4 RELEASE FORM Participation in the OPERA NIGHT 2017 Program requires the signature below of the participant or, if the participant is under 18 years of age, the signature of a parent or guardian. The undersigned, for myself and on behalf of any minor children participating in the workshops and concerts, hereby release the SOCSA Academy and its past, present, and future officers, agents, representatives, successors, assigns, and insurers from any and all liability, loss, damage, costs, claims and/or causes of actions, including but not limited to all bodily injuries, damages or loss of property arising out of participation in OPERA NIGHT The undersigned, for myself and on behalf of any minor children participating in the workshop, give my permission for the SOCSA Academy to use photographs and/or video of me (or my minor child) for publicity of OPERA NIGHT 2017, including but not limited to internet publication and inclusion in any SOCSA publication. 5 PARTICIPANT S COMMITMENT STATEMENT As a Participant in Opera Night Program, sponsored by the South Orange County School of the Arts Academy, I commit to abide by these specific rules: 1. I understand that participation in the Opera Night Program requires me to be present at all assigned courses, workshops, Master Classes, rehearsals, and all three (3) Opera Night Concert Performances. PRIOR NOTIFICATION IS REQUIRED FOR ANY ABSENCES. 2. I understand that all proceeds and donations from these performances will benefit the South Orange County School of the Arts Academy. 3. I understand that my parents and I will be solely responsible for transportation to and from all Opera Night 2017 related activities including all concert performances. 4. I understand that the Opera Night Program requires a high level of commitment and self-discipline. Should I cause problems of a musical or behavioral nature detrimental to the SOCSA Opera Night program, I understand that I could be dropped from the program. Such action may be taken only with the full knowledge and approval of the Opera Night Director and SOCSA Artistic Director. 5. It is my sole responsibility to practice all music/song repertoire and coursework assigned me during the Opera Night Program, arriving well prepared to each Saturday Workshop and rehearsal. : o I have read and I understand this Commitment form, and I enthusiastically accept participation in the Opera Night Program.
