3 Dear Student, Thank you for your interest in the PIH Family Medicine Residency PA/NP Clerkship. Our residency has a proud tradition of training medical students, Physician Assistant students, and Nurse Practitioner students from all over the country. We are proud of graduating more than 150 Family Medicine physicians and serving Whittier and its surrounding communities since The medical staff at PIH enjoys participating in the education and training, and as a former resident of this program, I can attest to the quality of training and preparedness residents and students receive. Our graduates have gone on to practice Family Medicine in rural and urban settings, Indian health practices, the inner city, and academic centers. The vast majority of graduates practice in Southern California. Many remain in the immediate areas and become members of the hospital s medical staff, taking on the role of mentor to our visiting students and staff residents. We have a number of staff NP and PA providers in our practice, and are also one of the few residency programs to have a Physician Assistant on our faculty. Students who choose their Family Medicine clerkship with us have a wealth of educational opportunities at their disposal such as, the Palliative Care Team, Pediatric and Adult Inpatient Hospital Care, a Tropical Medicine Program, and a Skilled Nursing Facility and Geriatric Home Visit Program to name just a few. A comprehensive didactic lecture series is also available, as well as community medicine projects and outreach. 4 THE PIH FAMILY MEDICINE CENTER The Family Medicine Center, established in 1979 within PIH, recently relocated to a medical building next to the hospital. The Family Medicine Center consists of approximately 10,000 square feet, which is dedicated to the office practice of the Family Medicine residents and faculty. There is a waiting room, business office, 3 nurses stations, 3 procedure rooms, and 18 consultation examination rooms. Videotaping and monitoring is available to enhance the teaching experience. In addition to the clinical area, medical education and facility offices are contained within the same unit. The center census is approximately 2000 visits per month, averaging 100 patient encounters daily. OVERVIEW Stand alone unopposed residency in a 444-bed state of the art hospital Stable program established and fully accredited for over 26 years Multi-disciplinary team of Physicians, PA s, NP s LCSW, Medical Students, and Residents Fully staffed Emergency Room One-on-one teaching from Attending Physicians Three Critical Care Units with Family Medicine continuity Family Medicine Center on hospital campus Family Medicine Tropical Medicine Program Family Medicine Palliative Care Program Family Medicine Home Visit / Geriatric Program HISTORICAL PERSPECTIVE Whittier was established in approximately 1887 as a small Quaker town. Health care in this community has centered around the family physician. Presbyterian Intercommunity Hospital (PIH) opened in 1959 with the majority of its patients being admitted and cared for by general practitioners. In the early 1960 s, university teaching programs were strongly embracing a concept of specialization and sub-specialization and the Whittier community followed suit. Later, in the 1960 s a rekindling of general practice occurred with the birth of a new specialty, Family Medicine. This new specialty was associated with a three-year residency program. The American Board of Family Medicine was developed and written boards followed the residency training to accredit individuals in Family Medicine. By the late 1970 s, PIH was looking for new and younger physicians in primary care specialties to expand the primary care services in the Whittier area. At the same time, the USC School of Medicine wanted to utilize community hospitals as Family Medicine residency programs. In 1979, after much planning, a combination program, the USC- PIH Family Medicine Residency Program, was begun utilizing intern training at LA County-USC Medical Center and two subsequent years at PIH. The PIH medical staff and administration worked to establish this program so that the new breed of family physicians would be trained locally and hopefully choose this area as a future practice
5 site. The partnership continued with USC until 2001, when PIH grew into a freestanding community residency program. In 2005 PIH teamed up with the University of California Irvine, and now enjoys a strong academic affiliation with the University and its Department of Family Medicine. The program saw patients for the first time in July of 1979 at which time there were sixsecond year residents and six first year residents. The first Family Medicine Center, located within the hospital, was built using private donations and hospital funds, and encompassed 5,300 square feet of space. The Family Medicine Center moved to its current location in the medical office towers located next to the hospital in January This state of the art facility encompasses the entire fifth floor and houses 20 patient examination rooms, 2 treatment rooms, 2 nursing stations, a large resident office, and faculty and administrative offices are on a separate floor. There are approximately 25,000 outpatient visits each year and 5,800 in-patient encounters. Patient demographics are similar to that of the private practitioners in Whittier. PIH Ed L. 6 PIH Family Medicine Residency PA/NP Student Rotation Welcome to the PIH Family Medicine PA/NP Clerkship Rotation. This rotation provides the student with exposure to the breadth and depth of the practice of Family Medicine. The student will begin to provide healthcare for a variety of patients and assume responsibility for problem oriented and preventive care across all medical disciplines. The student will also be introduced to principles of case management including discussion and decision-making around End of Life concerns. Students are expected to have basic medical knowledge consistent with their experience and level of training. Durin the rotations students will be expected to be self directed in their learning, improve their clinical skills, and develop a stronger foundation of medical knowledge. In the interest of making this the best possible experience for you, we have outlined some of the objectives for your time with us. This is not intended to confine you, but to give you some reachable goals. We encourage you to let us know if you have special interests and would like to spend more time in a particular area. ACHIEVABLE OUTCOMES The student will: 1. Learn how to provide competent and compassionate care to patients. 2. Identify the chief complaint, take a pertinent history, establish a problem list and develop a differential diagnosis and treatment plan. 3. Learn to apply basic principles of Evidence Based Medicine. 4. Become familiar with preventative care recommendations. 5. Learn the advantages of and participate as part of a multidisciplinary healthcare team. DUTIES AND RESPONSIBILITIES Attend daily morning report (you may leave after Peds MR if you do not have inpatients assigned) Participate in patient care at the Family Medicine Center when assigned o Introduce yourself to the preceptor o Ask before you pick up a chart to see a patient o Discuss patient to the PMD before presenting to the preceptor o Charts must be completed before you see the next patient o All outpatient charts must be co-signed by the PMD Participate in the Family Practice Inpatient Service (FPIS) o Take night call during the FPIS part of your rotation o Admit patients to the service when assigned by the Senior Resident o Write daily progress notes, discuss findings and plan with team members (ALL PROGRESS NOTES WILL BE CO-SIGNED BY THE SENIOR RESIDENT, ALL ORDERS WILL BE WRITTEN BY THE RESIDENT)
7 o Sign out to the team when you leave the hospital for home or to another assignment Prepare and deliver a brief topic discussion on a medical problem or interesting patient case (usually at Morning Report) Dress and Behave Professionally o The manner of dress should be appropriate for this institution, which includes a student jacket and nametag and, for men, a tie. Behavior should always be professional. Actively Seek out Evaluation o You should expect continual feedback and an end of rotation summation of your performance. It is your responsibility to meet with the faculty in charge to go over your end of rotation evaluation. Attendance o Most students have such a short amount of time for their rotation with us. Every effort should be made to not miss any of your assigned time with us. If you are unable to attend, please advise Martha Perez as soon as you are aware of a potential absence (562) x8516, as well as your In-Service team senior resident when assigned to the FPIS. Other duties may be assigned as interesting or seasonal activities present, (i.e., breast cancer screening, school age immunization, etc). SCHEDULE* First Day: Meet with Martha Perez at 8:30 am Ste. 301 to pick up badge & Schedule Meet with assigned Faculty at 9:00 am for orientation / tour & patient assignment FPIS Morning report Daily at 8:30 am in room E in the Conference Center Morning rounds with FPIS team See & write progress notes on assigned inpatients. Be prepared to present at morning report Participate in at least one call during the FPIS part of your rotation Geriatrics Thursdays, 8:30 am with Geriatrics Team (as assigned) Attend at least one geriatric home visit Community Medicine Community resource visits (2 to 4 sessions) arrange with Donna King, RN Home based care (at least one visit) arrange with Dr. Nelson Dalla Tor Family Medicine Center As assigned by Clerkship Coordinator or faculty between 8:30 am to 5:30 pm. You will be assigned to a resident or faculty in the office and will see three or four of his/her patients and discuss their management with him/her. Meet weekly to discuss patient s seen, present workups, and review your literature reviews. Conferences Attend daily conference over the lunch hour (locations noted on Weekly Activities Schedule) * Schedule subject to change based on interests of applicant and other residency scheduling issues
