2 What must you do when someone dies? You need to inform a number of people and organisations and complete certain documents required by law. If you are a relative or friend of the deceased you can do some of these things yourself. Others will need to be done by the executor or administrator of the estate. There is plenty of support available to help you through this difficult time (please see page 15 for details of how we can help). You won t be able to do everything right away, but it s important, as soon as possible to: tell the family doctor contact a funeral director, if you intend to use one. If the death occurs at home or in a nursing home the funeral director will need to be contacted to move the deceased to a Chapel of Rest and begin arrangements for the funeral check the will for any special requests What documents will you need when registering a death? When you go to register the death you must take with you: The medical certificate of the cause of death, signed by a doctor unless the coroner is issuing the paperwork If available take the deceased s birth certificate, marriage/civil partnership certificate and NHS Medical Card After the information has been recorded into the death register, the registrar will issue the necessary forms and certificates. If a post-mortem examination is not being held, the registrar will give you: a Certificate for Burial or Cremation (called the green form1), giving permission for the body to be buried or for an application for cremation to be made. If the deceased is to be buried or cremated outside of England or Wales the coroner will issue the necessary forms A Certificate of Registration of Death (form BD8); issued for the Benefits Agency In certain circumstances a death may be reported to the coroner and an inquest may need to be held. The coroner s officer will provide more information. Burnley Cemetery Office Burnley Council s Bereavement Services provides a caring and sensitive service for people at a difficult time. Staff can give helpful advice and guidance relating to all aspects of funerals and memorials. This brochure has been produced to provide more information on the cemetery and crematorium and the service that we provide. Opening hours The Cemetery Office is open between 8.40 am to 4.45 pm Monday to Friday. This is the main administration office for both the cemetery and crematorium. For further information please telephone / The cemetery and crematorium grounds are open every day of the year9 am to 8 pm (or sunset whichever is earlier). The Crematorium Office (Availability of a crematorium attendant is subject to whether or not a cremation service is taking place). 3 The Crematorium Chapel Burnley Crematorium was officially opened on 28th August The chapel, built in the traditional style typical of the period, provides seating for 180 mourners, plus a standing area at the rear of the chapel. A 40-minute period is allocated to each funeral service. The whole funeral service may take place at the crematorium, or a service can be held at a church or other venue followed by a Committal only at the crematorium A loop system, disabled access, parking and toilets are all provided. The Garden of Remembrance The garden is split into three sections spring, summer and winter. When cremated remains are laid to rest in the Garden of Remembrance, a record is kept of the section of the garden where the cremated remains are placed. If a family member expresses a wish for their own cremated remains to be strewn in the same section of the Garden of Remembrance as their relative or loved one, then this can be arranged. For further information please contact a member of the bereavement staff. An area known as The Delph has been provided for the planting of commemorative trees. A small plaque can be placed in front of the tree commemorating a loved one. The Delph is situated at the end of the first path from the car park. For further information please contact a member of the bereavement staff. Relatives and friends of the deceased are respectfully asked to refrain from placing tokens of any kind on the remembrance garden as they quickly deteriorate and become unsightly. Those wishing to bring flowers for special occasions or anniversaries are asked to place them in receptacles within the Chapel or beneath the Walls of Remembrance. An attendant will be pleased to advise. Music Music is a very important part of a funeral service. The chapel is equipped with a computerised music system, with a large library of many different types of music. An organ is also provided in the chapel, but an organist must be provided, usually by the funeral director. Ask the staff about providing your own music. The Delph The Gardens 4 5
4 The Books of Remembrance The Books of Remembrance are on view daily, providing a lasting memorial to your loved one. The name of the deceased and a suitable epitaph of up to eight lines is recorded on the date of your choice, which could be the date of death, a birthday or an anniversary. The pages will be turned daily so that inscriptions may be viewed on every anniversary of the date requested or any other day by appointment. The lettering is hand-written in the finest traditions of mediaeval illuminated manuscripts, and the volumes are richly bound and tooled. The Books of Remembrance have been placed in a special Remembrance Room, which is adjacent to the chapel, with its own separate entrance opposite the Walls of Remembrance. Memorial Cards Memorial cards are available for individual entries. The date and inscription is carefully inscribed inside exactly as it appears in the Book of Remembrance. The Cemeteries Burnley has two cemeteries within the borough, both providing full burial facilities and each having new graves available. The exclusive right of burial in a grave can be purchased for a period of 40 years. At the end of the period, the lease may be renewed; otherwise the rights transfer back to the council. The registered grave owner will receive a deed to the grave. This is a legal document and should be kept in a safe place. Bereavement services must be contacted for advice if it is wished to transfer the deed to a new owner. Burnley Cemetery Burnley Cemetery was opened on 1st June The cemetery is the final resting place of many prominent people who helped to shape Burnley. The Victorian section of the cemetery is home to some very important and interesting memorials however care must be taken when visiting this section due to the size and age of the memorials. Burnley Cemetery provides full burial facilities and has new graves available. The cemetery has a chapel where services may be held if required. The chapel was opened in 1923 and has recently undergone major refurbishment to reflect all religions and beliefs. The chapel has full disabled access and toilets. The Walls of Remembrance Memorial plaques of Lakeland slate bearing a dedication to a loved one are available for a 25 year term. The plaques are attached to the purpose built Memorial Walls which are situated within the Garden of Remembrance at the crematorium. A memorial brochure containing information and application forms for both the book and the walls is automatically sent to the applicant for cremation, if the ashes have been placed in the garden, after the cremation has taken place. Memorial Brochures can also be obtained from the Cemetery Office. The cemetery chapel 6 7
5 The graves available at Burnley Cemetery are as follows: The Lawn Sections The exclusive right of burial for a grave within the lawn sections can only be purchased at the time of burial. Graves within the lawn sections are grassed over for easy maintenance and periodically mown by the cemetery grounds maintenance team. No planting of flowers is permitted on the lawn sections. Conventional Graves The exclusive right of burial for a conventional grave can be pre-purchased for a term of 40 years or purchased at the time of burial. Conventional graves can be planted out with small flowers. However no plastic fences or chippings are permitted and bereavement services has the right to remove any infringements on conventional graves. Ash Plots An ash plot for the interment of cremated remains has recently opened. The plots can take up to four interments of cremated remains. The same rules apply to purchase the exclusive right to an ash plot as they do with a full grave plot. No planting out or surrounds must be placed in front of the plots and the only memorial allowed is a desk type memorial for easy maintenance. 8 9
6 Muslim Sections There are two Muslim sections within Burnley Cemetery, the newest section situated at the top of the cemetery being opened in Grave spaces are no longer available in the lower section, but burial records are kept and are available for inspection at the cemetery office. St John s Public Cemetery, Padiham St John s Public Cemetery, Padiham was opened on 23rd February New graves are available to purchase but only the conventional type (see previous page). Cremated remains can be interred in the ground in front of the Walls of Remembrance. Please contact Burnley Cemetery office for more information. Family Research Bereavement Services receives many requests from people researching their family history. The registers for both Burnley and Padiham Cemeteries and Burnley Crematorium are held at the Burnley Cemetery Office. Bereavement Services staff can conduct searches for you or you may arrange an appointment with the office to come and view the records yourself. Basic burial records on microfiche are also held at Burnley Central Library. Safety Inspections of Memorials Bereavement services are currently carrying out safety inspections on all memorials within Burnley and Padiham Cemeteries. The inspections are being undertaken in accordance with advice issued to local authorities by the Health & Safety Executive and the recent Memorial Safety Guidance 2009 issued by the Ministry of Justice. A yellow safety-warning label is fixed to any memorial that is found to be unsafe. This is to advise owners to contact Bereavement Services and not to touch the memorial unless they are having the memorial repaired by a qualified memorial mason. A list of registered memorial masons can be obtained from the cemetery office. Our memorial inspection team will make memorials safe using a temporary support system, or in some cases laying the memorial flat on the ground or setting it into the ground. Memorials must be repaired using a ground-anchor system. Any new memorial must also be fixed with a ground-anchor system.
