2 Vision and Mission Vision: An integrated, secure and forward-looking information management system that will facilitate the provision of timely, patient-centered and high quality healthcare across the continuum of services, the management of emergency and other situations important to the public s health, and continuous quality improvement of healthcare and public health processes and outcomes. Mission: LANES seeks to improve healthcare delivery in Los Angeles County and surrounding areas by ensuring that health information is available when and where it is needed in a safe and secure manner.

3 LANES A public / private collaborative whose Board is comprised of: Community Clinic Association of Los Angele County County of Los Angeles Health-e-LA Hospital Association of Southern California LA Care Health Plan Efforts underway to fill the four remaining seats: a physician group; a commercial health plan; a health advocacy group; and an independent organization. The LANES Board selected Public Health Foundation Enterprises (PHFE) as the LANES Fiscal Intermediary and Western Health Information Network (WHIN) as the LANES Technology Partner. A key component of the LANES Initiative is to develop a Health Information Exchange.

4 WHIN Role of WHIN: Allow health information to be available when and where it is needed for patient care and that this information is safe and secure. Be the foundation for more timely, patient-centered and high quality care in the greater Los Angeles area. Enable coordination of care and improve chronic disease management. Be a single, inclusive and comprehensive solution to be incrementally deployed.

5 WHIN: A Community Organization Community Support Data Participation Agreement Medical Home Public Service Founded as the Long Beach Network for Health (LBNH) in 2003, Western Health Information Network (WHIN) is a community organization dedicated to improving health outcomes by having the best information available at the point of care.
