2 Why are we here? The IT Committee has assigned 20 champions to ~356 organizations Goal of the champions is to: Develop relationships with PPS partners Share key centralized IT strategies Extend critical information to partner organizations Current IT strategy Targeted partner engagement 2
3 Mount Sinai PPS Vision The goal of the Vision is to create: Population health focused, Integrated delivery system for Manhattan, Queens and Brooklyn that, Improves the quality of care and health outcomes for our most vulnerable populations, While reducing overall costs The Delivery System Reform Incentive Payment (DSRIP) Program is a tool by which our PPS can build its population health infrastructure over the next five years. 3
4 DSRIP Overview & Goals Delivery System Reform Incentive Payment (DSRIP) Program has three key goals: 1 Safety Net care delivery transformation at the state and system level 2 25% reduction in avoidable hospital use 3 Value-based payment and population health sustainability 4
5 How do we achieve these goals? 1. Participate in the 10 DSRIP projects below, selected from a list of project options provided by the state: 2.a.i. Create Evidence Based Integrated Delivery Systems 2.b.iv Care transitions intervention model 2.b.viii Hospital- Home Care Collaboration 2.c.i. Communitybased health navigation services 3.a.i Integration of Primary Care and Behavioral Health 3.a.iii. Medication Adherence Program in Community Sites 3.b.i. Evidencebased strategies for disease management (cardiovascular) 3.c.i Evidencebased strategies for disease management (diabetes) 4.b.ii. Chronic Disease Prevention 4.c.ii Increase early access to, and retention in HIV care 2. Leverage these projects to achieve DSRIP project metrics and milestones 5

10 Key Assumptions for Centralized IT Services Strategy Complete set of Centralized Services designed to perform the four functions listed in the previous slide All services in this presentation and the associated materials only contain the technology aspect of these services. The operational and clinical aspects will be informed by the other PPS committees. The Centralized Services Strategy is currently in draft form. All services and scope are subject to funding allocated to support the current strategy, validation from clinical and project teams, and further clarification of state services/requirements 10

17 Why is this important for you? We succeed if you succeed All partners MUST perform for the entire PPS to succeed. If one partner fails in meetings metrics, that impact will potentially result in reduced funding available for the entire PPS Assessments Partner investments/ requirements 17
18 Data Sharing Pathways MSPPS IT Committee has developed data sharing pathways that will help facilitate successful patient lookup/ query, alerts, and messaging, subject to constraint by the partner s level of IT maturity: 1. Pathway A Partner organization has EHR and its connected to an HIE/RHIO (bi-directional) 2. Pathway B Partner organization has EHR or HIE that is capable of sending data (uni-directional) 3. Pathway C Partner organization has EHR but no HIE or the HIE is not capable of sending data 4. Pathway D Partner does not have an EHR or an electronic platform 18
