3 Our Dermatology Specialists Kevin L. Smith, M.D. Dr. Smith is a native of Atlanta, and has been practicing Dermatology in Gainesville since He graduated from the University of Georgia in He completed medical school at the Medical College of Georgia, and his residency in Internal Medicine at the University of Florida. Following a year of practice, Dr. Smith returned to the Medical College of Georgia and completed his Dermatology residency in He received his Board Certification in Dermatology in Dr. Smith is a Fellow in the American Academy of Dermatology. He is also a member of the American Medical Association, the Medical Association of Georgia, the Hall County Medical Society, and the American Society for Dermatologic Surgery. Michael B. Reynolds, M.D. Dr. Reynolds is a native of Atlanta, and has been practicing Dermatology in Gainesville since He graduated from Georgia Tech in He completed his medical school and Dermatology residency at the Medical College of Georgia, where he was Chief Resident. He received his Board Certification in Dermatology in Dr. Reynolds is a Fellow of the American Academy of Dermatology, as well as a member of the American Medical Association, the Medical Association of Georgia, the Hall County Medical Society, the Alpha Omega Alpha Honor Society and the American Society of Cosmetic Dermatology & Aesthetic Surgery (ASCDAS). Dr. Reynolds is also an Assistant Clinical Professor at the Medical College of Georgia.

4 Vanessa Marcy, PA-C Ms. Marcy, a native of Minnesota, joined our practice in She completed her Physician Assistant training at Emory University and received her certification in Ms. Marcy has focused the majority of her practice on Dermatology and Pediatrics. She is a member of the Georgia Dermatology Physician Assistants Association as well as the American Academy of Physician Assistants. She is fluent in Spanish. Davina Braselton, PA-C Davina Braselton, a native of Escondido, California, relocated to Gainesville, Georgia in She completed her undergraduate studies at Gainesville College and the University of Georgia. Ms. Braselton completed her Physician Assistant training at the Medical College of Georgia, and received certification in She has maintained a primary focus in the area of Dermatology since the completion of her training. Ms. Braselton joined our Gainesville office in She is an active member of the American Academy of Physician Assistants, Georgia Society of Physician Assistants, Georgia Association of Physician Assistants and the Georgia Dermatology Physician Assistants Association.

5 Payment for Services Payment for services is due at the time of the visit unless other arrangements are made in advance. We are happy to file any applicable insurance claims. However, the patient and/or guardian is financially responsible for all services rendered. We participate with several insurance plans. However, it is the patient s responsibility to verify our status with their policy prior to their visit. Additionally, it is important for all patients to understand their insurance benefits, including deductibles and co-payments, prior to the visit. Phone Calls We welcome phone calls from our patients. Please be aware that it is sometimes difficult to return calls during a busy clinic day. In the event you reach our voice mail, please leave a message with contact information, and we will return your call as soon as possible. If your call is for a prescription refill, please include the name of the prescription, your pharmacy, your date of birth, and current contact information in your message. Appointment Times We schedule visits by appointment only. Please help us stay on schedule by arriving for your appointment on time. If you are more than 15 minutes late for your appointment we will ask you to reschedule. If you must cancel an appointment, please do so at least 24 hours in advance. As with any medical practice, emergencies can occur, requiring attention. We appreciate your patience.

6 Office Hours, By Appointment Office visits are scheduled by appointment only. Emergency work-in appointments are scheduled by a nurse. We cannot accommodate walk-in visits.
