The National Diabetes Prevention Program has been launched by the Division of Diabetes Translation of the Centers for Disease Control and Prevention (CDC) to systematically scale and sustain prevention efforts for type 2 diabetes in communities across the United States. CDC has been authorized by Congress to lead this nationwide program, which is based on the Diabetes Prevention Program (DPP). DPP was a ground-breaking research study that demonstrated that the millions of people in the U.S. who are at high risk for type 2 diabetes can prevent or delay the disease by making modest lifestyle changes through a structured program. As part of the National Diabetes Prevention Program, trained facilitators called Lifestyle Coaches deliver a lifestyle change program to groups of adults who are at high risk for type 2 diabetes. The programs take place at a variety of CDC-recognized sites around the country, including community-based organizations, health care facilities, and worksites. The National Diabetes Prevention Program includes four key elements: 1) training the workforce to deliver the lifestyle change program, 2) establishing recognition standards for programs to assure quality and consistency, 3) initiating lifestyle change programs at recognized sites, and 4) raising awareness of the program among high-risk populations, health care providers, and insurance providers.
2 The National Diabetes Prevention Program is a critical opportunity for collaboration among federal agencies, community-based organizations, health payers, health care professionals, academia, and others to reduce the number of people in the U.S. with type 2 diabetes and prediabetes. For more information about the National Diabetes Prevention Program, please visit the CDC website ( 2. What is the National Diabetes Prevention Program Lifestyle Change Program? The backbone of the National Diabetes Prevention Program is the lifestyle change program designed to help prevent or delay type 2 diabetes in adults who are at high risk for the disease. During the lifestyle change program, a trained Lifestyle Coach works with a group of participants to help them reduce their risk of developing type 2 diabetes by: 1) losing weight through healthy eating, 2) being more physically active, and 3) learning to identify and address barriers to healthy eating and physical activity. The Lifestyle Coach tailors the program to ensure it is culturally-appropriate and accounts for food preferences and other group needs. All participants work towards goals of losing 5-7% of their starting body weight and reaching and maintaining 150 minutes of moderate physical activity each week. The National Diabetes Prevention Program lifestyle change program lasts approximately one year, and is divided into two parts: Core Sessions: The 16 core sessions are focused on the process of adopting lifestyle changes for healthy eating and physical activity. These sessions are designed to help participants develop lifelong skills for healthy living and reinforce step-by-step change. Groups of participants generally meet with their Lifestyle Coach each week at the same time and location. Post-Core Sessions: Following the core phase, participants attend post-core sessions on a monthly basis. The post-core sessions are intended to provide additional support and learning opportunities to participants, and help them transition to independently maintaining their lifestyle changes.
3 3. What is the National Diabetes Prevention Program curriculum? The Diabetes Training and Technical Assistance Center (DTTAC) at Emory University collaborated with partners from the University of Indiana (Plan Forward), the University of Pittsburgh (Group Life Balance) and CDC to develop the National Diabetes Prevention Program curriculum. The curriculum was adapted from the original Diabetes Prevention Program (DPP) research study materials for use in group delivery and on a national scale. Additional training and support materials for Lifestyle Coaches and Master Trainers were also developed by DTTAC and its partners. Portions of the curriculum are available for viewing on the CDC website ( The National Diabetes Prevention Program curriculum consists of the following elements: Lifestyle Coach Training Manual Used to train Lifestyle Coaches to effectively deliver the lifestyle change program. Includes information on diabetes, the Diabetes Prevention Program research study, target audience, group facilitation, and comprehensive information and guidance on the lifestyle change program. Master Trainer Supplement Materials to assist Master Trainers of Lifestyle Coaches. Includes information on preparation, training agenda, activities, and evaluation. Lifestyle Coach Facilitation Guide (Core) A step-by-step guide to assist Lifestyle Coaches in facilitating each of the 16 core sessions of the lifestyle change program. Participant Notebook (Core) Contains worksheets and handouts for participants to use and reference in each of the 16 core sessions of the lifestyle change program. Lifestyle Coach Facilitation Guide (Post-Core) An annotated version of the post-core participant handouts which include additional facilitation tips for Lifestyle Coaches. Participant Notebook (Post-Core) Contains worksheets and handouts for participants related to each session s content in the post-core phase of the lifestyle change program. Additional materials that are used by participants during the lifestyle change program include: Food and Activity Tracker: Participants will write down their daily food intake, physical activity, and weight in a log book. The Lifestyle Coach reviews the completed tracker and provides feedback each week. Fat and Calorie Counter: Participants will use this book (or equivalent resource) to identify fat and calorie information for the food and beverages they consume.
4 4. What is a Lifestyle Coach and who can become one? Lifestyle Coaches are trained facilitators who deliver the National Diabetes Prevention Program lifestyle change program at CDC-recognized sites. They are trained in the national curriculum and possess strong interpersonal and group facilitation skills. Lifestyle Coaches for recognized sites will be responsible for leading sessions, conducting regular weigh-ins, reviewing participant self-monitoring records, and submitting program data to the CDC in conjunction with their site coordinator. Lifestyle Coaches do not have to be health care professionals. The success of the lifestyle change program relies heavily on the dynamics between the Lifestyle Coach and the participants working together as a group. The Lifestyle Coach is responsible for fulfilling several key roles: Provide information and support Provide information, highlight progress, and work with the group to answer questions, generate ideas, and problem solve. Create accountability and a sense of commitment within the group. Provide guidance Work with participants to identify barriers to a healthy lifestyle and find solutions to those barriers. Establish strategies to reach goals with each participant and monitor progress towards those goals. Deliver the lifestyle change program as it was designed, and set clear expectations for participation. Leverage group dynamics Serve as a facilitator and promote group discussion and interaction. Foster relationships with and among participants. 5. Can individuals not affiliated with an organization be trained as Lifestyle Coaches? Lifestyle Coaches need to be affiliated with an organization that is seeking CDC recognition to deliver the lifestyle change program. If you are interested in becoming a Lifestyle Coach but are not affiliated with an organization, please consider partnering with an existing organization or recognized site in your community. You can check the CDC website for a list of organizations that are applying for recognition (
5 6. How can individuals in my organization receive Lifestyle Coach training from DTTAC? DTTAC will provide affordable, comprehensive Lifestyle Coach training services, as well as access to an online learning community, for organizations seeking CDC recognition to offer the National Diabetes Prevention Program. After your organization applies for CDC program recognition, the next steps are to contact DTTAC to explore options for Lifestyle Coach training. If your organization has 8 to 12 individuals who would like to be trained as Lifestyle Coaches, DTTAC can provide a local or in-house training on dates that are convenient for you. If your organization is interested in training fewer than 8 Lifestyle Coaches, DTTAC will provide quarterly regional or Atlanta-based training options. Depending on the location and number of trainees, the total costs of training will range from $1,200 to $2,000 per participant. What is a DTTAC Master Trainer and who can become one? DTTAC Master Trainers are facilitators who are extensively trained in the National Diabetes Prevention Program curriculum and operate under the direction of the Diabetes Training and Technical Assistance Center (DTTAC). DTTAC Master Trainers demonstrate in-depth knowledge of group dynamics, adult learning, and facilitation and are experts in training Lifestyle Coaches to deliver the lifestyle change program. DTTAC maintains a cadre of Master Trainers who provide Lifestyle Coach trainings throughout the country. Master Trainers must have previous experience with a DPP-based lifestyle intervention or comparable facilitation experience, expertise in adult learning and group facilitation, and familiarity with the National Diabetes Prevention Program. 6 8. What types of organizations can offer the National Diabetes Prevention Program lifestyle change program? A wide variety of organizations may offer the National Diabetes Prevention Program lifestyle change program including: Non-profit and community-based organizations Corporations and businesses Faith-based organizations Educational institutions Health care facilities Fitness and wellness centers Organizations may also support the National Diabetes Prevention Program by working to raise awareness of the program, referring high risk individuals to recognized sites, and providing financial or logistical support to sites. 7 9. How do I know if my organization has the capacity to offer the National Diabetes Prevention Program lifestyle change program? The CDC s Diabetes Prevention Recognition Program has developed a document for assessing your organization s capacity to deliver an effective lifestyle change program that meets recognition standards. Visit the recognition section of the CDC website ( see appendix A in the program standards and operating procedures document). If your organization does not have capacity at this time, you may want to support other sites in your area. The following are some issues to consider: 1. Does my organization have access to high-risk populations who are eligible to participate in the lifestyle change program? Participants are eligible if they meet certain criteria, including a diagnosis of prediabetes or other risk factors. 2. Is my organization able to finance the lifestyle change program or partner with a payer? Costs to consider include Lifestyle Coach training, staff time, space for holding group sessions, and printing. 3. Do you have staff members or volunteers who can serve as Lifestyle Coaches? If your organization has multiple Lifestyle Coaches, you may want to consider having a dedicated site coordinator as well. 4. Are you able to satisfy HIPAA requirements? HIPAA requirements are in place to keep individuals medical information confidential. Note that HIPAA requirements vary by state. 5. Is your organization able to collect and submit required data to CDC? In order to become a CDC-recognized site, you must collect specific data related to participants progress.
8 10. How can my organization become a recognized National Diabetes Prevention Program site? CDC s Division of Diabetes Translation has created the Diabetes Prevention Recognition Program (DPRP) to set standards and assure that the National Diabetes Prevention Program is being delivered in a high quality, consistent way across sites. In order to become a site with pending recognition status, your organization must do the following: Apply for recognition through the Diabetes Prevention Recognition Program (DPRP) Use an approved curriculum Agree to the program duration of 1 year Agree to the program intensity of 16 one-hour weekly in person, group-based sessions (core phase) followed by at least 6 one-hour monthly in person, group-based sessions for the remainder of the one-year program (post-core) Submit required data to CDC Pending recognition is granted to organizations that apply for recognition and agree to the curriculum, duration, and intensity requirements. Organizations will collect and submit data to CDC every six months. Full recognition is granted after 2 years if the organization demonstrates that their lifestyle change programs are effective.
