2 Choosing your Performance HMO Primary Care Provider (PCP) The Performance HMO Plan separates doctors and hospitals into three distinct networks (Network 1, Network 2 and Network 3). Networks are based on each participating provider group s quality outcomes and health care costs. A key decision in managing your health is choosing your primary care provider (PCP) from one of the 3 Performance Plan networks. Using the grids shown below, you would review and select your network (1, 2 or 3), then choose the medical group within your county. Once you decide on the medical group, you can choose a PCP that best meets your health care needs. 6 Why a PCP is Important Once you select the CS VEBA Performance HMO network and the medical group you want to enroll in, you can then choose your PCP from that network. Having a PCP is an important way to help you stay healthy. You and your PCP will work as a team, along with nurses, pharmacists, and other health care providers, to manage your overall health. Your PCP will get to know you and your current health status. If your health changes, your PCP will be more likely to recognize it, as he or she will be familiar with you and your medical history. Your PCP can use this knowledge to advise you on appropriate treatments, and help you make medical decisions that suit your lifestyle and daily habits. If you don t have a PCP, or you re not comfortable with the doctor you have, think about choosing a new one. Steps to consider when choosing your doctor Figure out what kind of doctor will best meet your needs. Family and general practitioners care for a wide range of health concerns. They may be able to treat family members of any age. Internists treat adults and may have additional training in specialties such as cardiology. Pediatricians provide care to children and adolescents. Geriatricians specialize in the care of older adults. Find your PCP using the online Provider Directory Search for a PCP or other health care professional by name, specialty or ZIP Code. The easy-to-use online provider directory puts participating/contracting providers at your fingertips. You can even get driving directions and print a map to the doctor s office. To find a participating/contracting health care provider in your Performance HMO network: If you are not yet a member and looking for a PCP or other provider in the Performance HMO network: 1 Go to welcometouhc.com/uhcwest 2 Find a Doctor in the dropdown list under I want to 3 On the next page scroll down under California to CS VEBA, and click on Performance HMO Networks 1, 2 and 3 4 Under Select Plan or Service Type, select your Performance Network in the dropdown list, and enter your zip code and provider type 5 You can then search by Location, Name or Medical Group If you are currently a plan member looking for a new PCP or other providers in the Performance HMO network: 1 Log in to myuhc.com, if you have not yet registered on myuhc.com, click Register Now 2 On the home page click Find a Doctor 3 Select Find a Provider 4 On the next page select your State in the two drop down lists 5 Under Select Plan or Service Type, select your Performance Network in the dropdown list, and enter your zip code and provider type 6 You can then search by Location, Name or Medical Group *Based on 2015 updated cost and quality assessments, medical groups may be available in more than one CS VEBA Performance HMO network.
