1 Form3 DOE Duke of Edinburgh s Award Scheme Expedition Approval Request 1. All D of E Expeditions operated by Aberdeenshire Council require prior approval from the Head of Establishment. Before a D of E Expedition can be approved, the expedition plans will be subject to two levels of checks: Quality Assurance checks from Aberdeenshire Council s D of E team to ensure that the planned expedition meets with the requirements of the D of E Expedition section. Health & Safety checks from Adventure Scotland, who are Aberdeenshire Council s Adventurous Activities Consultants. Final approval will be based on the advice from the Adventurous Activities Consultants. It should be noted that the AAC s advice will be primarily concerned with safety management. 2. This completed form should be submitted electronically along with attachments to: The D of E QA Worker D of E Administration Office Wyness Hall Jackson Street Inverurie AB51 3QB awards@aberdeenshire.gov.uk Once satisfied that the expedition fulfils the criteria required by the D of E and Aberdeenshire Council, the form will be forwarded to Adventure Scotland to check that the proposed expedition complies with Health & Safety requirements. Recommendation for approval will then be sent to the Head of Establishment, and copies sent to the D of E Award Group leader and the D of E QA Worker. It should be understood that Adventure Scotland s recommendations constitute advice regarding Health and Safety. The responsibility for approving the proposed activity or event remains with the Head of Establishment. Please note, D of E Award Group leaders should ensure that their proposed Expedition complies with the requirements of the D of E Expedition section. If in doubt, please seek advice from the D of E QA Worker, Youth Accreditation Support Team. 3 (a) FOR EXPEDITIONS TO NORMAL COUNTRY Complete Parts A and Part B of this form. 3 (b) FOR EXPEDITIONS TO WILD COUNTRY (as defined in the Handbook for D of E Leaders) Complete PART A ONLY of this form PLUS Notification form for Expeditions in Wild Country (Green) form NB: Although Bronze and Silver level expeditions in wild country are not expressly forbidden, the Gold level Expedition Training Framework must be used pedition_forms.aspx Attach a copy of the green form to this application Send the original copy of the green form to the D of E Expedition Co-ordinator 4. MAPS AND ROUTE CARDS should be attached showing the route planned, indicating Start and Finish points (and planned times), Campsite(s) (and times of planned arrival and departure), also any identified alternate or emergency routes.

RISK ASSESSMENTS: Where notifiable activities are being provided on an IN-HOUSE basis, the Council requires that the relevant generic risk assessments which have been compiled for each activity are read, understood and implemented by all supervising members of staff as necessary and appropriate. In addition, any planned activity should be informed by the specific Standard Operating Procedures detailed in the Safety Policy. Please refer to In addition, staff must also consider any additional risks that are specific to their planned activity. These should be detailed in section 8.3 below as a site specific risk assessment.
