2 History of Homelessness Severe and prolonged psychiatric disorders affect about % of the adult population. Significant psychiatric history or current impairment is commonly found among 20-30% of the homeless population. People with psychiatric disabilities have at least a tenfold excess risk of homelessness above that of the general population. It is estimated that 1 in 20 adults who experience severe psychiatric disabilities is homeless in the United States.
3 History of Deins/tu/onaliza/on As deinstitutionalization proceeded in the 1960 s and 1970 s, the typical length of stay in psychiatric hospitals shortened dramatically, whereas the number of admissions to these institutions increased. Beginning in the early 1980s, economic factors, combined with a rapidly shrinking pool of inexpensive housing throughout much of the country, contributed to a dramatic rise in the number of homeless people with serious mental illness.
4 Coordina/on of Care for people with a SMI designa/on Unfortunately, housing assistance programs, mental health services, and social service programs generally remain fragmented and difficult to negotiate. People with serious psychiatric disabilities are among the poorest of the poor. Their personal income is often limited to Supplemental Security (SSI) the Federal entitlement program that provides an income to people who are assessed to be permanently disabled. People who rely on SSI for their income are too poor to obtain decent housing without other forms of assistance.
5 Sca=ered Site vs. Project Based Scattered Site Housing is located through out the community and tenant chooses where they want to live to be near natural supports such as: Schools, clinics, family, church, etc.) Services are delivered in individuals home. Participant can move to a new location once lease is up after one year. Project Based Housing is located at one fixed site location with the housing subsidy attached to that unit not necessarily with the person. Most offer on site supportive services based on each individual or family need, a community type environment.

7 Permanent Suppor/ve Housing (PSH) Decent, safe, and affordable community- based housing that provides tenants with the rights of tenancy under state and local landlord tenant laws and is linked to voluntary and flexible support and services designed to meet tenants, strengths, needs and preferences.
8 Housing Programs Arnold vs. Sarn Lawsuit Settlement Mercy Maricopa Integrated Care (MMIC) created PSH scattered site for people in the RBHA with a SMI designation. Additional Supports in PSH provided by: Marc Community Resources, Southwest Behavioral Health (SWBH), Community Bridges, Inc., PSA Behavioral Health and Arizona Health Care Contract Management Services (AHCCMS)
9 Housing Programs ABC Homeless Housing Program Funding from U.S Department Housing and Urban Development (HUD) Continuum of Care Programs (Old SHP& S+C) Tenant Based Rental Assistance, Permanent Housing Scattered- site, utilizing private rental market Serve disabled populations, specifically individuals designated to have a Serious Mental Illness (SMI) Must meet *HUD definition of homelessness Referral from RBHA case managers or advocate
10 HUD Homeless Defini/on ABC Homeless Housing Program Applicant lives in places not meant for human habitation, such as cars, parks, sidewalks, and abandoned buildings (on the street). Applicant has been in an emergency shelter. (CASS, Watkins, Sojourner, UMOM, etc.) Applicant is in transitional housing for homeless persons who originally came from the streets or emergency shelter (Must be time limited up to 2 years)
11 Housing programs ABC Homeless Housing Program ABC contracts with housing providers (HOM Inc., Biltmore Inc.) to provide all housing related services such as eligibility, Rent calculations, Housing Quality Standard (HQS) inspections, etc. Landlord and participant have a lease Housing Provider and landlord sign Housing Assistance Payment Contract (HAP) Program runs similarly as Section 8 program, except terminations occur as a last resort. Annual recertification and housing inspection required (Yearly) Start- Up box upon move- in ($50 Gift Card)
12 Quali/es of a Good PSH Program Housed individuals/ families are integrated into their community of choice. Supportive services provided to assist in maintaining housing. (Voluntary and Flexible) Rent is subsidized based on income (30%) Individual chooses own unit and location to live based on personal needs and preferences to be around family or natural supports in the community. Termination is always last resort (RETENTION!)

14 Subsidized Housing not PSH Section 8 now called Housing Choice Voucher Program Available for people who have a very low income Some have preferences for living/working in city Must submit pre- application for placement on waitlist Long waitlist and at minimum annual updates Once selected from waitlist Determine eligibility based on income, checks for criminal background, citizenship and if owing money to other housing programs
15 Subsidized Housing not PSH cont.: Landlords can also screen applicants, and they can choose to not accept Section 8 Vouchers Annual requirement for recertification and housing inspection Terminations Many housing authorities have a one- strike rule Understand rules and report all changes of income and household composition

Time limited and service oriented Many are targeted to working families Some are targeted to homeless youth Some programs offered within the RBHA often are listed as transitional such as Restart, and ACT Housing (Assertive Community Treatment) HUD does NOT consider Half- Way houses or Rehab as transitional

19 What this looks like: Housing Specialist: Each person is paired with a Housing Specialist who serves as a living example that recovery is possible. They are able to relate to the person and, most of all, offer hope in ways that traditional services may not be able. Behavioral Health Technician: A BHT ensures all individuals receive clinically appropriate treatment planning and medical records are up to date and accurate.
20 What this looks like conbnued: BENEFITS SPECIALIST : This professional helps to ensure the financial stability of individuals by monitoring government benefits and helping individuals navigate the application and renewal process. PROGRAM COORDINATOR : This Professional is responsible for the daily operation of program activities to ensure services are delivered effectively and individuals needs are met. PROGRAM MANAGER This professional provides clinical and administrative oversight and direction of program, monitors outcomes, fidelity to best practices and program development.
21 Housing OpBons: TransiBonal Living Program (Relias Training) Eligibility: Determined to be SMI and Title 19, must be Enrolled with MMIC Not for individuals waiting for Level II 24 hour prior authorized residential treatment Program Description: Short term transitional housing opportunity for up to 30 days to assist individuals transition to permanent housing/residential treatment Staff is onsite during day into early evening, but not overnight

29 What we know in Permanent SupporBve Housing: Under a 5 year (extended to 6 year) grant funded by SAMHSA starting in 2009 the Hope Network program with Marc Community Resources is designed to provide rapid access to housing with strong links to community services focusing on: recovery, treatment, resiliency, and wellness. Also known as Permanent Supportive Housing Services. Through a third party organization: Inter- growth with Tom Pynn, CEO and Associates located at 910 W Kathleen Rd Phoenix, Arizona 85023, evaluations at baseline, 6 months, and exit were completed using the assessment tool G.A.I.N.S. (Global Appraisal of Individual Needs).

44 The Hope Program with Marc Community Resources has been providing Permanent Supportive Housing Services for participants within Maricopa County seeking to obtain and sustain independent living for the last 6 years. In that time the Hope Program has expanded to assist not just the population defined as chronically homeless, but also those with Non-Title 19 status, those paying rent independently, and many others. The teams adopted motto is If we can t do it, we will find out who can! The team has been successful in assisting 87% of program participants in maintaining their independent living. The Hope team works towards ending all homelessness for all member of the community by building resilience, independence, community connection, belief in self, and most importantly HOPE and keeping it alive and thriving!
