To make your visit easier, we have provided this Patient Information Guide. It is designed to acquaint you and your loved ones with the services available at Mercy Hospital and to give you helpful information regarding your hospital stay. If you need additional information or assistance, please feel free to ask any member of our nursing staff. Founded by the Sisters of Mercy in 1904, Mercy Hospital of Buffalo is one of the largest hospitals in Western New York and has the busiest emergency department in the region. Upon completion of a $32 million hospital addition in the spring of 2010, we will open the doors to a state-of-the art emergency facility to respond to the needs of our community. With 24-hour, specialized heart and stroke services, the hospital is home to the Catholic Health Heart Center, providing a full range of cardiac care services including open heart surgery and cardiac catheterization, and operates a Stroke Center designated by the New York State Department of Health. 4 Credentials About Mercy Hospital of Buffalo Mercy Hospital of Buffalo, part of Catholic Health, is one of the largest hospitals in Western New York with the region s busiest emergency room (ER). Mercy Hospital s new $32 million ER, expected to be completed in the Spring of 2010, will triple the size of the current facility and revolutionize emergency care in the area. The hospital is home to the Catholic Health Heart Center providing a full range of cardiac care services including open heart surgery, cardiac catheterization and minimally-invasive interventional cardiology procedures. 5 Patient Safety Mercy Hospital of Buffalo Takes Pride In The Care We Provide. In partnership with you, we wanted to offer a list of ways we can work together to maintain a safe environment. 1) When you are being admitted to the hospital, you will have an identification band placed on your wrist. Offer to show your ID wristband to staff when they enter your room. You can expect staff to check it before giving you medications, drawing blood, or taking you for procedures. If they do not, please ask them to. If your wristband has to be removed for any reason, you can expect the staff to replace it. 2) If you are scheduled for surgery, you can expect staff to confirm the correct location of your surgery and mark the correct site. 3) You can expect nurses, physicians, and other caregivers to cleanse their hands with disinfectant soap or hand gel before and after patient contact. Feel free to ask your caregiver if they have washed their hands before providing you care. 4) You can expect staff to label all blood specimens at your bedside. 5) You can expect staff to introduce themselves and tell you what they are doing. All staff can be identified by their badge, which has their name and photo image. 6) Make sure your physician knows what medications you are currently taking. This includes prescription and over-the-counter medications and supplements such as vitamins and herbs. 7) Alert your physicians and nurses about any allergies you may have had to medications in the past. 8) If you or your family notices a significant change in your condition notify the nurse immediately. Our organization has a trained team of professionals (Rapid Response Team) ready to respond and assist the nursing staff in addressing your condition. 9) If you have suggestions to improve patient safety or have safety concerns, we encourage you to speak with your doctor or nurse. Additionally, you may call to reach the Patient Representative.

6 Amenities Patient Representative The Patient Representative program is designed to help you with any questions or concerns that may arise during your stay with us at Mercy Hospital. If you or your family members have any questions about your hospitalization, hospital policy, or have a special request, please ask to see the patient representative or call Information shared is, of course, handled confidentially. There is no charge for services rendered by the Patient Representative. Telephones Telephone service is available for a daily rental fee of $3.25 per day. Should you choose telephone service, you will be able to receive incoming telephone calls and place outgoing local calls from the telephone in your room. Outgoing calls can be made by dialing 8 and then the telephone number you are calling. Incoming calls can be received between 7 a.m. and 9 p.m. daily. Long distance telephone calls are made at your own expense by dialing 8 and then 0. Please be considerate of your roommate and/or other patients when using the telephone. Cell Phone Usage: The use of cellular phones in the hospital is prohibited in all patient care areas due to the potential interference with some medical life-safety equipment. Please look for designated areas for cell phone use including admissions, main lobby, waiting areas, and cafeteria. Television Television services are available for a daily rental fee of $4.25 per day. A TV/Telephone Service representative will visit your room daily to collect rental fees and to assist you with service. There is no charge for viewing patient and health information channels. Flowers and Mail Any mail, packages, or flowers will be delivered daily to your room. Outgoing mail may be given to your nurse. Mail received after your discharge will be forwarded to your home. Gift Shop The Mercy Tree Gift Shop, staffed by volunteers, is located off the 2nd floor main lobby and is open daily from 9:30 am to 8 pm. The shop carries fresh flower arrangements, cards, magazines, toiletries, candy, balloons, and a variety of gifts. You may reach the shop by calling E-Cards Visit to send your family members and friends an E-Card.

7 Amenities Newspapers Newspapers are for sale in the main lobby. The Mercy Tree Gift Shop has other reading materials. Notary Public If you require notary public services, please contact Mercy Hospital s administrative offices at Special Communication Needs The hospital safeguards the rights of patients with physical challenges and/or special communication needs. All patients are assured equal and effective access to our health care services through qualified sign and foreign language interpreters and necessary auxiliary adaptive devices. 8 Visitor Parking & Visiting Hours Parking & 24 Hour Valet Parking Parking is available for a nominal fee in the hospital s parking ramp located at the corner of Abbott Road and Lorraine Avenue. Handicapped parking is available on all levels of the ramp. The hospital is not responsible for theft or damage to vehicles while parked on hospital property. For your convenience, valet parking is available 24 hours a day, seven days a week, at the hospital s front entrance. Visiting Hours GENERAL: 11:30 a.m. - 8:30 p.m. CARDIAC CARE UNIT/INTENSIVE CARE UNIT/OPEN HEART UNIT: 11 a.m. - noon, 4-5 p.m., 8-9 p.m. (Immediate family members only) PEDIATRICS: Parents may visit 24 hours 7 MCAULEY WEST: 11:30 a.m. - 2 p.m., 5-8 p.m. MATERNITY: Fathers: 24 hours; Grandparents: 11:30 a.m. to 8:30 p.m.; Siblings: by appointment; Open Visitation: 11:30 a.m. to 8:30 p.m. Smoke-free Campus Policy Mercy Hospital is a Smoke and Tobacco Free Campus, which means that smoking is prohibited inside the hospital and on all Mercy Hospital property (hospital and off-site locations), including all buildings, grounds, parking ramp and lots, vehicles, and adjacent sidewalks. Patient Meals Hundreds of meals are prepared daily at Mercy Hospital and each patient tray receives special care and attention. Nourishing and well-balanced meals are an important part of your treatment and recovery. A Registered Dietition works closely with your physician to provide you with individualized nutritional care. You will receive a meal based on your physician s order. Please alert your caregivers to any allergies or food intolerances that you may have. Should a special diet be prescribed as part of your post-hospital treatment, a registered dietition or diet technician may provide instructions so you can properly manage your nutritional needs at home. Outpatient nutrition counseling is also available for individualized nutrition care. Meals are served at the following times: Breakfast: 7:10-9:30 a.m. Lunch: 11:10 a.m. - 1:30 p.m. Dinner: 4:10-6:30 p.m. Guest Trays Our Food and Nutrition Department will make meals available to relatives and friends of hospitalized patients for a nominal fee.

9 Food Services & Spiritual Care Cafeteria Services The cafeteria, which is open daily from 6:30 a.m. to 7 p.m., is located on the third level of the McAuley Building. SUBWAY Restaurant A SUBWAY Restaurant is located off the cafeteria on the third level of the hospital s McAuley Building. Hours are Monday through Friday, 6:30 am to 12 midnight; Saturday/Sunday, 8:30 am to 12 midnight. Vending Machines Vending machines are located in the cafeteria on the third level of the McAuley Building. Vending machine service is available 7 days a week, 24-hours-a-day. Spiritual Care A Message for Patients and Loved Ones Each of us at some time has experienced the illness of a loved one. It can be a very anxious, lonely, and even frightening time for patients and family members. A Chaplain is on call 24 hours-a-day. If you would like someone to talk with you, want to share your gratitude and relief, or simply want someone to be with you during your difficult time, please call us. A Chaplain can be reached through a nurse, any hospital staff member, or the hospital operator by dialing 0. Mercy Hospital Chapel The Mercy Chapel, located on the third floor, is a place of quiet religious beauty for prayer and meditation and is open from 8 a.m. to 8 p.m. daily. All masses are broadcast live on patient televisions, Channel 28. Catholic Mass: Monday - Friday at 12 noon and Sundays at 6:30 a.m. The Spiritual Care Department participates in the Clinical Pastoral Education Program. Student interns work with the Chaplains and assist with visiting patients.

10 Admission What to Bring Bring a driver's license or other proof of identification, as well as appropriate insurance cards, including Medicare and other insurance identification cards, to ensure receipt of proper credit for your insurance coverage. To make your stay more comfortable, we here at Mercy recommend that you bring the following items: Bathrobe Slippers Desired Walking Shoes Nightgown or Pajamas Toiletries The hospital will provide other necessary items. Electric razors and blow-dryers are permitted, however the hospital does not take responsibility for their safekeeping. Mercy Hospital is not responsible for cash, valuables, and personal items you take to your room with you. This includes eyeglasses, hearing aids, dentures, canes, prostheses, wheelchairs, and other easily misplaced items. If you wear dentures, eyeglasses, or contact lenses, you should keep them in protective containers, labeled with your name, when not in use. Never place dentures on meal trays, put them under the pillow, or wrap them in napkins, otherwise they may be easily lost. All jewelry and valuables should be sent home or placed in the hospital safe. If any personal items are brought to you or taken home after admission to your room, notify your nurse who will then add to or delete these items from the original listing of your personal belongings. Any items left behind in rooms not claimed within one month (30 days) after discharge, will be disposed of. Please notify hospital staff if you are parking your vehicle in the hospital parking lot during your stay. Consent for Treatment When you are admitted to the hospital, you will be asked to sign a Consent for Treatment and Acceptance of Conditions of Treatment form. By signing these forms, you enable your physician and hospital staff to give you the care you need during your stay. It also permits release of medical information to facilitate payment of insurance claims directly to the hospital. Hospital Records Upon arrival, information was obtained from you or your family to create your hospital records. While some questions may have seemed personal, all information is necessary for your records. It is important that you provide the name and phone number of a family member or friend we can notify in

11 Insurance & Hospital Bills case of an emergency. You should also consider assigning a health care proxy during your hospitalization in the event you are unable to speak for yourself. For more information on assigning a health care proxy, refer to the publication, Your Rights as a Hospital Patient. Please keep in mind that your hospital record is confidential. It is for hospital or billing use only. Unless subpoenaed by court authority, we cannot report its contents to anyone without your written consent. Financial Arrangements Your Blue Cross, Medicare, Medicaid, HMO, or private insurance must be validated at the time of admission. Patients admitted without insurance coverage are personally responsible for their entire bill, and arrangements for payment will be required prior to an elective admission. The hospital may require patients to sign promissory notes to ensure payment for portions of their bills not covered by insurance. Your daily charges include your room accommodations, nursing care, bedside meals, special diets, linen service, housekeeping, and other hospital-related services. Our billing staff is available to answer any questions you may have regarding your hospital bill. As a courtesy, we file all health care insurance claims for you. Hospital insurance plans do not always provide full coverage of your hospital bill; your hospital insurance coverage is a contract between you and your insurance company. Although the hospital will assist in processing and expediting your claim, you are ultimately responsible for your account with Mercy Hospital. Unless other payment arrangements are made prior to treatment, you will be expected to pay in full the difference for all charges not covered by insurance. We will attempt to secure payment from your insurance carrier. While you are a patient here with us at Mercy Hospital, you will be cared for in a variety of patient settings. Your treatment may involve the services of many different health care professionals. Depending on the tests or procedures ordered by your doctor, there may be several physicians involved in your care and each may bill separately for their services. Not all of these physicians participate in all plans. It is your responsibility to check with your insurance carrier to see which physicians are participating members and choose your doctors accordingly or make any necessary arrangements with your health insurance carrier if a physician you select does

12 Patient Care Services not participate in your plan. You must submit a claim form to your physician, in addition to the one you submit for hospital charges, before your insurance company will process the hospital bill. Please check with your insurance company directly if you have any questions concerning coverage. They make the final decisions on payments. If you have any questions about your bill, please call Ambulatory Surgery Your surgical procedure may be one that meets the criteria for ambulatory surgery. Ambulatory surgery patients are admitted directly to the Ambulatory Surgery Unit and are usually released the same day. If an overnight stay is required, you will be transferred to a nursing unit to receive extended care as directed by your physician. Critical Care Units The 18-bed Intensive Care Unit (ICU) is located on the eighth floor of the McAuley Building. The ICU accommodates medical and surgical patients requiring constant acute care. A waiting room is located next to the unit. The 12-bed Phase I Cardiac Care Unit (CCU) is located on the second floor of the McAuley Building. The CCU accommodates patients needing intensive cardiac monitoring, care, and treatment. A waiting room is located next to the unit. A 6-room Open Heart Unit (OHU) is located on the second floor of the McAuley Building next to the CCU. PLEASE NOTE: Patients in ICU, CCU, and OHU may not receive flowers. Visitors are limited to two at a time during visiting hours. Family BirthPlace The Family BirthPlace completed a major renovation in August 2009 featuring all private patient rooms, including four Celebration Suites, and a new family waiting area. Birthing rooms are equipped for labor, delivery, and recovery, including several rooms equipped with Jacuzzi whirlpool tubs. Modernized patient rooms also feature a private shower and bath, as well as a sleeper sofa for loved ones. The Celebration Suites, available at an additional charge, offer patients and their families all the comfort and amenities of upscale "hotel-like" accommodations to celebrate the arrival of their newborn. The Family BirthPlace also features a full-term nursery and a Level II Neonatal Intensive Care Unit equipped with state-of-the-art technology to provide exceptional care for your baby. The hospital also operates a specialized perinatal testing unit and a parenting education service in conjunction with EPIC (Every Person Influences Children).

13 Patient Care Services Emergency Department Mercy Hospital s Emergency Department is staffed by a team of specially-trained emergency physicians, physician assistants, nurses, and immediate treatment assistants who are committed to providing you and your loved ones with high quality emergency care 24-hours-a-day, 7-days-a-week. Mercy s ER of the Future In the spring of 2010, patients and visitors to Mercy Hospital will be greeted by a new state-of-the-art Emergency Room. The hospital is currently in the process of building a 54,000 square-foot addition to the hospital that will triple the size of the current ER facility and revolutionize emergency care in the region. Upon completion of the estimated $32 million construction project, the new ER will feature all-private exam rooms, including 29 universal treatment rooms, three airborne isolation rooms, and two resuscitation rooms, along with expanded space for nursing and support services. During construction, all hospital services and current emergency services are operating as usual. However, hospital entrances and parking will require temporary modifications to accommodate the construction. Subsequently, we apologize for any inconvenience this may cause you and your loved ones. The Heart Center As Catholic Health s center of excellence for complete cardiac care, the Heart Center at Mercy Hospital brings together the most advanced cardiovascular care and leading-edge medical technology, offering patients world-class treatment, comfort, and convenience. Led by the area s finest cardiology team, the Heart Center is staffed by experienced doctors and nurses who are committed to bringing the nation s best medical practices to their patients- every time, every day. This means the Heart Center consistently offers exceptional care with the lowest mortality rates and best results in the region. For patients, that translates into better outcomes, safer care, shorter hospital stays, and an improved quality of life. The Heart Center provides comprehensive cardiac care services, including open heart surgery, diagnostic and interventional cardiac catheterization, electrophysiology, and a full spectrum of diagnostic testing.

14 Patient Care Services Imaging Services Thanks to a unique partnership with Siemens Medical Solutions, Mercy Hospital, along with the entire Catholic Health System, is raising the bar in bringing the most advanced imaging technology to Western New York. Our experienced staff offers superior service using state-of-the-art equipment providing such testing as 64-slice CT and CTA, MRI, ultrasound, nuclear medicine, non-invasive vascular ultrasound, radiology, and interventional radiology. Pediatric Care Unit Our Pediatric Care Unit is located on the fourth floor adjacent to the Family Birth- Place. A brighter unit is the result of recent refurbishing. The 10-bed pediatric unit provides short stay and overnight care for common medical and surgical problems in children of all ages. A family-focused, family-friendly atmosphere is provided by our staff of doctors, nurses, and ancillary personnel who are trained specifically for pediatric patients. Palliative Care Unit Mercy Hospital offers a specially designed Palliative Care Unit featuring four private rooms, a semi-private room and a spacious family room. Each room features a warm, home-like atmosphere with comfortable furnishings and amenities. Working in partnership with the professionals of Hospice Buffalo, Inc., the unit's staff are specially trained to help patients and their families who are facing the end of life s journey. The unit was recently remodeled and expanded in 2008 through the support of the Mercy Hospital Foundation, the Hospice Foundation, and generous donors. Stroke Services (Designated Stroke Center) Recognizing the first warning signs of stroke and getting expert medical help quickly is critical for successful treatment and recovery. Mercy Hospital has a trained stroke team that provides prompt diagnosis and treatment. Experienced neurologists and other clinical experts lead the team. Specialized C.A.R.E. Unit Mercy Hospital's C.A.R.E. (Comprehensive Admission Reception Expert) Unit is a specialized unit that cares for patients admitted from the ER who are waiting for inpatient beds; patients who are in observation awaiting admission or discharge; patients directly admitted to the hospital; patients requiring outpatient transfusion; and patients recover-

15 Patient Care Services ing from Cath Lab procedures. Designed to improve patient satisfaction and safety, the C.A.R.E. Unit operates 24 hours a day, Monday through Friday. The unit recently expanded from six to ten beds in February 2009 with plans to further expand in the near future. Telephone and television service are complimentary for patients in this unit. State-of-the-Art Telemetry The medical/surgical units at Mercy Hospital of Buffalo have 78 telemetry beds with state-of-the-art telemetry capabilities. There are an additional 6 telemetry beds in our specialized C.A.R.E. unit. For enhanced patient care, telemetry beds provide progressive monitoring and in-house hospital rehabilitation of the cardiac patient. These capacities allow Mercy Hospital to continue as a center of excellence in medical/surgical services and emergency care. Mercy Hospital Foundation The Mercy Hospital Foundation was established in 1978 to raise new funds and steward existing resources which support, improve and promote the care and service provided by Mercy Hospital of Buffalo. Since that time, the Foundation has distributed more than $12 million to benefit the people and programs of Mercy Hospital, including substantial investments toward such projects as the Family BirthPlace, the Catholic Health System s Heart Center located at Mercy Hospital, and the construction of Mercy Hospital s new Emergency Department. Angels of Mercy Program Honoring Outstanding Caregivers The Foundation also sponsors the Angels of Mercy program, which allows patients or their families to make a donation in honor of a caregiver who they feel has done an extraordinary job. Envelopes are provided in your admissions packet and in each waiting room and nursing unit. The associate who is honored receives a special angel pin to wear and is recognized by hospital administration, his or her managers, and colleagues. All gifts in honor of Angels of Mercy go directly back to help purchase equipment and provide continuing education for our caregivers. 18 Patients' Bill of Rights As a patient in a hospital in New York State, you have the right, consistent with law, to: 1) Understand and use the rights. If for any reason you do not understand or you need help, the hospital must provide assistance, including an interpreter. 2) Receive treatment without discrimination as to race, color, religion, sex, national origin, disability, sexual orientation, source of payment, or age. 3) Receive considerate and respectful care in a clean and safe environment free of unnecessary restraints. 4) Receive emergency care if you need it. 5) Be informed of the name and position of the doctor who will be in charge of your care in the hospital. 6) Know the name, positions, and functions of any hospital staff involved in your care and refuse their treatment, examination or observation. 7) A non-smoking room. 8) Receive complete information about your diagnosis, treatment, and prognosis. 9) Receive all the information that you need to give informed consent for any proposed procedure or treatment. This information shall include the possible risks and benefits of the procedure or treatment. 10) Receive all the information you need to give informed consent for an order not to resuscitate. You also have the right to designate and individual to give this consent for you if you are too ill to do so. If you would like additional information, please ask for a copy of the pamphlet "Do Not Resuscitate Orders A Guide for Patients and Families." 11) Refuse treatment and be told what effect this may have on your health. 12) Refuse to take part in research. In deciding whether or not to participate, you have the right to a full explanation. 13) Privacy while in the hospital and confidentiality of all information and records regarding your care. 14) Participate in all decisions about your treatment and discharge from the hospital. The hospital must provide you with a written discharge plan and written description of how you can appeal your discharge. 15) Review your medical record without charge. Obtain a copy of your medical record for which the hospital can charge a reasonable fee. You cannot be denied a copy solely because you cannot afford to pay. 16) Receive an itemized bill and explanation of all charges. 17) Complain without fear of reprisals about the care and services you are receiving and to have the hospital respond to you and if you request it, a written response. If you are not satisfied with the hospital s response, you can complain to the New York State Health Department. The hospital must provide you with the Health Department telephone number. 18) Authorize those family members and other adults who will be given priority to visit consistent with your ability to receive visitors. 19) Make known your wishes in regard to anatomical gifts. You may document your wishes in your health care proxy or on a donor card, available from the hospital.

19 Patients Responsibilities Patient Responsibilities 1) Give Full Information: Tell the staff responsible for your care, everything you can about your condition, including all symptoms, medication you are taking, previous illnesses and hospitalizations. 2) Report Any Changes: Inform the staff immediately about perceived risks in care provided and changes in symptoms, medications or general condition. 3) Follow Your Instructions: Take all medication as prescribes. Listen carefully to instructions about diet, exercise, etc. Tell the staff if you are having trouble following the instructions. Ask questions if you need additional information. 4) Keep All Appointments: Plan carefully and schedule all tests and optional treatments in advance. If you must cancel an appointment, try to give at least a 24 hours notice.
