1 Active Choices Offering you the Choices you deserve! activesefton.co.uk Active Sefton is a partnership between Sefton Council, NHS Sefton and the local voluntary and community sectors.
2 Active Choices is a great way to get discounts on leisure activities for children, young people and adults in Sefton. These activities include sports, library activities, golf and bowls to name a few. It also entitles you to a discounted membership rate at any Active Sefton leisure centre. There are two levels of discount available and the level you are entitled to will depend on your circumstances. This brochure is colour coded to help you identify the most appropriate level for you - simply follow the colour code that is identified below. Level 1 Discounts (up to 40%) This level is for anyone who receives additional support as detailed in the level 1 criteria, so to be eligible you must meet the criteria in level 1 stated in the green section. Registered disabled users and their carers automatically qualify for this level. Level 2 Discounts (up to 20%) This level is for anyone, at any time at any facility and features all discounts stated. Blue: Discounts Available. To apply for an Active Choices card there is a short application form available at the back of this brochure, which you can complete and return to any Active Sefton leisure centre or library. If you are interested in applying for an Active Choices card and would like support completing your application, a member of our team will be happy to assist you with your application. Please bring any evidence required (as outlined in the following pages) to your local Active Sefton leisure centre or local Sefton library. Companion Card: All individuals who are registered as disabled are automatically eligible for a level 1 card. If you are registered as disabled and need support to access our activities or leisure facilities, you can obtain a companion card. This card then entitles you to take a relative, friend or carer with you when you visit and both of you are entitled to discounted usage.
3 Level 1 Cost: 6.00 per year Discount: Up to 40% If you live in Sefton and receive any of the support detailed below, then you are eligible for a level 1 card. Alternatively, if you are aged sixty and above and live in Sefton, you automatically qualify. On a level 1 card you can apply for yourself, someone you care for, or for your whole family. If you are a member of the armed forces you are also eligible for a level 1 card. Individual Eligibility Criteria If you would like to apply for a level 1 card for yourself or someone else, you or the person you are applying for need to be: Aged 60 years or above Unable to work because of illness Registered as disabled Current member of the armed forces Family Eligibility Criteria If you or your family receive any of the following support then you may be eligible for a level 1 card: Income Support / Pension Credits Job Seekers Allowance Housing Benefit Council Tax Benefit Child Tax Credit On a New Deal Option (aged 18-24yrs) Partners of above Children of above up to and including 18yrs Children aged 7yrs or under must go on the parent s card; children aged 8yrs - 18yrs can have their own card.
4 How To Apply To apply for your card, all you have to do is provide us with a copy of your eligibility letter / form / stamp / age / proof of address. Some examples are detailed below. Benefit Letter/ DWP Office Stamp for Income Support or Pension Credits Benefit Letter / JSA Office Stamp for Job Seekers Allowance Recent Bill Notification Letter Action Plan Birth Certificate (re children named on a family card or for an individual over 60yrs) Incapacity Benefit Disability Living Allowance Severe Disablement Allowance Attendance Allowance Industrial Disablement Allowance War Disablement Allowance Carer's Allowance Bus Pass (re individual over 60yrs) Please note the names of the support packages may have changed. Level 2 Cost: 3.50 Junior / 6.00 Adult / Family / Group* Discount: Up to 20% A level 2 card is a discount card for everyone - you do not have to meet any criteria and anyone can purchase this card to get discounts to leisure activities across Sefton. You can purchase this card for an individual or for the whole family. Will I Need to Provide Any Paperwork? If you are purchasing a level 2 card, you do not need to bring any paperwork with you. *A group membership entitles discounts to group sports activities, not individual activities.
5 Discounts Level 1 Level 2 Both levels of Active Choices discount card provide a range of discounts on a number of activities. Discounts Available Active Sefton leisure centres: swimming, gym use, classes, sauna / steam, fitness memberships and inductions (level 1 only) and badminton court hire. Be Active, Active Kidz and Active Totz sessions Sefton Libraries: DVD and CD hire Golf (Southport Golf links & Bootle Golf links only) Monday - Friday (not available weekends or bank holidays) Crown and Flat Green Bowls (only available to level 1 holders) Entry to Dunes Splash World, Southport
