2 Who we are Meridian Health is a leading not-for-profit healthcare organization in Neptune, New Jersey, comprising six hospitals and over 1,700 acute care beds and 725 post acute care beds. 95 locations in central and coastal NJ, $1.6 billion annual revenues, 12,000 team members, 2,400 volunteers, over 2,000 affiliated Physicians. Network includes: physician practices, Home Care agencies, Hospice and Rehabilitation facilities, ambulatory care facilities; occupational health centers; rehabilitation and fitness centers; ambulance and medical transport: five behavioral health centers Named one of HealthCare's most wired in U.S. 11 years in a row Named one of Fortune s 100 Best Companies to Work For for the past two years.

5 Meridian Health Phase 1 Quickly integrate its six hospitals and SNF/LTC facilities in two central New Jersey counties. Integrating facilities to improve patient outcomes and reduce costs Initial pilot involved 5 hospitals and 10 physicians in two counties One month into program numerous physicians were regularly using HIE with over 60 more requesting data exchange with their EMRs HIE successfully integrates multiple systems, including Siemens, MEDITECH, Sunquest, MEDHOST, T-Systems, EDIMS, and others, into its broad information platform All six hospitals and 4 long-term facilities are now live.

6 Today Phase I implementation of the CareAlign Suite included the following modules: Secure web portal and patient chart access Distribution and display of continuity of care (CCD) documents among all physician practices Patient lists, reminders and alerts In-patient flow sheets Patient data aggregation Record locator service and patient matching API Integration Engine Workflow Tools Results management and notification Secure messaging

12 RESULTS/BENEFITS Meridian Health Phase one implementation completed in March 2011 and already in use by many physicians throughout the hospitals and practices Additional physician participation and usage increasing at a rapid and steady rate Physicians seeing the benefits of the solution as everyday clinical tasks in the office are made easier Physicians are able to re-check patient results quickly and easily See a comprehensive patient view of new patients prior to the visit These functions were previously not easily available Physicians accessing Meridian HIE (ICA CareAlign) from their offices find it easy to use Requires minimal training due to the highly intuitive nature of the solution

13 In process and Future Future deployment of CareAlign s EMR Lite will be available to all medical staff participants - will include prescribing and other meaningful use features and functions (Hosted) Phase II of the CareAlign implementation includes inbound and outbound CCDs Integration with retail pharmacy data Clinical/Disease dashboards, rules and protocols across the continuum.
