5 FAMILY COVENANT The National Directory for Catechesis states that when children are baptized, parents accept the responsibility to bring up their children in the practice of the faith. At the same time, the Church promises to help parents foster their children s faith and assists them specifically in their role as catechists of their children. This Covenant with the Faith Formation Program for Saint John the Baptist Church and Saint Mary of the Lake Church and the parents and/or guardians of the children involved in this program reinforces the role of Catholic parents and the Parish community. A participating parishioner is a parishioner who attends Mass on weekends and Holy Days of Obligation, participates in the sacraments, participates in the activities of the parish, and contributes to the offertory collection in a recognizable way by using envelopes. We promise to support our child in every way possible to fulfill the obligations of the Faith Formation Program and make religious education a priority in our child s life. Attendance as well as arrival/dismissal times will be adhered to and absences reported to the Faith Formation Office. We will endeavor to help our child memorize prayers and learn the material in the books that are utilized in the Faith Formation Program We will continue to support our child in the Faith Formation Program and be involved in their growth in the Catholic faith. We promise to be fully involved participating parishioners as described above and are willing to be good stewards of our time, talent and treasure. Student's Name: Parents/Guardians Signature: Date: Date: Coordinator: Date: Please complete and return to the Faith Formation Office with the Registration Form.
