1 CITY OF HARRISBURG COMPREHENSIVE BENEFITS SUMMARY Management Employees Health Care Medical Medical coverage becomes effective on the first day of the month following the date of hire. Employees who document to the City that they are adequately covered by other health insurance may, upon providing substantiating documentation and a written request to the City, opt to receive a monthly reimbursement in the amount of $ (taxable) in lieu of medical coverage for each month that medical coverage through the City would have been in effect. This reimbursement is normally issued the first payroll of each month. All employees hired on or after February 1, 2008 will contribute a percentage of their base salary pay toward the cost of health care benefits as follows: Single Coverage: 2% Two Person Coverage: 4% Three Person Coverage: 5% Four or More Person Coverage: 6% Such contributions will become effective on the employee's date of hire and will be deducted beginning with the employee's first paycheck. Highmark Blue Shield APPO Blue 100@ The PPO Blue program includes most preventive services and has a $100 deductible (individual) or $250 deductible (family) on certain services. Most physician office visits and preventive care visits require a $20 co-payment per visit, provided the services are with a participating provider. Emergency room services require a $50 co-payment which will be waived if admitted. For a more inclusive list of PPO Blue 100 benefits, please consult your benefits handbook.

2 DENTAL COVERAGE (United Concordia Toll-free) Dental coverage becomes effective on the first day of the month following the date of hire. Listed below is a brief description of the benefits. The Basic Program Diagnostic Services...100% UCR (examination, x-rays) Preventive Services...100% UCR (teeth cleaning, fluoride for children) Minor Restorations...100% UCR (fillings) General Services % UCR (repair of dentures, simple extractions, root canal therapy) Oral Surgery...100% UCR (surgical removal of teeth) Prosthetics, Crowns, Inlays and Onlay Restorations...85% UCR Periodontics % UCR Orthodontics... 85% UCR ($1,300 lifetime maximum coverage; dependent children only to age 19) Maximum of $1, per person for all covered services provided in a calendar year. It is to the employee's advantage to use UCCI participating providers since they must accept UCCI's allowance as payment in full for covered services. The employee is responsible for any co-insurance, amounts exceeding the maximum, and/or any service not covered by the City's provider. A non-participating provider may charge the employee the difference between its fee and the UCCI allowance in addition to any co-insurance, amounts exceeding the maximum, and/or any service not covered by UCCI. VISION COVERAGE (National Vision Administrators ) The vision coverage offered is National Vision Administrators (NVA). This benefit becomes effective after three months of employment. Please consult your benefit booklet for a more detailed description of benefits. NVA is a preferred provider network. That means when you go to a preferred provider, you will receive coverage for standard services. Preferred providers also give discounts on additional pairs of lenses and frames, and on optical accessories. Furthermore, preferred

3 providers file the claims for the patient. If the employee chooses to go to a non-preferred provider, the employee will have to pay additional money out-of-pocket for covered services and will not receive any discount on additional glasses, contacts or optical accessories, and may have to file the claim themselves. For a detailed list of your NVA benefits, please see the attached summary. PRESCRIPTION DRUG COVERAGE (Express Scripts Inc.) The prescription drug program is currently administered through Express Scripts. This benefit becomes effective after three months of employment. Covered Drugs: Your prescription benefit plan includes coverage of the following: Federal Legend Drugs (medication which requires a prescription written by doctor by law), Insulin, Insulin Needles/Syringes, Diabetic Diagnostic Supplies, Oral Contraceptives and Prenatal Vitamins. Limits: For prescriptions filled by retail pharmacies, you are allowed up to a thirty (30) day supply. For each mail order prescription, you are allowed up to a ninety (90) day supply. Refills: Refills will be allowed when 80% of the previous order would have been consumed. Co-Payment: For employees hired on or after 2/1/08: Retail Mail Generic $ 0.00 $ 0.00 Brand Restricted (no generic available) $25.00 $50.00 Brand $30.00 $60.00 Network Pharmacies: You will find that using an Express Scripts participating pharmacy is the most convenient way to use your prescription benefits. We strongly encourage you to use only participating pharmacies for the best service and to help contain costs. With over 42,000 participating pharmacies, there are many locations to choose from. Mail Order Pharmacy: Your Express Scripts mail order pharmacy is located in Bensalem, PA. Express Scripts's customer service toll-free number is Mail order forms are available in the Bureau of Human Resources (406). You can charge your prescription by credit card or send a check. Your prescription will be sent to you by regular first class mail.

4 LIFE INSURANCE COVERAGE The group life insurance is a term policy, currently administered by Lincoln Financial/Jefferson Pilot Life Insurance Company. The policy is in the amount of $35,000 (not based on salary) with double indemnity for accidental occurrences. This benefit is employee coverage only and becomes effective after three months of employment. Employees may purchase up to an additional $50, in life insurance coverage, at their own expense, at City rates. Employees will be afforded the opportunity to purchase such insurance in minimum $5,000 blocks on an annual basis during open enrollment periods. PENSION PLAN The City of Harrisburg=s pension plan is administered through the Pennsylvania Municipal Retirement System (PMRS). All employees (except for elected officials) must participate in the pension plan through payroll deduction of 5% of all gross earnings. All pension contributions are pre-taxed. Contributions are placed into an account with PMRS and currently earn a guaranteed minimum of 6.0% interest. The notation of Plan B on your paystub indicates both the amount deducted for pension for the current pay period as well as the year-to-date total. Your pension deduction, along with interest earned will be refunded upon separation of employment (allow 6-10 weeks for payment), unless the employee has 10 or more years of credited service with the City, at which time the pension deduction plus interest can be vested. Plan B Plan B members (hired on or after 12/30/74, or who elected Plan B benefits prior to 12/30/74 in accordance with Section 1 of Ordinance No. 35 of 1984) who attain 65 years of age are eligible for a normal retirement. Effective 01/02/02 the percentage factor used to calculate the pension benefit for Plan B employees will be increased from 1.7% to 2.0% per year of service, with a maximum benefit of 75% of final average salary. Final average salary of Plan B members will be calculated on the basis of the final average annual compensation, including longevity, shift differential and overtime earned and paid during the highest 3 consecutive years of employment. Early Retirement Plan B - Effective January 1, 2000 the definition of early retirement for Plan B members is 55 years of age and 10 years of service. Effective January 2, 2002 a reduction in benefits will be affected for each month under the normal age in accordance with PMRS= Aactuarial equivalent@ reduction factors. 5 For informational purposes only, the following are the actuarial equivalent reduction factors currently in effect. Please note that the table is subject to change. Aqe % of Benefit Age % of Benefit BENEFITS UPON RETIREMENT Management employees hired on or after February 1, 2008 will receive 100% of the health care product in effect at the time of their retirement. Health care coverage will not be provided for the employee's spouse and such will not include vision, dental or prescription drug coverage. Each retiree will receive a $5,000 paid-up life insurance policy upon retirement when the required forms have been filed with the Benefits Coordinator within 30 days of the employee's date of retirement.
