1 Suite 15, Parkwood Office Centre th Street Tel: (780) March 2016 Mr. Pedro Mendonca Project Procurement Specialst Alberta Infrastructure Street, 2 nd Floor, Suite 2700 Edmonton, AB T6H 5V7 pedro.mendonca@gov.ab.ca RE: Field Inspection Report #34 Week Ending April 1, 2016 JH Picard Catholic School Plan No.: F; Project ID: B3165A-0001 Dear Mendonca, This letter report outlines our field inspections for week ending April 1, There are no regulatory or health concerns to highlight. Air Monitoring air monitoring results indicate that fibre levels were below the set regulatory limit of (0.01 f/cc) for all samples that could be analyzed. All occupational samples were below the respiratory protection factor (10 f/cc) half-face respirator utilized by the workers. On March 31 st the ambient samples on the main floor South corridor was overloaded with debris and unable to be analyzed. This is mainly due to other trades working in the area and the power being off in the building with the potential decrease in dust suppression measures. Inspection Summary: Control Measure Breaches in Containment Mechanical Fixtures and protrusions sealed Wash Station Negative Air Pressure Proper PPE Use of Dust Suppression airless water application 6mil Disposable Bags Designated Waste transfer and storage area Control Measure in Place (/No) No N/A N/A No Date of Deficiency Transferred by Truck Edmonton Kenora Kingston Ottawa Sudbury Thunder Bay Waterloo Winnipeg Vancouver
2 Field Inspection Report #34 J.H Picard Catholic School DST Reference No.: BE-ED Page Regulatory Concerns: No concerns Scheduling: Moderate-risk glove bag abatement completed in the Main and Second Floor South for Phase 3 construction. Site Meeting Notes: March 31 st, 2016 A pre-abatement visual inspection was conducted for the moderate-risk glove bag abatement on the main and second floor South corridor. All precautions were in place. Permission to proceed with abatement was provided. The ambient sample on the main floor corridor was overloaded and unable to be read. This is mainly due to other trades working in the area. This is compounded with the fact that the power supply was turned off in this area, causing the dust suppression controls to be ineffective. April 1 st, 2016 Air monitoring in progress for the main and second floor South corridor glove bag abatement. The laboratory report of analysis indicates there were no procedural deficiencies. This concludes our findings for this week, should there be any concerns please contact the undersigned. Sincerely, Per. Prepared by, Reviewed by, Sean Colquhoun, B.Sc. Picard Catholic Schools Client: Alberta Infrastructure DST File No.: BE-ED Lab Report #: LR-ED Risk Level: Moderate Risk Glove Bag - Main Floor South Contractor: APSI Analysis Date: Air sampling cassettes were submitted to DST's laboratory for determination of fibre concentrations by Phase Contrast Microscopy (PCM). Samples were prepared and analyzed in accordance with the National Institute of Occupational Safety and Health (NIOSH) Method 7400, dated August 15, Analysis was performed using the "A" counting Rules. It should be noted that NIOSH Method 7400 does not differentiate between asbestos and other fibres. Only fibres with a length greater than 5 µm and a length-to-width ratio equal or greater than 3:1 were counted. Fibres with a diameter smaller than 0.25 µm cannot be detected using this method.
