterans of the World, Inc. and Affilliates, for the gift of these flags. For more information about the "Flags for Veterans" program, call Toula Wootan, community development officer, at TELEPHONES Telephones are available in each patient's room, and a courtesy telephone is available in each family gathering area. Outgoing calls can be made by pressing 9 for a dial tone, or you may make calls to internal Community Hospice employees by direct-dialing their extensions. Long-distance calls may be made from a patient's room by using a personal calling card or by calling collect. To call a patient's room from outside the Acosta-Rua Center, dial and follow the voice prompts. Outside callers wanting to contact the nurses station may dial INTERNET ACCESS Each room is set up with Internet access. Please ask your nurse for assistance. MEALS Patients' meals are planned and prepared by our Community Hospice chefs. A menu will be provided each day for patients to select the meal of their choice for the following day. Our chefs are available to discuss preferences and food allergies. DRINKS/SNACKS A family kitchen is located near the main nurses station at the entrance of the south patient wing. Coffee, drink and snack machines, a microwave, refrigerator and ice are all available for your convenience.

5 IT S YOUR COMMUNITY HOSPICE PROFESSIONAL SERVICES Social services are available to all patients and their families. In addition, Community Hospice chaplains are on hand to provide spiritual guidance and support. They can also assist you with contacting clergy from your faith community. The Acosta-Rua Center's chapel is immediately to your left as you enter the main lobby and is open for prayer and meditation at all times. Depending on availability, the chapel may be used for memorial services if desired. Please contact your chaplin or psychosocial specialist for more information. Please notify your Acosta-Rua Center nurse to request a chaplin, psychosocial specialist, or volunteer for any other assistance. HOUSEKEEPING The Acosta-Rua Center is cleaned daily. Each day, the patient's room is cleaned and sanitized. However, if you require housekeeping assistance, please notify the nursing staff immediately. A washer and dryer are available for our patients' use. Please see the nurses station for information and assistance. RESTROOMS Each patient s room has a private restroom. Public restrooms are located to the right of the main corridor toward the dining room. Please refer to the floorplan in this booklet for exact locations. PETS Pets belonging to patients are allowed to briefly visit their owners in their rooms but are not allowed to stay overnight. VOLUNTEER SERVICES Patient/family volunteers who have attended an 18-hour training course are available to provide companionship, respite and emotional support, as well as one-time errands for patients and their families. Specially trained complementary care volunteers can provide services such as hand massage, progressive relaxation techniques and music therapy. A volunteer pet therapy program includes dogs and cats that visit patients upon request. For more information or any additional questions you may have, please contact your Community Hospice nurse.

6 DR. GASTON J. ACOSTA-RUA CENTER FOR CARING SAFETY INFORMATION Your safety as well as your comfort are important to us. For your convenience, the Acosta-Rua Center's front entrance is open daily from 6:30 a.m. until 9 p.m. Guests arriving between the hours of 9 p.m. and 6:30 a.m. must ring the intercom to the left of the front doors. The security guard on duty will notify the nurse, check in guests, issue visitor badges and escort visitors to the room. SECURITY The Acosta-Rua Center is staffed with continuous security guard service. The security guard staff provides courtesy escorts to your vehicle during evening hours. For an escort, please notify your nurse. All visitors must enter and exit the Acosta-Rua Center through the main lobby. Each time you visit, you will be asked to sign in at the reception desk and obtain a visitor badge. SMOKING To provide a safe environment for patients, visitors and staff, the Acosta-Rua Center is a smoke-free building. Smoking is permitted in these designated smoking areas: Back porches/walkway Front entrance FIRE DRILLS For the safety of our patients, staff and volunteers, fire drills are conducted regularly at Community Hospice. Patients and their visitors do not usually participate in fire drills. However, during a drill alarm bells will sound, the strobe lights will flash and the fire doors will close. Please remain calm during the drill, and remain in your room unless requested to do otherwise. VALUABLES Valuables such as jewelry, credit cards or large sums of money should remain at home. Community Hospice cannot be held responsible for any loss of valuables brought into the Acosta-Rua Center by patients or visitors. Established in 1979 as the first hospice program in Northeast Florida, Community Hospice of Northeast Florida helps nearly 900 patients live better with advanced illness each day in Baker, Clay, Duval, Nassau and St. Johns counties, through the support of approximately 700 employees and 900 volunteers. The nonprofit organization has served more than 53,000 children and adults at home, in long term care facilities, hospitals and through four inpatient care centers throughout Jacksonville. Community Hospice Foundation generates philanthropic and community support for patient care services, family needs and community programs such as Community PedsCare, a pediatric palliative and hospice program.
