Our physicians and staff strive to offer personalized care to each patient while maintaining the level of compassion and respect each person deserves. Our focus is on healing the whole person and our mission is to provide care for the mind and the spirit, as well as the body. Please know that our staff is always here to answer any of your questions and schedule your next appointment or procedure. About Medicine Specialists of Florida Hospital: Medicine Specialists is part of Florida Hospital s Internal Medicine Residency Program. It is a practice of internal medicine physicians specializing in the diagnosis and comprehensive care of adults with chronic illnesses, infectious diseases and complex health issues. The practice consists of 18 physicians, both on-staff faculty and residents, working together to consider all parts of an adult s health. As a teaching facility, we are up-to-date on all the latest, cutting edge medical information, education, techniques, and procedures. You may see a resident physician whom has already graduated from medical school and is here for additional training and experience in a supervised setting. Each resident physician is overseen by an experienced faculty physician, so you essentially receive two opinions. You can expect your physician to be extremely thorough and take the time needed to answer all your questions. Prescription Refills: For your safety and efficiency, prescription refills may be requested only by your pharmacy. Please call your pharmacy to request the refill, and the pharmacy will fax us a refill request. When requesting a NEW prescription, or a change to an existing prescription, please remember that we need your name, date of birth, your phone number, and the pharmacy phone number. Please allow hours for your prescription to be approved and filled. Directions: I-4 EAST I-4 WEST (towards Daytona Beach) (toward Disney/Tampa) Take I-4 East to exit 85 Princeton St Take I-4 West to exit 85 Princeton St Turn right onto Princeton St. Turn left onto Princeton St Make the first left onto Orange Ave. Continue thru the stop lights until you reach King St. Make a right onto King St and you may park in the King St parking garage for patients/visitors. After parking, take the elevator to the 3 rd floor of the Medical Plaza in order to reach the North Tower elevator. Please take the North Tower elevator in the Medical Plaza to the 2 nd floor in order to reach the Family Health Center in Suite # 235.
2 YOUR FIRST APPOINTMENT Your appointment is on M TU W TH F, The physician you will see is: Check-in time: Appointment time: Please arrive at your check-in time, 15 minutes prior to your appointment. This allows us time to process any additional information that may be required. If you are late for your appointment time it may be necessary for your appointment to be rescheduled, or your appointment may be delayed to accommodate patients who were on time. Please call the office, (407) , if you are delayed beyond your control. Please review the list below to prepare for your first appointment: Bring your insurance card and/or your current insurance information: We cannot verify your eligibility or benefits without this information. Obtain your medical records from previous physicians: This is particularly important if you have any pre-existing conditions or chronic conditions. Complete your Medical History Form: The form attached below provides Medicine Specialists with you and your family s medical history. It also provides our staff with your current health maintenance, including medication and recent procedures or conditions. (Please feel free to bring the actual medication bottle(s) with you to the appointment). If you are unable to keep your appointment, please call (407) at least 24 hours in advance to cancel or reschedule.
