3 The Barkers Pet Resort Enrollment Packet and Policies Page 1 Client Agreement 1.! Owner agrees to pay the rate for daycare, boarding, bathing, and grooming in effect on each date the pet is checked in to The Barkers Pet Center. 2.! Owner further agrees to pay all charges for special services requested, all veterinary cost if needed, for the pet during the time said pet is in the care of The Barkers Pet Center. 3.! Owner assumes any expense or liability for injuries that Owners pet may inflict upon any human or other pet while at the Barkers Pet Center 4.! Owner further agrees that the pet shall not leave The Barkers Pet Center until all charges are paid. 5.! By signing this contract and leaving said pet with the Barkers Pet Center, the Owner certifies to the accuracy of all information given about said pet. 6.! Owner specifically represents to The Barkers Pet Resort that pet has not been exposed to rabies, distemper or bordetella within a thirty-day period prior to being left for boarding, daycare, grooming or bathing. 7.! All charges incurred by owner shall be payable upon pick up of pet. The Barkers Pet Center shall use any and all legal remixes to collect any and all unpaid charges resulting from boarding, daycare, grooming, bathing, or other service provided by The Barkers Pet Center at the Owner s expense. 8.! The Barkers Pet Center shall exercise reasonable care for the pet delivered by the owner to the Barkers Pet Center for boarding. It is expressly agreed by the Owner and The Barkers Pet Center liability shall in no event exceed the less of the current chattel value of a pet of the same species or the sum of $ per animal boarded. The Owner further agrees to be solely responsible for any and all acts or behavior of said pet while it is in the care of The Barkers Pet Center 9.! If the pet becomes ill or if the state of the pet s health otherwise requires professional attention, The Barkers Pet Center in it s sole discretion may engage the services of a veterinarian or administer medicine or give other Requisite attention to the path and the expenses Vera shall be paid by the owner. Owner gives consent to the Barker's Pet Center to act and owner's behalf in obtaining emergency or any veterinarian care at owner s expense necessary by the Barker's pet center or any of its employees. Owner indemnifies and holds the Barker's Pet Center harmless for said expenses. 10.!For medication which must be given by hand, the owner will be assessed a fee of $4.00 per administration. 11.!This contract contains the entire agreement between the parties. All terms and conditions of this contract Shelby binding on the heirs, administrators, personal representatives and assigns of the Owner and the Barker pet center. 12.!Owner is aware that by leaving said pet at the barkers pet center, or any other pet facility, send pet is at a higher risk of contracting kennel cough for other viruses. Although all of the pets are required to be vaccinated, No vaccine is 100% guaranteed. There are some strains of kennel cough not covered by the Bordetella vaccine. Owner understands that owner is solely responsible for any and all medical bills incurred to pet illness. 13.!Owner understands that there are risks and benefits associated with group socialization of dogs. Owner agrees that while the socialization and play is closely and carefully monitored by the Barkers Pet Center staff to prevent injury, it is still possible that during the course of normal play the dogs may receive minor nicks and scratches from roughhousing with other dogs. Any injuries to the dog will be pointed out by staff upon pick up. 14.!If any boarding dogs are using daycare service and show signs of aggression with other dogs, pet will be removed from daycare and assigned private time play/ walks. This up automatically applies to all nonneutered or spayed dogs. 15.!Owner understands that if pet is not picked up on the date specified and a separate agreement and no notice given to the Barkers Pet Center after three days The Barkers Pet Center will proceed abandonment of animal procedure for handling as per guidelines provided bye Florida statue

4 The Barkers Pet Center Enrollment Packet and Policies Page 2 Pet Grooming We want your pet s experience here at The Barkers Pet Center to be as pleasant and enjoyable as possible and it's important to understand that every pet response to grooming and bathing different. Here are some additional things we would like you to be aware of: Skin and hair conditions: Grooming can expose pre-existing skin conditions you may not have been aware of because they were either dormant or covered by hair. Please let us know if your dog has any allergies but even the mildest shampoo may cause an allergic reaction resulting in redness or itching. Joint and bones: Please let us know if your pet has any bone or muscle conditions. Grooming is such a physical process, some pets with bone or joint conditions may have those conditions irritated during the grooming process. Matting: Heavy matting or knotting of the hair is both harmful to your pet skin health and painful to your pet because every time they move, the skin is pulled and tugged which results in red patches all the inflamed skin. Conditions such as hotspots, bacterial, and fungal infections can also grow under mats because some severe mats may actually be so tightly bound to the skin that it is difficult to get our de-mating tools or clippers in between the hair and skin which could results and injuring your pet with grooming took. Please be aware of this before leaving your pet in our care. Shave Down: When you choose to shave your pet down completely, skin conditions can be exposed. Please be aware that their hair may not grow back in the same manner. After shave down some sensitive areas may become red or inflamed. After shave down some sensitive areas may become red or inflamed. After shave down we recommend to use sunscreen when outdoors to prevent sunburns. Senior Pets: Seniors pets may experience more stress during the grooming process. This could result in pet fainting, having seizures, heart failure, stroke or even death. We re always extra gentle and take extra time with senior pets. You can also choose to wait for your pet in the store and see grooming process through our glass doors and window. Aggressive Pets: Our grooming staff will use muzzle on your pet if they find your pet to be aggressive towards groomer. This is just a safety precaution to keep our staff and your pet safe. Puppy Haircuts: We don't recommend giving your puppy a complete haircut before the age of 12 weeks. A puppy s hair is growing and forming. Clipping the body of the hair too early can result in damaging the hair texture or stun the growth. We recommend to gently introduce the grooming process with baths and nail clippings; before you move unto full grooms. Hyper and Overwhelmed Pets: Even though our grooming staff is highly experienced and very patient, some pets are more skittish and wiggly than others. When grooming a dog or a cat there is always a risk of nicking or cutting the animal. Especially when they move a lot. We look forward to working with your pet regularly so they always look, feel and smell great!

5 The Barkers Pet Center Enrollment Packet and Policies Page 3 I have read and understand the above and I hereby release The Barkers Pet Center, and its staff from any responsibility of the above mentioned process and any medical problems that may be uncovered and/ or may occur during and after the grooming process. Should my pet need veterinary care during or after the grooming process. I authorize care to be administered and agree to pay any and all veterinary fees for my pet s care and agree not to hold The Barkers Pet Center and owners and its employees responsible in any way. This authorization, unless revoked in writing, will apply to every visit your pet makes to our facility. General Liability Release You hereby release The Barkers Pet Center, its agents, officers, subcontractors, employees, animal owners, customers and potential customers of The Barkers Pet Center from any and all liabilities for injuries to yourself, your pet, or your property, which arise with any way out of services provided by or as a consequence of your association with The Barkers Pet Center. You understand that there are certain risks involved with pet ownership, training, and care including, but not limited to, fights, bites, and the transmission of disease. By your signature below, you acknowledge and accept responsibility for these and all other risks and release The Barkers Pet Center from any injuries that might occur, no matter the cause. (Including illness that would have normally occurred had the pet not been at The Barkers Pet Center.) This agreement lasts the entire relationship between you and The Barkers Pet Center. All answers on this sheet are accurate to the best of your knowledge. Owner s Signature: Date: Emergency Medical Care If in our judgment your pet requires medical care and we are unable to reach you, please sign below granting permission for us to obtain medical care for your pet. I give permission for The Barkers Pet Center to seek medical attention for my pet if they find it necessary even if they are unable to reach me. I understand that I am solely responsible for the payment of all medical bills for my pet. I release The Barkers Pet Center, its owners and employees of any and all responsibility, claims, damages, and debts from arising out of related medical care. This includes, but is not limited to transaction to / from the veterinarian clinic and choice of any local animal hospital. Owners must provide proof of vaccines before their pet can obtain services (some exclusions apply to grooming clients)! Dogs Distemper/Parvo Bordetella less than six months Rabies! Puppies (under 4 months) must have at least 2 rounds of puppy shots and bordetella. Proof of rabies at 4 months is required.! Cats FVRCP Rabies FeLV Yearly negative FeLV & FIV test

6 Doggy Day Care Policies! All dogs are required to wear an appropriate play collar. It needs to be made of cotton / nylon. No pinch collars or choke chains allowed in playgroups. Identification tag is recommended.! Owners are allowed to bring personal belongings. All belongings, including food if brought, need to fit in our provided cubbies.! If we notice a dog infested with fleas or ticks, we will immediately bathe them and treat them for fleas or ticks. Additional fee will apply.! The Barkers Pet Center provides food for all overnight guests at additional fee per serving. If your dog requires a special diet please bring your own.! In order to maintain the safest playgrounds possible, we have to enforce certain playroom etiquette. Therefore occasionally we do have to discipline the dogs.! If a dog shows aggression (growling, snapping) towards a handler, they will be immediately moved to their suite. They will have alone playtime until check out or until owners have had a private training session and they are approved to attend daycare by a trainer.! All dogs must be kept current on vaccinations in order to play. No exceptions made.! Owners are aware that there are certain risks with allowing your dog to play with other dogs. When dogs play together the following is possible; bites, punctures, scratches, broken bones, bruises, transmission of disease, and others.! All owners are responsible for their own pet.! In order to remain a doggie daycare member, all daycare dogs MUST attend daycare once every 6 months or have another Temperament Test. Grooming Policies! Prices quoted on the phone are estimates only. The groomer will be able to give you an exact price one he/she has seen the pet! If we notice a pet infested with fleas or ticks, we will immediately bathe them with a flea/tick shampoo and spray. An additional fee will apply.! Heavy matting that causes extra grooming time will incur an additional of $30.00 for each half hour.! Our groomers pride themselves in working with difficult pets. However, if a pet s behavior becomes unmanageable, the groomer may opt to stop. As a result, this may mean an incomplete groom, however the price will remain the same.! All grooming pets must be current on vaccines. The Barkers Pet Center is not responsible for any illnesses that occur due to your pet not being fully vaccinated. Owners Signature: Date:
