Original UK STEP ONE: Obtain, Complete & SIGN These Documents You must send/bring your current passport if this service is required in order to: - Replace expired or damaged passport - If you require having changes made in your passport due to marriage, divorce, sex change, etc. - If your appearance has significantly changed, e.g. after undertaken plastic surgery, accident, etc. - You require to extend your expired passport due to valid visas - If you are a parent applying on behalf of your under 16 year old child who has a UK child passport Original passport is not required if you are applying to replace your lost or stolen passport however you will have to complete a Lost or Stolen Notification Form (please refer to this section below). Any Other s You Hold If you have any other passports you hold in your name and they are valid, you must send it/them along. Original UK Birth Certificate If your child is under 16 years old, we can help you obtain the first child passport. This service is not applicable if your child is turning 16 in the next few weeks or is already 16 and has to apply for the first adult passport. Four Style Colour Photos All four photographs must be identical. If any of the photos do not get used, we will return them to you. We strongly recommend getting a photograph taken by a passport photographer as he/she is familiar with passport style photograph requirements. Alternatively, you can a good quality digital photo to info@travelvisapro.co.uk and we can print it for you for additional fee of 10 + VAT for a set of two photos. Your photos must be taken within the last three months.

2 UK Application Form This form and filling guide can be obtained from any Post Office or Travel Visa Pro Office. Please complete and SIGN UK Application Form. If you need any assistance we can provide the VIP service for an additional fee of 20 + VAT. A Proof of the Name/Title Change (if applicable) If your name or title has changed due to marriage, civil partnership, divorce, etc., please provide the ORIGINAL certificate to prove it. Lost or Stolen Notification Form If you lost your passport of it has been stolen, you will be asked to complete a Lost or Stolen Notification Form that can be obtained from any Post Office or Travel Visa Pro office. Authorisation Letter Please fill and sign Authorisation Letter which will allow us to collect your passport from the Office on your behalf. Please leave blank the name of the person you are authorising to collect your passport as more than one person from our team can do it. We will complete this part ourselves once we know who exactly has been designated to do this task. STEP TWO: Complete Travel Visa Pro Order Form Please ensure you provide full details of delivery address (where passport will be returned to you) and payment information (so we avoid any delay when collecting the payment). If you are applying for passport for more than one person, but all passports have to be returned to the same address and paying details are the same, you need to complete the form only once. However if passports have to be returned to multiple addresses or payment details are different, please complete a separate form for each delivery address or payment method. STEP THREE: Make a Payment We accept the following types of payment: - Debit/Credit card - Cheque. Make cheques payable to Travel Visa Pro Ltd - PayPal. VAT) If you require passport to be returned to you by post, please see Royal Mail price guide further below and add the costs.
