4 File: F SECTION 3.0 DESCRIPTION OF PRINCIPAL PARTIES 3.1 The prime contractor, Willow Spring Construction (ALTA.) Ltd. is a quality provider of office/retail interiors contracting, general contracting, design build and construction management services to Western Canada s industry and businesses. 3.2 Derrick Concrete Cutting & Construction Ltd. provides a variety of almost every type of concrete cutting, construction or specialty demolition. SECTION 4.0 LOCATION OF INCIDENT Avenue, Edmonton.(Attachment A Map). SECTION 5.0 EQUIPMENT, MATERIAL AND OBSERVATIONS 5.1 Equipment and Material Cushion Cut / Longyear Wall Saw Header Model # 360 (Attachment C Photograph #1) Concrete Slab measuring 1.3 m wide x 2.21 m long x 15 cm thick and approximate weight of 816 kg. (Attachment C Photograph # 6) 5.2 Observations Occupational Health and Safety were notified about the incident after the worker had succumbed to his injuries at the hospital. The incident occurred at approximately 10:15 a.m. on June 22, 2009 and the worker died at approximately 1:20 p.m. on June 22, When the investigating officers arrived at the location the scene had been cleaned up and the equipment removed. The slab of concrete was lying on the ground outside a building under construction, where it had fallen after being cut from the building s concrete wall The procedure for wall sawing includes the placement of wedges in the bottom and top cuts on both sides of the opening, approximately 15 to 30 cm from the vertical cuts. Prior to making the final cut, there is to be a track or safety strapping bolted across the opening so the wall will not fall prematurely. In addition, the fall area is to be cleared. SECTION 6.0 NARRATIVE DESCRIPTION OF THE INCIDENT 6.1 On June 22, 2009 two workers, Worker1 and Worker2 were assigned to proceed to a job site to cut out a doorway in a pre-cast concrete wall at a facility under construction. 4 August 11, 2010

5 File: F Worker 1 and Worker 2 arrived at the work site and were directed to the location where the doorway was to be cut. The two workers performed a pre-job hazard assessment and then set up their equipment. 6.3 Worker 1 and Worker 2 mounted a wall saw to cut an opening through the pre-cast concrete wall. 6.4 After the wall saw had been mounted the two workers were ready to cut the doorway. Worker 2 proceeded to start cutting the pre-cast concrete for the doorway. Worker 2 cut the bottom, top and right side and Worker 1installed metal wedges in the bottom cut, Worker 2 then proceeded to make the last left side cut for the doorway. 6.5 When Worker 2 finished the last cut on the left side he shut the saw off and Worker 1 unhooked the hydraulic hoses. Worker 2 went to the cab of the truck to reduce the idle speed which was operating the power take off for the wall saw. 6.6 Worker 1 was at the rear of the truck when the slab of concrete fell and pinned him against the truck. 6.7 As Worker 2 was at the cab of the truck he heard a moan at the rear of the truck. When he went to the rear of the truck he discovered Worker 1 pinned underneath of the slab of concrete that had been cut for the doorway. Worker 2 immediately called for assistance. 6.8 Other workers at the work site came to the assistance of the injured Worker 1 and lifted the slab of concrete off him and performed emergency first aid. Emergency medical services were called and when they arrived, the injured Worker 1 was transported to the hospital for further medical assessment. 6.9 Worker 1 succumbed to his injuries at the hospital. SECTION 7.0 ANALYSIS 7.1 Direct Cause The Worker received serious crush injuries that became fatal later that day as a result of him being crushed under a falling concrete slab. 5 August 11, 2010

6 File: F Contributing Factors The two workers did not follow the procedure developed by Derrick Concrete Cutting & Construction Ltd There was insufficient wedging of the concrete slab as required in the procedure referred to in Section There were no wedges placed in the top cut prior to making the last left side cut. SECTION 8.0 FOLLOW-UP/ ACTION TAKEN 8.1 Employment and Immigration; Occupational Health and Safety Occupational Health and Safety issued an order to Derrick Concrete Cutting & Construction Ltd. to conduct an incident investigation, prepare a report and have it available to Occupational Health and Safety for review Occupational Health and safety issued an order to Derrick Concrete Cutting & Construction Ltd. to submit for review; training records of the fatally injured worker and co-worker, orientation records, and a copy of the hazard assessment that was prepared prior to the start of the work where the incident occurred. 8.2 Industry Derrick Concrete Cutting & Construction Ltd. submitted an incident investigation report to Occupational Health and Safety for review Derrick Concrete Cutting & Construction Ltd. submitted the following documentation to Occupational Health and Safety; training records of the fatally injured worker and co-worker, orientation records and the hazard assessment that was prepared prior to the start of the work where the incident occurred. 8.3 Additional Measures No further measures at this time. 6 August 11, 2010
