4 Purpose of the RFP To identify non-profit organizations that will provide STD clinical preventive services in order to reduce the spread of STDs among high-risk consumers of safety net health care and STD services in Chicago Fund up to 2 community-based organizations to promote and expand STD screening and treatment among 2 STD high-risk populations: Men who have sex with men (MSM) African American and Latina women ages 15 24
5 Available Funding $309,000 anticipated to be available annually through a cooperative agreement between the CDC and CDPH for an initial term of June 1, 2014 through December 31, CDC may reduce the allowable amount that CDPH can use to support activities under this RFP for years 2-5; therefore, the CDPH reserves the right to reduce funding awards as needed in subsequent years of funding cycle. It is expected that annual reductions will not exceed 1.5% Contract renewal is possible for up to two additional periods, each period not to exceed one year, and is contingent on funding availability and prior program and fiscal performance.

7 Program Goals: Service Category I (MSM) 1. Target screening and treatment efforts to the reduction of syphilis and Gonorrhea (GC) incidence and their respective sequelae among high-risk MSM. 2. Enhance STD clinical preventive education and counseling services with patients, sex partners, and other MSM suspected to have or be at high-risk for syphilis, HIV/syphilis, or GC coinfected patients. 3. Improve integration of STD clinical preventive services into routine clinical care for high-risk MSM utilizing safety net health care.
8 Program Goals: Service Category II (AA & Latina women, 15 24) 1. Provide Chlamydia (CT) and GC screening and treatment to African American and Latina women ages years who are underserved, uninsured, and/or underinsured in high morbidity areas throughout Chicago. 2. Enhance STD clinical preventive education and counseling services with patients and their sex partners. 3. Improve the integration of STD clinical preventive services into routine clinical care to high-risk women.
9 Important Notes Separate requirements for each Service Category under Organization Experience and Target Population (see pp of the RFP). Separate requirements for each Service Category under Program Implementation Category (see pp of the RFP). Service Category I (MSM): Award funds used specifically for screening and treatment and should only be used for MSM who are under or uninsured. Service Category II (AA & Latina Women 15 24): Award funds used specifically for screening and treatment and should only be used for female patients under 25 years of age who are under- and uninsured.

12 Required Documentation Program Work Plan (Appendix E, RFP p. 33) Key Personnel Form (Appendix F, RFP p. 34) Internal Revenue Service 501(c)3 tax exempt determination letter, if applicable Copy of Articles of Incorporation Copy of the most recent Financial Statement OMB Circular A-133 Audit if Respondent received $500,000 or more in federal funds during fiscal year Copy of the completed City of Chicago Economic Disclosure Statement List of Board of Directors (must include place of employment), if applicable Proof of insurance Proof of Medicaid billing authority MOUs from potential partners Supporting Documentation, if applicable.
13 Submission Format 1 original and 4 copies, and 1 CD Minimum 1.5 line spacing and 11 point font Margins of at least ¾ inch on all sides Submit only unbound proposals (no staples, ring binders, or covers) All documents should be on 8 ½ X 11 inch paper Print double-sided Include a table of contents Attach only required supporting documentation Sequentially number all pages, including all attachments No Fax will be accepted
14 Submission Format 1 original and 4 copies, and 1 CD (or thumb drive*) Submit to: Chicago Department of Public Health DePaul Center Room South State Street Chicago, IL Must be received by 4:00 pm Central on October 7, 2014 *not in RFP but an acceptable form of submission
15 Evaluation Criteria Respondent s relevant experience in providing the proposed service and reaching the target population(s) in Respondent s Chosen Service Category Respondent s ability to properly staff and recruit healthcare facility settings Soundness of proposed plan to conduct all STD screening, treatment, partner services and/or linkage to care in order to successfully reach target populations and meet the project goals, objectives and performance standards; Soundness of proposed plan to conduct culturally appropriate STD prevention education activities; Soundness of proposed plan to integrate STD/HIV screening into routine clinical services to address any clinical and prevention service gaps;
16 Evaluation Criteria (Continued) Soundness of proposed plan to utilize a sustainable system of STD/HIV screening, linkage to care and treatment activities, including a plan for use of funds for screening and treatment services only when appropriate; Respondent s ability to routinely provide data to CDPH in the required timeframe and in accordance privacy and confidentiality laws; Soundness of proposed monitoring and evaluation activities; and Soundness of proposed budget and delegate agency s financial capacity and stability to manage a program of the size and scopes contemplated
