1 REQUEST FOR STATEMENT OF QUALIFICATIONS FOR PROFESSIONAL ENGINEERING AND CONSTRUCTION INSPECTION CONSULTING SERVICES Submission Due Thursday, December 17, 2015, 4:00 pm (local time)
2 REQUEST FOR QUALIFICATIONS The City of Groveport, Ohio is accepting Statements of Qualifications from qualified engineering firms, licensed to perform work in the State of Ohio and interested in providing engineering and construction inspection services in connection with a consulting engineering contract. Interested parties shall obtain the Request for Qualifications packet on the City s website at by contacting the City Engineer Stephen Farst at (614) , or via at sfarst@groveport.org. Engineering firms interested in being considered should submit their Statement of Qualifications in a sealed envelope no later than 4:00 p.m. on Thursday, December 17, Statements received after this deadline will be neither accepted nor considered. Statements of Qualification should include information regarding the firm s history; education and experience of owners and key technical personnel; the technical expertise of the firm s current staff; the firm s experience in performing engineering studies, design, construction administration and construction inspection; availability of staff; the firm s equipment and facilities; references; and any previous work performed for the City of Groveport, Ohio. See the Request for Qualifications packet for further instructions. Six (6) copies of a Statement of Qualifications, no more than 25 pages in length, should be transmitted to: Marsha Hall, Administrator, 655 Blacklick Street, Groveport, Ohio, 43125, for distribution to an evaluation committee. As required by Ohio Revised Code , responding firms will be evaluated and ranked in order of their qualifications. Following this evaluation, the City of Groveport will maintain a pre-qualification list and will enter into contract negotiations with the most highly qualified firm for a general services agreement. The City may elect to also enter into contract negotiations with other highly qualified firms for specialized engineering services or to fulfill required duties should the City determine at their discretion, it is not in its best interest to utilize the services of the primary firm from time to time. 3 INSTRUCTIONS for Statement of Qualifications NOTE: Six (6) copies of a Statement of Qualifications, no more than 25 pages in length, shall be transmitted in a sealed envelope to: Marsha Hall, Administrator, 655 Blacklick Street, Groveport, Ohio, 43125, for distribution to an evaluation committee. DEADLINE: Thursday 12/17/2015 4:00 pm = ============================== = Section REQUIRED CONTENT of the Statement of Qualifications 1. Cover letter containing a statement of interest and overview of qualifications stating also, the firm name, address, telephone number and name of the individual expected to be the City s point of contact for the submission and for coordinating future work. 2. Competence of the Firm. Please provide: a. Information regarding the firm s history, including number of years in business. b. Owner s names and names of key personnel expected to be assigned to City work. c. Brief discussion of the firm s specific abilities and expertise (reference please Section 3, for the detailed description of prequalification service areas). d. Resumes of key personnel expected to be assigned to the City s work. Resumes shall identify education, technical training, certifications, and other information to demonstrate competency of the firm s staff. e. Evidence of compliance with Ohio Revised Code, chapter 4733, section , enabling firm to lawfully provide professional engineering and/or surveying services. Copy of firm s current Certificate of Authorization from the State Board of Registration for Professional Engineers and Surveyors will suffice. 3. Qualifications & Experience. Please provide: a. The firm s experience in performing engineering studies, design, construction administration and construction inspection, on public works (i.e., municipal, county, state) projects; b. Discussion of similar project experience by the firm s owners and key personnel. Discussion should include project descriptions and address the key individuals role in these projects. c. Copy of the firm s current ODOT prequalification letter if applicable. Firms that are not prequalified by ODOT should include up to four (4) pages of qualifications and experience related to those prequalification areas under which the firm requests the City to evaluate. Please refer to the following ODOT website link for ODOT s Prequalification Information and definitions of their Prequalification 2 P age
4 areas, contained in Part II of document entitled, Consultant Prequalification Requirements and Procedures Manual Ability of the firm to perform the required service competently and expeditiously as indicated by the firm s workload. Please provide: a. Discussion of availability of the firm s staff; b. Discussion of the firm s equipment and facilities, and their availability for City work; 5. Past Performance. Please describe: a. Work and/or services completed by the firm, with emphasis on work completed for the City of Groveport and for other municipal clients b. Firm s approach and success in controlling cost c. Firm s approach and success in meeting contract deadlines 6. Location of the firm s office from which work is expected to be managed. 7. List of References, including name of owner/client, the owner/client contact person familiar with firm s work, nature of the specific work for the owner/client, and contact information (phone, , address). 3 P age
5 ANTICIPATED SCOPE OF WORK The anticipated scope of services under an engineering contract may include, but not be limited to the following: 1. Make studies of existing or newly collected engineering data, reports, etc., which may have been made previously by the City, County or other agencies and give full consideration to the same; 2. Provide complete detailed plans, including necessary field work, specifications, and estimates of costs and provide contract documents consisting of Advertisement, Information to Bidders, Proposal and Proposal Bond Forms, and Contract Performance Bond Forms for the construction of projects as ordered by the Administrator or Council; 3. Furnish to the City the necessary copies of detailed plans, specifications, estimates and contract documents required by them for contracting for and prosecuting the work. Tracings of plans, field books and field records shall become the property of the City if so desired, otherwise shall remain in the files of the Engineer for future reference; 4. Assist at all letting, tabulate proposals and bids, and report same to the City; 5. Presents plans to and assist in obtaining approval of such plans from any Municipality, County, State or Federal Department or other political subdivision which may have jurisdiction in the development of the project; 6. Prepare all current and final pay estimates for construction work in conformance with the conditions of each contract; 7. Provide land surveying field parties to perform construction staking layout; 8. Provide project representation during construction, including furnishing of full-time resident inspection personnel as the work requires and furnishing of construction management services when requested for Capital Improvement Projects; 9. Advise and recommend to the City in the matter of testing materials and reviewing laboratory results; 10. Inspect completed work and submit a final report for the acceptance of the project. 11. Maintain a complete record of the progress of the work and all incidents relative to the construction process; 12. Provide GIS related assistance to the various City departments; 13. Provide plan reviews of private development site plans for compliance with City standards upon request that may be submitted to the City 4 P age

Conduct technical studies, provide planning services and mapping, and provide other technical assistance to the City Council, Administration, employees and Boards and Commissions, as may be requested by the Mayor or Administrator; 15. Property or easement acquisition: Furnish personnel to make property surveys, legal descriptions of same, make easement surveys, prepare descriptions, and assist in the acquisition of both property and easements across private property; 16. Attend meetings of City Council, Planning & Zoning Commission, Board of Zoning Appeals, and other board or committee meetings as requested; 17. Other services: Arrange for the services of an independent agency to make development or redevelopment studies, plans, and guidelines, and for such test borings, subsurface explorations or other investigations as may be required for the determination of the foundation conditions for streets, sewers, water mains, elevated tanks or other types of structures. The services so arranged for shall be subject to the approval of the City. 5 P age
