1 NARI Dallas Web Site Program All NARI Greater Dallas members receive a free white page listing in our database, which includes your company name, address, phone number, fax number, contact person and listing of certified remodelers in your company. Furthermore, consumers can search for your company using one of four search engines found on our Web site. There is an alpha (by company name & by contact last name), zip code and category search. You will automatically be listed in the alpha search (by company name & contact last name) and in the zip code search (by your company zip code). To receive your free listing, you do not need to do anything, you will automatically be listed. If you would like to expand your listing to include 25 words; your company logo; and/or a hyper link to your already existing company Web site; please fill out the form below. You will also need to fill out the form below if you would like to sponsor the NARI Dallas Web site; be included in the category search; or if you would like to expand the number of zip codes that you are currently listed under. To include a copy of your logo, please it in.gif or.jpg format to: [email protected]. Your logo will have to fit into an 82 X 82 pixel square. If you need help shrinking your logo to fit this space we will be happy to help. Please return your completed application to the address below. 2 Please choose either Option 1 OR Option 2 (not both) and Option 3 and or 4 (if desired): Option 1: A basic white page listing (name & contact info) with 25 words of text highlighting your company. Plus, you may select up to 15 zip codes (a zip code can be 2 to 5 numbers, i.e. 752** would pull up all zip codes starting with 752 but it only counts as one of your 15 zip codes) that you would like your name and information listed under in the "Find a NARI Pro" section of the naridallas.org Web site. Your company logo will also be displayed and you may choose three free categories to be listed in. Cost: $ per year Option 2: A hyper link to your already existing company Web site from your listing in the NARI Greater Dallas on-line database. When you choose this option, you receive 25 words or less of text, 15 zip codes, your logo and three category inclusions free of charge. In addition, your will appear as a link. Cost: $ per year Option 3: Please choose the additional categories that you would like to be listed under from the list on the following page. This is only available as an add-on to Option 1 or 2. 4 Sponsorship Option Sponsorship Option: Members also have the option to sponsor the NARI Dallas Web site. A Web site sponsorship would include a hyperlink to the sponsor s Web site using a copy of the company s logo as a hot button. If a logo is not available, then the company s name can be used. The company s name would be placed in bold letters and underlined, and the name would act as the hot button to the sponsor s Web site. The logo or the name is to occupy an approximately 100 X 100 pixel area on the NARI Dallas Web site. 5 PARTICIPATION BY NARI GREATER DALLAS MEMBERS A. NARI Membership Each PARTICIPANT must be a member of the National Association of the Remodeling Industry Greater Dallas Chapter and be in good standing with dues current in order to participate in the NARI Greater Dallas Web site program. B. Membership Dues Current Should a PARTICIPANT S membership dues not be paid to NARI within thirty (30) days after their renewal date, then the PARTICIPANT will be dropped from further participation on the NARI Greater Dallas Web site. Their name will be removed from the database of members until such time as their dues are paid and current. No refunds or credits for time lapsed will be made to members for the time that their name and information (including Web pages and links) are removed from the site due to non-current dues. C. Cost to Participate There is no cost to NARI Greater Dallas members for a white page listing as well as inclusion in the zip code search (by your company s ZIP code only). However, there are additional options that each company can choose from. The costs associated with each of these options are explained above. D. Payment Method Payments for all levels of participation must be received in advance with a completed Application for Participation. Payments by PARTICIPANT can be made by company check or by credit card. A returned check fee of $25.00 will be assessed for all returned checks. E. Refunds There will be no refunds given to NARI Greater Dallas members by NARI Greater Dallas or PCA Consulting, Inc. under any circumstances, including, but not limited to the following reasons: a NARI Greater Dallas member s membership is not renewed for any reason (including, but not limited to, non-payment of dues or for conduct unbecoming a NARI member); a NARI Greater Dallas member moves out of the Dallas area or stops working in the home improvement industry; or the NARI Greater Dallas terminates the program. TERMS OF AGREEMENT A. Terms of Agreement The terms of this agreement will be for a 1-year (12 consecutive months) period of time. The provisions for participation, fees and charges may be changed by NARI Greater Dallas, at its discretion, within thirty (30) days of written notification to the PARTICIPANT. B. Beginning date of program Each PARTICIPANT S 25 words or less of text and/or hyper link will be placed on the Web site within two weeks following the receipt of a completed/signed application and payment in full. Your one-year agreement will commence on the day that your Web page link and/or 25 words or less appears on the NARI Greater Dallas Web site. You will receive a copy of the contract signed and dated by NARI Greater Dallas once your application has been processed. The date the application is signed by NARI Greater Dallas is the exact start date of your contract. Page 5 of 7

6 C. Termination of program NARI Greater Dallas, at its sole direction, may terminate the Web site program at any time upon thirty (30) days written notice to PARTICIPANT. OTHER OBLIGATIONS AND COMMITMENTS A. Indemnity PARTICIPANTS shall indemnify and hold harmless NARI Greater Dallas and PCA Consulting, Inc., their agents and employees, from and against any and all claims by or on behalf of any person or persons, firm or firms, corporations of corporations, for damages or injury to person or property arising from and on account of the management, organization and operation of the NARI Greater Dallas Web site. PARTICIPANT, its agents and employees will further indemnify and save NARI Greater Dallas and PCA Consulting, Inc. harmless from or on account of any act of negligence, misrepresentation or any other tort by PARTICIPANT, its agents, contractors servants, employees, and from and against all costs, including reasonable counsel fees, expenses and liabilities incurred in or about any such claim or action or processing brought thereon; and in case any action or proceeding be brought against PCA Consulting, Inc. or NARI Greater Dallas by reason of any such claim. B. Warranty NARI Greater Dallas and PCA Consulting, Inc. make no warranty, either expressed or implied or any representation whatsoever. Page 6 of 7
