1 Cranes & Critical Lift Plan Purpose The purpose of these guidelines is to familiarize Trader Construction Company (Trader) personnel with general procedures relative to the operation of cranes at the PCS Phosphate plant site and, when required, be able to develop a critical lift plan for the specific job being conducted. General Procedures The following general procedures are provided to ensure safe operation of both Trader owned cranes and rental cranes: Only authorized competent CCO qualified Trader personnel will enter the cab or attempt to operate a crane. Trader s operator will be in good condition, both physically and mentally. Cranes should be kept in safe operating condition and will be inspected before they are operated. Trader s operator will be responsible for housekeeping in the cab and on the crane. All cranes will be equipped with rate load limits. The crane boom will not be positioned closer than a 10-foot radius to a power line when the boom is extended to its utmost length nearest the power line. Warning signs with wording "Warning-Unlawful to operate this crane within 10 feet of high voltage line" will be attached inside the cab of the crane; also a similar sign will be placed on both sides of the cab. Tag lines will be used when handling long loads or when travelling with materials. Do not try to hold on to materials to control the load. A standard signal system will be posted on the crane, and the Crane Operator will take signals from only one (1) Signalman. If in doubt about a signal, the Operator will not move any part of the crane until a clear signal is given.

3 STOP EMERGENCY STOP Common Safety for Crane Operation A crane operator should always move loads according to the established code of signals, and use a signaler. Hand signals are preferred and commonly used. Only a qualified person should give signals to the crane operator. There should be only one designated person at a time giving crane signals. A crane operator should move loads only on crane signals from one person. A crane operator must obey STOP signals no matter who gives it. The person giving crane signals must be in clear view of the crane operator. The person giving crane signals must have a clear view of the load and the equipment, The person giving crane signals must keep persons outside the crane's operating area. Any request or questions should be addressed to the signaler. The person giving crane signals should never direct a load over a person. No part of a crane or load should come within 15 feet of any power line. High power transmission lines may require additional clearances. Each crane operator should be held directly responsible for the safe operation of the crane. Whenever there is any doubt as to SAFETY, the crane operator should stop the crane and refuse to handle loads until safety has been assured. These safety rules should by no means be considered complete for all situations of crane operation. The Operator will test the brakes and check the boom angle and any time the load is approaching the rated capacity, the load will be lifted a few inches and the brakes applied. If they do not hold properly the load will be lowered and the brakes adjusted or repaired. All outriggers will be used and the boom angle changed as needed prior to attempting a lift of near maximum loads. Before repairs are made on any mobile crane, especially repairs performed in the clutch or drums, the boom will be lowered to the ground.

4 No one will stand or walk under a suspended load, nor will the Operator swing a load over persons working in the area. No one will ride the hook, load, headache ball, or cable at any time. The only exception would be in the event of an emergency, at which time a person may ride the hook, provided that the person is tied off securely above the headache ball. Do not slow the crane boom too rapidly when carrying a load, since the momentum of the load could upset the crane. Do not overload the boom. Make sure all slings, ties, and hooks are properly placed and secured before hoisting. Keep hands and clothing clear of cables feeding into drums. Lower the boom, shut off the power, and lock the controls before leaving the machine. No load will be left suspended when the crane is unattended. Do not lean against a crane whether it is idle or waiting for the Rigger. Keep a fire extinguisher on all cranes and in good operating condition. In the event the boom contacts overhead power lines, stay in the machine until the power is turned off. All grounds men will stay clear of the crane or its load when working near power lines. When loads extend past the front or rear or past the sides of the equipment while operating on roads, the load will be flagged for visibility. Rigging Do not throw or drop objects from any elevation unless the area is roped off, flagged, and designated as a drop area. No one will pass under a suspended load. Full body harnesses will be worn when working in high places. Give safety the first consideration in all handling of materials. Familiarize yourself with types of slings available for easiest and safest lifting.

5 Remove damaged lifting equipment so it cannot be used to make another lift. Remove the loose materials, such as bolts, tools, and lifting gear from the load before it is moved. Have one (1) authorized person give approved crane signals to the Crane Operator. Make sure the slings are safe and the load is properly balanced. Store lifting equipment in proper racks and places so it does not become damaged. Tag lines will be used when handling long loads or when travelling with materials. Never try to hold on to the material and control the load simultaneously. Avoid slack with choked loads when using eye and eye cabled slings. Signals used to govern the movements of the crane will be recognized standard system with which all Mechanics doing rigging will be familiar. All cranes travelling in the fenced areas will be directed by an attendant on foot; outside the fenced areas a vehicle may be used by the attendant. When the boom is retracted and the crane is not travelling with a load, no attendant is required. Cranes when not in use should be removed from operating areas and secured in a safe manner (cab locked). Policy for Crane Set-up on Railroad Tracks Trader s superintendent, project manager, and/or safety personnel will notify affected PCS Phosphate operations department personnel as far in advance of set-up as possible. Trader will coordinate with key PCS Phosphate railroad management personnel to position the rail switch to isolate the affected spur, and lock the switch with a safety lock. If no switch exists or if throwing the switch blocks too much track, a derailer will be installed. Trader will consult with key PCS Phosphate railroad management personnel Local personnel regarding which rail the derailer is to be placed. Trader personnel will install a blue flag ahead of the derailer to warn away other operators of switch engines or locomotives.

6 MINIMUM CLEARANCE FROM ENERGIZED OVERHEAD ELECTRIC LINES (All dimensions are distances from live part to employee) Voltage (nominal, kv, alternating current) Minimum rated clearance Up to ft (3 m) ft (4.6 m) ft (6 m) 351 to ft (7.6 m) ft (9.1 m) ft (10.7 m) ft (12.2 m) ft (13.7 m) Clearance values calculated using: (Initial kv-50kv) x (4 in/10 kv) x (1 ft/12 in) = increased distance (ft) over 10 ft. Add this value to 10 ft to yield minimum rated clearance. Lifts the operator believes should be considered critical; Lifts made when the load weight is 75% of the rated capacity of the crane load chart or more (not applicable to gantry, overhead or bridge cranes); Lifts without the use of outriggers using rubber tire load charts; Lifts using more than one hoist on the same crane or trolleys; Lifts involving non-routine or technically difficult rigging arrangement (to include lifts involving Multiple Lift Rigging; Lifts involving submerged loads (EXCEPTION: lifts that were engineered to travel in guided slots throughout the lift and have fixed rigging and/or lifting beams, i.e., intake gates, roller gates, tailgates/logs,); Lifts out of the operator s view; EXCEPTION: if hand signals via a signal person in view of the operator or radio communications are available and in use, load does not exceed two tons AND is determined a routine lift by the lift supervisor. Critical Lift General Considerations When using cranes or hoists, the following are identified as critical lifts requiring detailed planning and additional or unusual safety precautions. Critical lifts are defined as: Lifts involving hazardous materials (e.g., explosives, highly volatile substances)

7 Hoisting personnel with a crane or hoist Lifts made with more than one crane Lifts where the center of gravity could change Before making a critical lift, a critical lift plan will be developed: By a qualified person and will include the crane operator, lift supervisor, and the rigger and signed by all involved personnel prior to the lift. For a series of lifts on one project or job, as long as the cranes, personnel, type loads and configuration do not differ. And documented with a copy provided to PCS Phosphate s Health and Safety Department prior to the lift(s) being made. And will include, as a minimum: The specific make and model of the cranes, the line, boom, and swing speeds; The exact size and weight of the load to be lifted and all crane and rigging components that add to the weight. The manufacturer's maximum load limits for the entire range of the lift, as listed in the load charts, will also be specified; The plan will specify the lift geometry and procedures, including the crane position, height of the lift, the load radius, and the boom length and angle, for the entire range of the lift; Site drawing will be included to identify placement/location(s) of crane, adjacent equipment and/or facilities, etc.; The plan will designate the crane operator, lift supervisor and rigger and include their qualifications; The plan will include a rigging plan that shows the lift points and describes rigging procedures and hardware requirements; The plan will describe the ground conditions, outrigger or crawler track requirements, and, if necessary, the design of mats, necessary to achieve a level, stable foundation of sufficient bearing capacity for the lift;

8 For floating crane or derricks, the plan will describe the operating base (platform) condition and any potential maximum list / trim; The plan will list environmental conditions under which lift operations are to be stopped; The plan will specify coordination and communication requirements for the lift operation; For tandem or tailing crane lifts, identify the requirements for an equalizer beam if applicable. Rental Cranes To maintain quality and safe crane operations, Trader will follow PCS Phosphate s Rental Crane Inspection procedure: Rental cranes are equipment which are mobile and can be utilized to lift material or personnel above ground level to complete a job. Examples are manlifts, scissor lifts, conventional or hydraulic cranes, etc. Upon arrival of the rental crane, Trader s superintendent which requested it will inspect or have it inspected by a designated competent person to assure it meets the requirements established. The inspection will include all areas specified on the PCS Phosphate s Rental Crane Inspection Report. The Rental Crane Inspection Report must be approved by Trader s superintendent, even though he may have designated competent person to do the actual inspection. The competent person must sign the report, also. Once the report is complete, the crane rental representative must also sign the form acknowledging agreement with any problems noted. When the required individuals have signed the report, distribution must be made as noted on the lower right half of the report.
