1 ADDENDUM #1 RELEASED SEPTEMBER 5, 2014 GYPSUM TO DOTSERO TRAIL BASE AND PAVING PROJECT Bidders shall acknowledge receipt of this Addendum #1 on the Bid Form. Failure to do so may render the Bidder s bid non-responsive. The following changes, additions, or deletions shall be made to the Bidding Documents and all other items shall remain the same. Question: What is Base and Paving testing frequency and type? See the attached revised Exhibit C Notes and Specifications. Question: Is item 6.31 on Pg. 47 of General Conditions to be deleted? Yes. Question: Is this a unit price contract? This is a combined Unit Price and Lump Sum Contract as specified in Exhibit A. Unit Price Quantities have been estimated with the standard Road and Bridge Department formula for all base and paving work. Quantities are amended by this Addendum to reflect revisions to driveway work and an error regarding shoulder base course quantity. Question: Is there a traffic control CDOT specification? See attached revised Exhibit C Notes and Specifications. Question: Are you going to add a mobilization bid item? No. Question: No permits or fees required by the Contractor? Correct. Eagle County holds the CDOT permit issued for the Gypsum to Dotsero Trail and as stated in the bidding documents, Contractor to review and adhere to the CDOT permit conditions attached to the Bid Documents as Exhibit E. Comment: Certified Weed Free Top Soil is not available locally. Top Soil provided for the work shall either be certified Weed Free or treated with an herbicide after placement and before native seeding. Herbicide treatment shall be performed by a licensed applicator and proposed treatment is subject to review and approval by Eagle County Weed Control department. 1

2 Clarification: Regarding Sterilant Sterilant will be applied to trail subgrade and shoulder area only by Eagle County Weed Control personnel following completion of the trail platform by Eagle County Road and Bridge and prior to the scheduled base and paving date. Revised Exhibit A Bid Form: The form has been revised to reflect quantity revisions as noted above. Revised Exhibit C Notes and Specifications: See following page for strikeouts as removed information and highlights for added or revised information. Driveways: Supplemental driveway exhibits are attached under separate cover and are part of this Addendum #1. 2

4 EXHIBIT C NOTES AND SPECIFICATIONS GYPSUM TO DOTSERO TRAIL BASE AND PAVING PROJECT 1. Project Purpose: Project purpose is to provide a 10 foot wide public trail for recreation and transportation on the south side of Highway 6 between Town of Gypsum and Dotsero as part of the Eagle Valley Trail. 2. Description of Work: a. Contractor will install three inches (3 ) of bituminous asphalt ten feet wide on a 13 foot wide four inch (4 ) deep aggregate base course platform compacted to the specifications cited below. The work will also include a compacted one foot (ABC) Class 6 plus base course shoulder on each side of the asphalt trails. b. The base and paving work will be installed on the trail subgrade currently under construction by the Eagle County Road and Bridge Department. Road and Bridge and ECO Trails will assist Contractor with base and paving layout. c. The compacted subgrade trail platform will be ready for paving as of September 29, Per the Bidding Documents, completion date is November 15. Reasonable extensions for base, paving and topsoil placement are possible subject to approval by Owner, CDOT and acceptable weather conditions. d. The base and paving project ( project ) consists of two parts, separated by approximately one mile, as shown on the attached Exhibit B map. e. Access and staging is available at the Duck Pond Eagle County Open Space property and BLM Horse Pasture river access, subject to review and approval by Owner. 3. Schedule of Work: a. Contractor to schedule pre-base and pave meeting with Owner for one calendar week prior to planned dates for commencement of the work. b. Contractor provide traffic control plan one calendar week prior to planned dates for commencement of the work. b. Contractor to confirm dates of work 48 hours prior to commencement of the work. c. Hours of construction shall be limited to Monday through Friday 7:30 a.m. to 6:00 p.m. No work will be allowed at night, Saturdays, Sundays or legal holidays without prior authorization from the Owner, in compliance with the conditions of project permits. 4

5 d. Per the CDOT permit issued for the project, no work shall be permitted on Highway 6 on Colorado State Holidays Monday, October 13 and Tuesday, November 11. Work can continue on non- CDOT property (County and BLM) if no traffic control impacts to Highway 6. Owner will assist Contractor with request for holiday work authorization if weather delays. 4. Specifications: a. All materials, equipment, installation and construction within the state highway right of way shall be in accordance with the following standard references as applicable: 1. CDOT Field Material Manual 2. CDOT Construction Manual 3. CDOT Standard Specifications for Road and Bridge Construction 4. CDOT M&S Standards 5. FHWA Manual on Uniform Traffic Control Devices 6. AASHTO Roadside Design Guide b. The base and paving trail cross section is as follows: 1. 4 of ABC will be installed as 13 feet wide of asphalt will be installed as 10 feet (10 ) wide 3. An additional 3 depth of ABC will be installed and compacted after asphalt installation as a one foot (1 ) shoulder compacted shoulder flush with the trail asphalt, on each side. c. The Contractor is not responsible for subgrade compaction or repair of soft areas. Contractor will not unduly disturb pre-pave compacted subgrade in preparation for the work. Contractor will participate in observing proof roll of subgrade by Road and Bridge at pre-base and pave meeting. d. Ingress and Egress access is open throughout the project, with exception of between Driveway #1 (Giordano) east to existing block wall is one-way access only. e. Contractor shall match or transition work to existing trail and driveway asphalt grades within project limits. e. Driveway Details and Pavement Scope of Work Quantities: #1 Giordano Residential: a. Currently paved b. Contractor to crackseal driveway surface under planned trail and apron location c. Contractor to overlay trail location and 4 foot x 28 foot wide aprons on north and south side of trail with ½ asphalt and transition as needed to existing driveway grades d. Quantities for new base and asphalt this driveway to serve as trail deleted from project quantities e. Estimated 1 TN of asphalt added to bid quantity 5

6 f. Crackseal quantity is incidental to cost of the work. #2 Duck Pond OS East a. Currently unpaved b. Driveway subgrade and excavation prep by Road and Bridge c. Contractor to add road base and asphalt aprons to each side of trail as follows d. 6 feet on south side of trail, measured from edge of trail x 15 foot width e. 6 feet on south side of side of trail, measured from edge of trail x 15 foot width f. Contractor to crackseal driveway surface under planned trail location plus 20 feet to Hwy 6 connection and 6 feet to south c. Contractor to overlay commensurate area with ½ asphalt and transition as needed to existing driveway grades d. Quantities for new base and asphalt this driveway to serve as trail deleted from project quantities e. Estimated 4.5TN of asphalt added to bid quantity f. Crackseal quantity is incidental to cost of the work. #6 BLM Lava Site a. Currently unpaved b. Driveway subgrade and excavation prep by Road and Bridge c. Contractor to add road base and asphalt aprons to each side of trail as follows d. 10 feet on south side of trail, measured from edge of trail x 15 foot width e. feet north side of trail, measured from edge of trail x 15 foot width f. Include Curve radius to tie into existing Highway 6 asphalt g. Estimated quantities of 9 TN base and 7 TN asphalt added to bid quantity 6

7 f. Driveway Details and Pavement Scope of Work Quantities: 1. Project includes 6 locations where trail will cross driveways. 3 driveways are currently paved, 3 are unpaved. 2. Eagle County Road and Bridge Department (R&B) will prepare all 6 driveways as follows: a. 3 paved driveways will be sawcut 12 x 12 dimension and asphalt removed prepavement. b. At all driveways, road base will be installed and compacted by R&B to required elevation to accommodate 3 of asphalt to be installed by Contractor. Owner to provide road base for this phase. c. R&B will install temporary lightly-compacted road base depth as necessary (+/-3 ) to maintain existing driveway grades to allow safe driveway use pre-paving. d. This work will be performed as close as practically possible to Contractor s start of work date. Driveways to be reviewed at pre-pave meeting. 3. Contractor to remove temporary lightly-compacted road base at all driveways as necessary to accommodate paving. Road base material may be used for shouldering or miscellaneous leveling as needed. 4. Overlay tack is incidental to the cost of the work. 5. Treatment of specific driveways: a. Attached illustrations are intended to generally depict treatment at each of six driveways. Illustrations are not to scale. b. Additional asphalt for extra two feet of trail width at sawcuts, aprons and overlays are included in the bid form quantities. c. Driveway #1 1. Install additional asphalt 1 foot x 26 feet each side of 10 foot trail at 3 deep to pave/fill sawcut area. 2. Install 2 foot x 26 foot apron on south side of trail measured from south side of sawcut, blend and transition to existing driveway grade. 3. Overlay driveway from north edge of trail sawcut area at a depth of 1.5 or less to meet edge of Highway 6 asphalt shoulder. Estimated overlay area is 1,500 feet. 7

8 d. Driveway #2 1. Install asphalt 4 foot x 15 foot x 3 deep apron on each side of trail and transition to existing unpaved driveway grade. e. Driveway #3 1. Install additional asphalt 1 foot x 17 feet each side of 10 foot trail at 3 deep to pave/fill sawcut area. 2. Install 4 foot x 17 foot x 1.5 deep apron on each side of trail and transition to existing paved driveway. f. Driveway #4 1. Install 6 foot x 26 foot x 3 deep aprons each side of trail and transition to existing unpaved driveway grade. g. Driveway #5 1. Install additional asphalt 1 foot x 32 feet each side of 10 foot trail at 3 deep to pave/fill sawcut area. 2. Install 6 foot x 32 foot x 1.5 apron on south side of trail measured from south side of sawcut, to blend and transition to existing driveway grade. 3. Overlay driveway from north edge of trail sawcut area at a depth of 1.5 or less to transition to edge of Highway 6 asphalt shoulder. Estimated overlay area is 1,700 square feet. h. Driveway #6 1. R&B to sawcut clean edge of existing driveway apron from Hwy 6 2. Install 15 feet x 17 x 3 from north edge of trail to meet existing driveway apron. 3. Install 15 feet x 4 feet x 3 apron from south edge of trail and transition to existing unpaved driveway grade. g. Base and asphalt shall maintain subgrade drainage cross slope of 2%. h. Fine grading of topsoil in preparation for seeding (by Owner) is incidental to topsoil placement. i. All staging areas shall be restored to same or better condition prior to the work, following the demobilization of base and paving equipment. j. At or prior to pre-base and pave meeting, Contractor shall submit mix design for Hot Bituminous Pavement and provide certification that Class 6 meets CDOT specifications. 8

9 k. Materials test results shall be provided to Owner. Contractor shall be responsible for providing all required traffic control devices necessary to complete the work in accordance with the guidelines set forth within the Manual on Uniform Traffic Control Devices, with latest revisions and the CDOT M&S Standards. b. The Contractor shall not have construction equipment or materials in the lane or shoulders of the highway at any time, unless approved or directed. c. Contractor employee vehicle parking is prohibited where it conflicts with safety, access or flow of traffic. No Contractor employee parking will be allowed within the clear zone unless approved by the Owner or CDOT. d. Proposed lane closures, though unanticipated as part of the work, are subject to review and approval by CDOT. The planned work does not take place on the highway paved travel lanes or shoulders. 1. Two weeks prior to beginning work, Contractor will submit a Traffic Control Plan for review by Owner and Colorado Department of Transportation (CDOT). The TCP will cover all phases of the work. 2. The Traffic Control Plan advanced warning and construction signs, flashers, barricades and flag people must conform to the Manual on Uniform Traffic Control Devices (MUTCD). 3. Any advance warning signs not in use for a particular activity shall be removed, placed outside of the clear zone or laid flat at least 4 from the edge of the shoulder. 4. All flagging personnel must be certified and their certification cards available for inspection at any time. 5. Lane closures must be as short as possible and per the MUTCD. Flaggers are required for each lane closure. 6. The TCP shall insure that closures of access points e.g. driveways is minimized. 9

10 7. Contractor will have a competent person at the work site at all times in responsible charge of the temporary traffic control. As this project is not expected to require beyond the Typical Application shown in the MUTCD and is not located on a roadway with particularly dangerous traffic conditions, a Traffic Control Supervisor is not required. 8. Two-way traffic shall be maintained at all times in the construction area in accordance with the MUTCD. 9. The Contractor shall not have construction equipment or materials in the lanes or the shoulders of the highway at any time, unless approved or directed. 10. Workers, Contractor, Suppliers, etc. shall not access the work area by crossing the roadways unless proper traffic control or other necessary precautions are provided. 11. Employee vehicle parking is prohibited where it conflicts with safety, access or flow of traffic. No employee parking will be allowed within the clear zone. Adequate employee vehicle parking is available in the BLM and County staging areas. 12. Dirt, mud or debris tracked onto the highway by the work activity will be removed daily by Contractor. 13. Activities within the highway must be shut down when pavement is wet and hazardous for the traveling public. 14. To meet conditions encountered in the field, minor changes or additions may be ordered and approved by CDOT. 15. Contractor shall keep a copy on site at all times of the CDOT permit for this work issued to Eagle County and provided in the Bidding Documents. 16. All costs incidental to the foregoing requirements shall be included in the traffic control lump sum contract price for the project. 6. Permits a. The Contractor shall conform to the terms, conditions and stipulations of the CDOT Special Use Permit, attached as Exhibit E. b. Contractor s insurance will include CDOT as an additional insured. 7. Utilities: a. Excavation is not anticipated as part of this work. If excavation occurs, Contractor to notify Utility Notification Center of Colorado (UNCC) at for utility locates. 10

11 b. Overhead power lines are present within work area and should be accommodated during the work. 8. General: a. Contractor selection shall be based on Base and Paving Bid and per qualifications and other criteria described in the Bid Documents; and not on the combined total of the Base and Paving Bid and Alternate Bid. b.
