3 What is a hysterosalpingogram (HSG)? An HSG is an x-ray examination to determine whether the uterus (womb) and the fallopian tubes are normal. It may reveal abnormalities including: Tube blockage Distortion of the uterus by fibroids or polyps Congenital or other physical abnormalities It is a useful routine investigation for some women with low fertility. It may show fallopian tube changes due to pelvic inflammatory disease. It may also be used as part of the assessment of the causes of recurrent miscarriage. An HSG cannot be done if: There is active Chlamydia infection. This can be tested for by taking a blood or urine sample. The Fertility Centre will have arranged this and will contact you if the test is positive. You are pregnant. To minimise the chance of pregnancy the examination is usually performed immediately after the end of a period (around days 5-10), but within the first 10 days of a menstrual cycle. What if I have irregular or infrequent cycles, am unable to attend for my appointment, or I have any other questions? If your periods are very erratic, the standard appointment within 10 days of your last period is not convenient, or you have any questions please phone the X Ray and Scanning department on (023) ext 4110 (Mon Fri 9:00 17:00) as soon as possible for further booking advice or to arrange another time. When will I get an appointment? We aim to perform the test within 3 months of the referral. 3

4 Preparation for the HSG. Please come with your bladder almost full - this helps to straighten the uterus for the test. Where should I come for the test? Please come to: X Ray and Scanning, North Entrance, C Level, Queen Alexandra Hospital. On arrival please report to reception and you will be directed to the fluoroscopy waiting area. You will be asked to undress and put a hospital gown on. Please feel free to bring your own dressing gown if you prefer. What will happen during the HSG test? You will be asked the date of the first day of your most recent period and to confirm you have stopped bleeding. We will ask you to sign a disclaimer to confirm that you are not pregnant if the test is taking place after day 10 of your menstrual cycle. We will help you lie correctly on the x-ray table and a speculum* will be introduced into the vagina to allow inspection and cleaning of the cervix. (*Instrument commonly used for internal examinations in women.) A small tube will be inserted into the lower part of the uterus and the speculum usually then removed. Some special fluid will be injected into the tube and images recorded of the fluid entering the uterus and tubes. Who will I see? You will be seen by a radiologist (a doctor who specialises in x-ray) and a specialist radiographer or radiographic assistant. How long does the HSG take? The actual test usually takes only about 5 minutes but time for changing and talking to the radiologist before and after the test adds up about 30 minutes in total. 4

5 Will it hurt? It is common to feel discomfort at some stage of the test, and this is usually similar to a period pain. You may wish to take some paracetamol before you come for the test, as this can help. Are there any common side effects? Vaginal discharge or bleeding are not uncommon after the test, but usually eases by the second or third day. Some pain is a common symptom, both during and after the test. This is usually similar to a period pain; it should be mild and settle spontaneously or with a simple painkiller such as paracetamol. Procedure failure: Unfortunately, there is always the possibility that the radiologist may not be able to complete the procedure. If this happens the radiologist may suggest repeating it at a later date, or your fertility doctor may organise an alternative investigation. What happens after the HSG? We may be able to tell you the result on the day, but sometimes more time is needed to check the pictures. Some people feel a little shaky after the test and we will ask you to wait a short while to make sure you feel well. You might like to bring someone with you so that they can accompany you home. You will have a follow up appointment at the Fertility Centre to discuss the result. What should I do and look out for once I am at home? Take it gently for the rest of the day. We advise you abstain from intercourse (sex) for a few days after the test, and also from swimming in public places. You should continue to take pregnancy precautions for the rest of the same menstrual cycle. 5

6 Take paracetamol if you have any pain. We advise you do not take aspirin for 24 hours following your procedure since this might prolong any bleeding that might occur. Please contact your GP or the Fertility Centre if you have any of the following: Prolonged bleeding or discharge after 3 days. Pain that is severe or does not settle. An offensive discharge or a high fever. What are the risks? Radiation: An HSG is performed under x ray guidance. Radiation exposure will be kept as low as possible. Allergies: We cannot perform the test if you are allergic to iodine. We need to know if you have a latex allergy. Infection: Rarely an internal infection can result. If you notice an offensive vaginal discharge or develop a fever please contact your GP or the Fertility Centre straightaway. 6

8 Consent What does this mean? Before any doctor, nurse or therapist examines you they must have your consent or permission. Consent ranges from allowing a doctor to take your blood pressure, (rolling up your sleeve and presenting your arm is implied consent), to signing a form to say you agree to the treatment or operation. It is important before giving permission that you understand what you are agreeing to. If you do not understand please ask. More detailed information is available on request.
