Deductible - a fixed monetary amount paid by the insured member for any defined benefit received under your policy Network health service provider: a provider of healthcare services in an agreement with Daman, to provide medical services i.e. hospitals, single and multi-specialty clinics, pharmacies, diagnostic centres etc. How To Obtain Your Medical Benefits How to use your Daman Card Present your Daman health insurance card at the network health service provider s reception. 5 As a Daman card holder, there is no need to pay for covered services obtained from the health service provider within Daman s network, except for those offered on reimbursement basis in case of emergency (deductible would be applicable as mentioned on the card). The network health service provider should inform you in advance in case there are deductibles or if any of the prescribed procedures/medical treatment is not covered by your health insurance plan. You will be asked to settle co--insurance/deductibles (mentioned on your card) either before or after the treatment (if applicable). The network health service provider will provide you with all the necessary information, if needed. Pre-Authorisation A pre-authorisation is a prior approval or official permission, given by Daman to the network health service provider, before certain procedures can be performed. This approval is required to support your network health service provider s choice of medical procedures and avoid any unnecessary procedures, hospital stays, or surgeries. Your network health service provider will inform you if a requested/prescribed procedure requires pre-authorisation. To facilitate and expedite the pre-authorisation process, always verify with the network health service provider whether the planned services are covered under your Daman health insurance plan. The network health service provider, along with Daman, will carry out the pre-authorisation process. Once the authorisation is granted, the network health service provider will notify you. In case the authorisation is not granted, then you can always request for an explanation. There is always a possibility that some services might not be covered, even if they have already been performed or prescribed (unless they were authorised prior to being performed, for example angiography, radiotherapy, etc.). When is Pre-Authorisation Required? Pre-Authorisation is required for all inpatient (staying in the hospital) and day treatment services. 6 Dental Treatments (as specified in the Schedule of Benefits) Any other benefits as specified in the Schedule of Benefits Emergency cases that are followed by a hospital admission do not require pre-authorisation from Daman but have to be notified by the network health service provider to Daman within 24 hours (or 48 hours in case of Premier TC plans). Reimbursement In case of treatments that have been availed outside Daman s network of health service providers, you will have to pay for the services provided. However, if such services are covered in your plan, you can apply for reimbursement. Reimbursing claims for covered procedures that are availed at a non-network health service provider: All reimbursement claims, for medical services availed in and/or outside the UAE, should be reported to Daman within 120 days (180 days in case of Premier TC plans) from the date of occurrence, and should be submitted with the following required documents: - Original itemised invoices with dates - Medical Report/Discharge Summary signed and stamped by the treating doctor (for inpatient services only) - Original drug prescription with a detailed pharmacy bill (itemised) - Diagnostic (lab/radiology) investigation reports and invoices (if any), report of the results only if single investigation cost is more than AED 1,000 - Copy of Daman card - Copy of personal identification card (e.g. Emirates ID, passport copy) Reimbursement claims submission can be applied online through Daman s web portal ( or through Daman s interfaces, either in branches or service points. Reimbursement forms are available on Daman s website. Reimbursement of appropriately documented claims (as per the list of documents above) will be settled within 15 working days from the date of receipt of the claim.
