Dear Friends, Philanthropy has the power to change lives by providing greater access to health care throughout our community, improve the patient and family experience, and support innovation and research that will build a better future for us all. Park Nicollet Foundation has more than 40 years of experience in transforming the love and care of a community into programs that grow into legacies that will impact generations. Our Love and Legacy Campaign to support Park Nicollet Frauenshuh Cancer Center is an opportunity to honor life and the human experience by creating a legacy that will give the gift of care and well-being to those in our community who need it most. We are fortunate to serve a community dedicated to making a difference across diverse issues, and Park Nicollet Foundation gives donors and patients the space to channel their giving spirit to the causes closest to their hearts. The enclosed giving guide outlines opportunities for your involvement and highlights key areas in which philanthropic support will be used to impact the lives of Park Nicollet Frauenshuh Cancer Center patients and families. With your help, we can transform our love into a lasting legacy that will change the lives of patients with cancer, and their families, for generations. Every one of us has something to give to make the future better for families living with cancer. Turn your legacy into better health and well-being for all. Together, we are building healthy lives. Sincerely, Robert and Jean Weiss Co-Chairs Frauenshuh Cancer Center Love and Legacy Campaign Committee Mark Wilkowske, MD Medical Director Park Nicollet Frauenshuh Cancer Center Christa Getchell, CFRE President Park Nicollet Foundation 3

Creating a Legacy of Love Our lives are defined by the things we love and the love that we leave behind. A cancer diagnosis is often life changing. It not only has a significant impact on the patients themselves, but also their families and friends. While they learn to cope with a new normal, many patients are understandably overwhelmed. Park Nicollet Frauenshuh Cancer Center offers the support needed to help patients and their families through their physical and emotional journey. Our patient-care model and resources are designed to heal the body, mind and spirit through exceptional medical care and integrative therapies. Our non-moving patient model brings care and services directly to our patients in their own rooms, helping to conserve energy for healing. Park Nicollet Frauenshuh Cancer Center is proud of the world-class care it provides. We are committed to building a lasting legacy of innovative, compassionate care that will grow with our community s needs. Our vision for the Park Nicollet Frauenshuh Cancer Center includes an expansion of our cancer center facility and the continued development of our Survivorship Program to address the special emotional and spiritual needs of cancer survivors. You can make your legacy a reality by making a gift to support the Frauenshuh Cancer Center through the Park Nicollet Foundation Love and Legacy Campaign. This giving guide will help you discern how your gift will make an impact and provide you with an opportunity to offer your support in a way that is most meaningful to you. 4

Survivorship Program Every gift will give patients with cancer the knowledge, skills and confidence to live their life beyond cancer. Completing an initial course of cancer treatment for patients is both a relief and a source of anxiety. The number of individuals in the United States who have a personal history of cancer has tripled over the past three decades. This staggering growth has prompted a national movement to improve the way our health care systems provide care to cancer survivors. Cancer survivors need to be knowledgeable about the type of cancer they have, the type of treatment they have received and the potential late side effects of the treatment they received. They also need emotional support to help them understand and cope with the illness they have overcome. The Park Nicollet Frauenshuh Cancer Center Survivorship Program is an essential extension of our patient-centered care and demand for the program is rapidly growing. With your help, our Survivorship Program will grow to provide all patients and families with the support they need to transition from treatment to life after cancer. $500,000 goal The Survivorship Program will provide support to patient education, wellness and life enhancements by offering programing free of charge or at significantly reduced rates. Program and services will include: Expanded Wellness Program, designed for survivors and supporting health and healing through nutrition and exercise Patient and family mental health support coordinated to meet the unique needs of survivors Development of a Patients Supporting Patients Program Resource Center Library materials focused on survivorship Educational series and materials developed for survivors Nurse practitioner visits coordinated for survivors Emergency basic needs support for patients through vouchers for transportation, groceries and specialized needs. Survivorship report included in medical records for seamless transition back to primary care Survivorship celebration 12 All gifts and five-year pledges, of any size, will make a tremendous impact on the lives of patients and families walking their cancer journey.

When you have been through cancer, you become more sensitive to the pain of others. You have been through it, understand that you have to live with it and start living your life again. You need to do the things you can do and accept the things you can t. Life goes on. 12-year cancer survivor Ronald with his wife Brenda

When we cast our bread upon the waters, we can presume that someone downstream whose face we may never see will benefit from our action, even as we enjoy the gifts sent to us from a donor upstream. - Maya Angelou 14
