2 Trade Up To Trex Transcend Rebate Form - Prequalification A. Eligibility and General Instructions - Prequalification 1. All information required on the Prequalification Form should be typed or legibly printed in blue or black ink. 2. In order to participate in the Trade Up to Trex Transcend rebate program, the homeowner and contractor must be pre-qualified by June 30, To prequalify, the homeowner/contractor must a) provide photographs of the composite deck being replaced (1 photo of the full deck with the house, 1 close up photo to identify the product), and b) have the deck verified by an authorized Trex representative. The product information (manufacturer, product, and date installed) should be provided on the form below. Note: Polaroid photographs will not be accepted. Digital photographs may be sent with rebate forms submitted via . Photographs will not be returned. 3. Trex Transcend decking products must be purchased between April 1 through July 31, 2012 and installed by August 31, All rebate materials (Rebate Submission form, completion photographs) must be submitted to the address below by September 15, The homeowner and contractor (if applicable) must sign and date the Rebate form, certifying the information. The authorized Trex representative must also sign the form to certify the product being replaced. 5.
