1 Directory for Substance Misuse s in Caerphilly

2 Background Substance Misuse services use a tiered approach in their approach and delivering of drug/alcohol services. These are as follows: Tier 1 Interventions include provision of drug related information and advice, screening and referral to specialised drug treatment. (Police, voluntary agency, health promotion) Tier 2 Interventions include provision of drug related information and advice, triage assessment, referral to structured drug treatment, brief psychosocial interventions, harm reduction interventions (including needle exchange) and aftercare. (Voluntary Agencies) Tier 3 Interventions include provision of community based specialised drug assessment and co-ordinated care planned treatment and drug specialist liaison. (Statutory Health, Independent Health, Social services) Tier 4 Interventions include provision of residential specialised drug/alcohol treatment, which is acre planned and care coordinated to ensure continuity of care and aftercare. (Health, social services) 2

3 s in Caerphilly CAERPHILLY SOCIAL SERVICES DRUG AND ALCOHOL TEAM Provides a service to people over 18 with a substance misuse problem via: Assessment Advice and support Gateway and assessment for suitability for residential rehabilitation Are requesting residential rehabilitation Have a diagnosis of mental illness Have children who are assessed as being in need, or in need of protection Are assessed as posing a risk to others Are people who are defined as vulnerable adults Have been assessed as requiring community care services In addition to this they provide the FWD group (Families with Dependence). The aim of this group is to support and advise parents on improving their ability to recognise the impact that their drug or alcohol misuse has on their child s development. Referral/Access All referrals must be made through the Adult s Duty and Information Team (ASDIT) on DRUGAID Drug aid is a registered charity offering a range of therapeutic and information services, including Assessment Information and advice 3

4 Relapse prevention and rehabilitation programmes Therapeutic support/ intervention for drug and alcohol users Fixed site needle exchange Training education Alternative therapies e.g. ear acupuncture All individuals over the age of 18 with a substance misuse problem (Drugs and alcohol) Referral/ Access Referrals can be via other substance misuse agencies, Tier 1 agencies such as GP, Probation, Housing etc or self-referral. Opening hours are 9am-5pm St Fagan s House, St Fagan s Street, Caerphilly Tel: (029) office@drugaidcymru.com Website: DRUGS AND ALCOHOL FAMILY SUPPORT (DAFS) DAFS deliver a coordinated provision of supportive environments for the sharing of experiences to any person who is affected by or concerned about someone else s substance misuse (including alcohol) of people in similar situations, telephone, face to face or in groups. It increases parents, family and friends awareness of effective, timely treatment for substance misusers, particularly for young people in vulnerable groups. The service is delivered by: Telephone or face to face support to parents and families Provide information and training to organisations that includes families Facilitate the delivery of a coordinated and self sufficient self help family support group Help families and friends understand the contributing factors to their families drug misuse 4

5 Any person, who is affected by or concerned about someone else s substance misuse. Referral/Access Referrals can be via other substance misuse agencies, Tier 1 agencies such as GP, Probation, Housing etc or self-referral. Fusion, First Floor, 42 Cardiff Road, Caerphilly Tel: (0800) (Freephone for clients only) admin@fusion-online.org.uk GP CONSORTIUM COMMUNITY PRESCRIBING This service is to stabilise the service users on appropriate substitute medication to alleviate withdrawal and cravings to reduce the use of illicit or non-prescribed drugs until he/she can achieve a drug free life with the support of the Open Access. This includes: Engaging and maintaining substance misusers in treatment Improving and supporting a client s motivation Reducing substance related harm Promoting service user involvement Ensuring drug testing is available from commencement of service Testing and vaccinating against blood borne viruses (BBV) 5

6 Have drug-related problems Are adult substance misusers who require specialist and / or structured treatment (i.e. those aged 18 years or over) Clients with less severe mental co-morbidity Partners of pregnant users should also be considered a priority, for example where a couple require treatment and then prescribing for only one of them may compromise treatment for the other Where there are child protection issues Chaotic injectors, long-term injectors, those using high risk injection sites People with a very short drug using history may be suitable for immediate rapid detoxification and where delay may lead to a more entrenched drug problem Priority and Prolific Offenders Who have been mandatory drug tested as part of the DIP All prison leavers Clients already in engaged in prescribing treatment (e.g. breaching a DRR, from prison or out of area) Referral Referral is via the Open Access (GOALS) and although delivered from bases throughout Caerphilly, the main base is St Fagan s House Caerphilly. 7 GWENT ALCOHOL PROJECT This service provides open access information, advice, Counseling and support services to alcohol users. This is delivered by: Providing a wide range of information on alcohol and their effect to alcohol users, concerned others and other professionals. Providing a range of accurate, appropriate, and factual information. This should be accessible and meaningful in terms of context, language, literacy level and comprehensibility. 2 Providing alcohol misusers with information, advice and support to remain healthy, until, with appropriate support, he or she can achieve an alcohol-free life. Providing structured support to reduce problems related to alcohol misuse, including health, social, psychological and legal problems. Providing access to practical supportive measures for harm reduction and for accessing health and social care. Providing advice and support to reduce the dangers associated with alcohol misuse Providing advice and support for reducing alcohol-related death. Alcohol misusers who require open access treatment. This may include engaging with their families, significant others, the wider community and other professionals. Clients with alcohol only issues. Access/ Referral Referrals can be via other substance misuse agencies, Tier 1 agencies such as GP, Probation, Housing etc or self-referral. Opening hours are 9am-5pm 7

8 Gwent Alcohol Project, First Floor, 42 Cardiff Road, Caerphilly Tel: (029) Drugaid Substance Misuse Drop in Centre This service provides open access information, advice, Counseling and support services to Substance Misusers with a focus on Stimulant/ cocaine use by: Providing easy to access crisis intervention and fast response harm reduction support and service delivery. Deliver services specifically developed to include seldom seen drug and alcohol users in particular stimulant users and women. This will include out of hours services, refreshments and food, crèche facilities, debt advice, diversionary activities, health care and psychosocial interventions. Accommodate different cultures of substance users through timetabled activities, separate entrances, a well-managed centre, well-trained staff and service users involvement. Provide facilitated structured group work as well as self help groups delivered through a new peer support scheme Training for service users for service user involvement activities and progression routes out of treatment. Diversionary activities including art and music to enable the development of the individuals involved as well as the centre itself. Up to date information provision and service users access to facilities such as the internet and telephones for personal research, self help and empowerment. Access to a range of partner agencies including domestic violence services, mental health care services, benefits services, education and employment services, complementary therapies and family support. Any individual with a substance misuse problem (drugs and alcohol with a main focus on stimulant misuse) 8

9 Access/Referral This service is delivered via a drop in centre, thus anyone can attend and access services delivered. Assessment will be completed for optional referral into mainstream services. The service is delivered evenings and weekends. Substance misuse drop in, St Fagan s House, St Fagan s Street, Caerphilly Tel: (029) DRUGAID HOME DETOX SERVICE To provide alcohol dependant residents with a community based medical and therapeutic intervention, which within a 10-week programme has the aim of helping them to make, and maintain positive changes in their alcohol use. The programme will include assessment, preparation for change, alcohol education, lapse management and medical supervision of withdrawal symptoms. The service offers: Advice, guidance and information on detoxification Screening for suitability for Home Detox Pre-treatment interventions Build self-belief Liaison with the relevant prescribing service Support for home and Community Detox Preparation and after care plans for clients Advice and guidance for carers Advocacy Home visits Holding whilst clients await entry into relapse prevention Liaison with housing providers It also: Engages alcohol dependant members of the community in a shared care alcohol programme. Supports clients in reducing they re drinking in the long term. Encourages and support clients in setting short-term goal of 3-6 months abstinence from alcohol. 9

10 Encourages clients to engage in community services relating to training and education and employment where appropriate. The is provided for adult alcohol misusers who require Home Detox treatment (i.e. those 18 years of age or over). This may include engaging with their families, significant others, the wider community and other professionals (e.g. teachers, generic social workers, primary care staff, criminal justice agencies etc). This is delivered by: Providing a wide range of information on alcohol and their effect to alcohol users, concerned others and other professionals. Providing a range of accurate, appropriate, and factual information. Providing alcohol misusers with information, advice and support to remain healthy, until, with appropriate support, he or she can achieve an alcohol-free life. Providing structured support to reduce problems related to alcohol misuse, including health, social, psychological and legal problems. Providing access to practical supportive measures for harm reduction and for accessing health and social care. Providing advice and support to reduce the dangers associated with alcohol misuse Providing advice and support for reducing alcohol-related death. 10

11 Gateway to residential rehabilitation or inpatient detox Links to A&E Controlled drinking programmes Alcohol misusers who require open access treatment or structured alcohol treatment. This may include engaging with their families, significant others, the wider community and other professionals. Clients with alcohol only issues. Access/ Referral Referrals can be via other substance misuse agencies, Tier 1 agencies such as GP, Probation, Housing etc or self-referral. Opening hours are 9am-5pm St Fagan s House, St Fagan s Street, Caerphilly Tel: (029) GWENT OPEN ACCESS LOCAL SERVICES (GOALS) The Open Access engages substance misusers in a continuum of service provision in order to reduce substance related harm and improve access to more rewarding and fulfilling lives. The service aims: To target high risk and local priority groups To engage substance misusers in treatment To reduce substance related harm To improve and support a client s motivation To promote and reward service user compliance To appropriately assess clients to allow individualised care pathways to be developed with clients To use recognised counselling approaches that support reintegration To help clients rebuild social networks outside their drug using community To ensure all clients are contacted if they disengage with the service from commencement of service delivery 11

12 The is provided for adult substance misusers who require open access treatment and / or care coordination including care coordination for Tier 3 s (i.e. those 18 years of age or over). This may include engaging with their families, significant others, the wider community and other professionals (e.g. teachers, generic social workers, primary care staff, criminal justice agencies etc). Clients with drug and / or drug and alcohol problems not alcohol alone. Access/Referral Self referrals and referrals from other agencies and professionals will be actively encouraged The will be accessed by clients by appointment and by drop-in St Fagan s House, St Fagan s Street, Caerphilly Tel: (0870) GWENT SPECIALIST SUBSTANCE MISUSE SERVICE (GSSMS) This service is to stabilise the service users with complex needs on appropriate substitute medication to alleviate withdrawal and cravings to reduce the use of illicit or non-prescribed drugs until he/she can achieve a drug free life with the support of the Open Access service. GSSMS deliver the following: Prescribing for opiate dependence (titration, community detoxification and maintenance regimes) Prescribing for withdrawal from opoids Specialist prescribing for problematic use Stabilisation and withdrawal from sedatives Community needle exchange Offer HIV and Hepatitis testing or referral to appropriate services Offer Hepatitis immunisation Gateway to Inpatient detox or referral to assessment for residential rehabilitation 12

13 Have generic drug related problems Are adult substance misusers who require specialist and/ or structured treatment: and 1. Have severe and enduring mental health issues 2. Are pregnant drug users 3. Families involved in child protection 4. Those who are HIV symptomatic 5. Chaotic homeless users Access/referral Self-referral or via open access services. The service is delivered 9am-5pm but also offer some out of hours and weekend hours The Old School Building, Mill Road, Caerphilly, CF83 3FD YOUNG PERSONS SPECIALIST SUBSTANCE MISUSE SERVICE (YPSSMS) YPSSMS works with young people and families within an appropriate environment establishing the Tier2 and Tier3 model for substance misuse services for children and adolescents. It delivers this service by: Facilitating young people to access the service Adopting a multi agency approach to young peoples needs Reduce the misuse of alcohol and drugs by young people through tailored services Provide young people and their families with appropriate support Develops the links between agencies to provide eservices for dual diagnosis and vulnerable young people Have generic drug/ alcohol related problems 13

14 Are young substance misusers (under 18 years of age) who require specialist and/ or structured treatment Access /referral Self referrals and referrals from other agencies and professionals Young Persons Substance Misuse BARNADOS SUBSTANCE MISUSE YOUNG CARERS The Young Carers Substance Misuse reduce the exclusion of young carers in Caerphilly in respect of those aspects of positive childhood experience and personal development to which they have a right. They deliver the service by: Offering individual care to young carers through assessed, planned, reviewed and focused task-based work to enable young carers to lead positive and healthy lives. Delivering group activities, which will be a mixture of issued based, and free time over a twelve-week period. This will include trips and activities during school holiday times. Providing peer support, encourage the sharing of common experiences and development of coping strategies Building the confidence and self-esteem of children and young people who have substance-misusing parents. Providing education about substance misuse, addiction and the types of treatment. Ensuring equality of access across Caerphilly, taking into consideration the urban/rural nature of the locality Developing and support partnership working 14

15 Children and young people under the age of 18, who have parents who are misusing substances. Access/ referral The will be based in Risca but will operate from separate bases situated throughout the region to ensure equitable access. Referral is via Self referrals and referrals from other agencies and professionals Barnardo's Caerphilly s MONITORING AND PRESCRIBING CLINICS (MAPS) This service is to stabilise the service users on appropriate substitute medication to alleviate withdrawal and cravings to reduce the use of illicit or non-prescribed drugs until he/she can achieve a drug free life with the support of the Open Access. This includes: To engage and maintain substance misusers in treatment To improve and support a client s motivation To reduce substance related harm To promote service user involvement To ensure drug testing is available from commencement of service Also to: Prepare service users for substitute prescribing, where appropriate. This includes advice and written information about methadone, buprenorphine or other pharmacotherapy, alcohol use, risks of overdose, drug interaction etc Ensure that service users are able to access pharmacy supervised consumption throughout the Gwent area Have a written care plan for all service users, that is subject to a minimum of a quarterly review in conjuncture with the client 15

16 Have drug-related problems Are in the GP registered population within Caerphilly LHB area and those homeless people for whom the Local Authority has accepted a duty of care and housing. Are adult substance misusers who require maintenancebased treatment (i.e. those aged 18 years or over Access/referral Clients of maintenance programmes who are adult drug users having been assessed as requiring symptomatic treatment for drug-related problems. The service is mainly 9am-5pm St Fagan s House, St Fagan s Street, Caerphilly Tel: (0870) DRUGAID YOUNG PERSONS SERVICE The Young Persons delivers an individualised and planned programme of assessment; intervention and care for certain identified children and young people who are misusing substances. The service aims to: To reduce factors in substance misusers lives that may be maintaining their use of substances, and developing their resilience through one to one support and counselling. Meeting needs that are consequential on their misuse of substances or the secondary effects of becoming involved in lifestyles in which substance misuse is a part (Including involvement in crime, poor housing and homelessness, unemployment and failure in school) To provide peer support, encourage the sharing of common experiences and development of coping strategies To build the confidence and self-esteem of children and young people who are misusing substances. To provide education about substance misuse, addiction and the types of treatment 16

The service aims: To assist in preventing the onset of substance misuse with young offenders and in doing so, assist in the Youth Offending service s objective of reducing the rate of offending and re-offending by young people. To reduce factors in substance misusers lives that may be maintaining their use of substances, and developing their resilience through one to one support and counselling. 17

To provide practical support in helping the user to access treatment Children and young people under the age of 18, who are substance misusers receiving a service from Caerphilly Youth Offending Team Access/Referral Referral is via Self referrals and referrals from other agencies and professionals. The service is delivered 9am-5pm but also offer some out of hours and weekend hours. The Provider will adopt a proactive approach to help service users attend appointments, this will include offering appointments in a location which is convenient to the service users 18
