Yes No VOLUNTEER DATES Summer Camp volunteers must commit to at least one (1) full week (Monday through Friday) or volunteer work. Please indicate what week or weeks you wish to volunteer May 31-June 3 (4 days) June 6-10 June June June 27-July 1 July July July Aug 1-5 Junior Camp Counselors are assigned to volunteer from 8 am to 4 pm; or 9 am to 5 pm each day during their assigned week(s) of camp. GROUP INTERVIEWS AND MANDATORY TRAINING All Summer Camp volunteer applicants are required to complete a group interview. Upon receipt of your application materials, we will contact you to schedule a group interview. Additionally, if you are selected for a volunteer position, you will be required to complete a group training session prior to the beginning of camp in order to retain your volunteer position. 3 PARENT PERMISSION FORM Science is Everywhere Summer Camp Volunteer Program at the National Museum of Nuclear Science & History (NMNS&H) I (we) the parent(s) of (please print child s full name) give my (our) permission for our child to take part in the activities of the Science is Everywhere Summer Camp Program at the NMNS&H. This includes, but is not restricted to, - assisting the Museum Staff in various activities related to the operation of the Science is Everywhere Summer Camp program. - creation or production of materials for Museum use within the camp program. - periodic trainings and activities within the Museum facility. We also grant the NMNS&H and the National Atomic Foundation my (our) permission to use - any original creation written or developed by my (our) child while involved in a Museum activity. - photographic images of my (our) child taken while he/she is involved in a Museum activity. 5 Science is Everywhere Science Summer Camp Parent Agreement VOLUNTEER NAME: PARENT NAME(S): VOLUNTEER DATES: May 31, 2016 through August 5, 2016 As the Parent(s) or Gaurdian(s) of a Junior Camp Counselor, your support of your child s volunteer work is important. Please review the agreement below. Volunteer Requirements My child is at least 14 years old, or will be at the time of volunteer service. I understand this position is an unpaid, temporary volunteer position. Volunteer Shifts and Schedule I will help my child be punctual for his/her volunteer shift and adhere to his/her assigned volunteer schedule. I understand that my child cannot volunteer for more than 8 hours per day. This means that I will drop my child off at the assigned time and pick him/her up at the assigned time. The Museum will not accommodate early/late drop-offs or early/late pick-ups I understand that when I arrive to pick up my child, he/she may be in the process of completing a task. He/she will be dismissed from his volunteer duties at the assigned time. My child will contact the Volunteer Manager if he/she is ill, if there is an emergency, or if there is any other reason that causes him/her to be late or to miss my volunteer shift. My child will provide at least 24-hour notice for any appointments that occur during camp hours that will result in an absence. My child will provide at least one week notice if he/she is unable to volunteer for his/her assigned week(s) of camp. Volunteer Expectations: My child is a role-model to Campers and his/her peer Volunteers. He/she will always display appropriate behavior. My child will wear appropriate clothing, following the dress code outlined in the Policy and Procedure Manual. I understand that Summer Camp follows a nut-free policy. I will assist my child in packing a lunch/snack that does not contain tree nuts or tree nut products. My child will receive a copy of the Science is Everywhere Summer Camp Volunteer Handbook. I will review the Policies and Procedures outlined. I understand that the Museum reserves the right to dismiss any volunteer who compromises the safety of him/herself, any camper, volunteer, or staff member. The Museum also reserves the right to dismiss any volunteer who does not fulfill the camp duties, or adheres to the camp Policies and Procedures, as outlined in the Science is Everywhere Summer Camp Volunteer Handbook.
