3 + NC PATH Identity NC PATH: Elevating Care. Eliminating Barriers. What is it? The North Carolina Program to Advance Technology for Health (NC PATH) is a partnership between Blue Cross and Blue Shield of North Carolina, NC Health Information Exchange and Allscripts. This program was developed to elevate the quality of health care, while eliminating barriers to health IT adoption for providers. BCBSNC has invested $13.4 million in NC PATH for technology, connectivity and support. NC PATH assists providers in: Streamlining coordinated care Elevating the quality of patient care Improving health outcomes Reducing health care spending Becoming HIT compliant
4 + NC PATH Program Overview NC PATH equips eligible independent physician practices, free clinics and qualified specialty providers across North Carolina with: Allscripts Professional Suite (EHR & PM) Integration with the NC Health Information Exchange (including direct secure messaging) Support in demonstrating Meaningful Use (for PCPs and free clinics) Support in achieving Patient Centered Medical Home certification or other quality measurements Collaboration with providers interested in the BCBSNC Blue Quality Physician Program Consulting and educational opportunities through program partners Access to community web portals and online tools Referrals to local support organizations
5 + NC PATH Identity Technology Solutions: Allscripts Professional Suite Regulatory compliance (Meaningful Use Stage II and ICD-10) Robust PM and revenue cycle management capabilities and more advanced financial and clinical analytics Extensive clinical content, real-time point of care/clinical decision support, and advanced clinical reporting capabilities Membership in a Regional Allscripts User Group and access to more than 15,000 members already in the Pro community and 2,700 in North Carolina Mobility and ease of use with native ipad and iphone solutions (not included with or required for NC PATH participants)
6 + NC PATH Identity Technology Solutions: NC Health Information Exchange NC HIE simplifies the process by which health care providers access and share patient health information. NC HIE is the state designated HIE and operates statewide. Without NC HIE: Each health care provider must build point-to-point connections With NC HIE: Each health care provider is connected through NC HIE; eliminates need for point-to-point connections
7 + NC PATH Support Solutions NC PATH participants are eligible for a number of support resources Software Training (EHR and HIE) Program Office Support Meaningful Use (through AHEC federal grant) Patient Centered Medical Home (PCMH) Blue Quality Physician Program Referrals to hardware vendors (hardware is not included in the NC PATH program) Membership in Allscripts Online Community
8 + NC PATH Identity Program Requirements: Eligibility NC PATH is open to free clinics and providers who: are independent (not affiliated with a hospital system), in-network (BCBSNC) and do not currently have a certified EHR in place. Primary Care Providers: Pediatricians Family practitioners Internal Medicine General practitioners OB/GYNs Specialty providers: Endocrinologists Cardiologists Pulmonologists Podiatrists Vascular Surgeons Nephrologists ENTs Other specialists that can demonstrate quality improvements (approved on a case-by-case basis)
9 + NC PATH Identity Program Requirements: Commitments As a program committed to elevating quality for physicians, NC PATH asks its participants to pursue higher quality of care through: Meaningful Use attestation PCPs receive AHEC support at no cost Patient Centered Medical Home (For PCPs) Measurable quality improvements: diabetes, hypertension, tobacco cessation, asthma, cholesterol (For specialty providers) BQPP (Available for able and interested providers)
10 + NC PATH Identity Program Requirements: Commitments A successful NC PATH participant: Commits resources for a timely and successful implementation and post implementation objectives Incorporates the technology into practice systems and day-to-day functions Acts in accordance with the NC HIE fair information use and privacy policies Participates in NC PATH end-user surveys and annual user group meetings
11 + NC PATH Identity Program Cost Thanks to BCBSNC s commitment, providers are only responsible for 15% of the cost of the program over 5 years; the program is free to free clinics Participants receive the EHR software solution (hosting, support, and maintenance), NC HIE connectivity and program office support Once a provider becomes a participant of NC PATH, the Allscripts Professional Suite license belongs to that provider After 5 years, the practice or free clinic must decide if it would like to continue using the support, hosting, maintenance and NC HIE services. Providers continuing after the 5 year term will be responsible for covering the ongoing maintenance costs for EHR and NC HIE connectivity
12 + NC PATH Identity Why NC PATH? NC PATH is the only EHR subsidy program of its kind in NC. It represents an enormous cost savings and provides support for 5 years. Integration to the statewide HIE (the designated connection for NC immunization registry, communicable disease reporting, state lab, and cancer registry). Providers can earn financial incentives by properly implementing and demonstrating meaningful use of a certified EHR and pursuing quality measures.
