1 Wenlock District Hospital, Mangalore Wenlock Hospital is a well known government District hospital located in the heart of Mangalore city. It was established in the year 1848 and since then it has been providing medical care to the poorer sections of the society of Dakshina Kannada, Udupi, Uttara Kannda, Shimoga, Chikmagalur, Hassana and Coorg district of Karnataka state. In addidition the patients from the neighboring districts of Kerala are also availing medical facilities from this hospital. Wenlock District Hospital is a 705 bedded hospital. An additional 200 bedded Peadiatric wing has been donated by Infosys foundation. This wing is named as Reaginal Advanced Peadiatric Care Centre. Administration: The hospital is headed by the District Surgeon and Superintendent. The following officers assist the District Surgeon with the following responsibilities. 1. Surgeon & Resident Medical Officer is in charge of Stores, Medical Record Department and the Medical officers of the hospital. The RMO is responsible for the regular functioning of the hospital. RMO is the treasurer of the Arogya Raksha Samithi of the Hospital. 2. Nursing Superintendent Grade I is in charge of the Nursing staff and Group D employees. She is responsible for the overall housekeeping of the hospital. She supervises staff nurses, senior staff nurses and Nursing Superintendent Grade II. 3. Principal, School of Nursing is responsible for the admission, teaching, examination of the nursing students and she is the coordinator and supervisor of the teaching faculty. School of nursing is at present functioning in the 1 st floor of the OPD block of the hospital. 4. Principal, Junior Health Assistants Training Centre is responsible for the teaching and conducting examinations of the Junior Health Assistant trainees with the coordination of tutors of the training centre. She is responsible for the welfare of the students as the training centre is a resident one. The training centre is located in the hospital premises. 5. Principal, Senior Health Assistants Training Centre is responsible for the teaching and conducting examinations of the Senior Health Assistant trainees with the coordination of tutors of the training centre. She is responsible for the welfare of the students as the training centre is a resident one. The training centre is located in the OPD block of the hospital. 6. Assistant Administrative Officers: There are 2 Assistant Administrative Officers to assist the District Surgeon in the administration of the hospital. Out of the two AAO posts one is delegated with the drawing officer powers. They are responsible for the financial affairs, Service related matters etc. The AAO s are assisted by 3 Office Superintendents, 10 First Division Assistant and 17 Second Division Assistants. However out of the 17 Second Division Assistants 13 posts are vacant.

FNAC/ Pathology/ Microbiology Laboratory 7. Malaria Laboratory 8. Color Doppler 9. Endoscopy Other Services 1. ART, ICTC: ART and ICTC are working under the District Aids Prevention and Control Unit. The District Aids Prevention and Control officer is the programme officer in the District. There are 27 ICTC Centers working in various Government hospitals and medical college hospitals in the district. Each ICTC is managed by one councilor and one laboratory technician. 2. Pharmacy Department: Pharmacy department consist of main store, sub store and OPD pharmacy. The main store which receives the bulk stock from the suppliers & GMS consists of two wings viz. 1. Tablets and injections. 2. Consumables, Disposables and Chemicals. Each wing is managed by the senior pharmacist assisted by the pharmacists. The substore procures tablets and injections from the main store and distributes to the wards and OP pharmacy is managed by the pharmacists. 3. Medical Record Department: Assistant Medical Records Officer is in charge of MRD. Under him 2 Medical Record Technician posts are sanctioned. The department is computerized by Kasturba Medical College. The OP/IP data is updated daily by the KMC staff. However the AMRO and 2 MRT posts are vacant. In view of this 1 Medical Record Technician has been appointed on contract basis with the approval of Arogya Raksha Samithi of the hospital.

5 5 4. DOTS: The Dot Centre which is under the administrative control of the District Tuberculosis Officer is headed by a Medical Officer and other field staff. The patients suffering from Tuberculosis are administered TB drugs and follow up action is taken up by the DOT centre. National Health Programmes: 1. District Mobile Opthalmic Unit: Is headed by a senior specialist and assisted by the camp co-ordinator of the rank of Deputy Health Education Officer. He is assisted by the 1 Senior Opthalmic Assistant and 1 Opthalmic Assistant. The DMOU organizes eye camps in various places of the District periodically to detect the eye defects. The deserving patients are brought to the hospital and surgeries are conducted including cataract operations. The camp coordinator in addition to organizing the camp arranges health talks to the rural population. 2. Urban Family Welfare Centre: There are 4 Urban Family Welfare Centre functioning under the administrative control of Wenlock District Hospital. Viz. 1. Urban Family Welfare Centre, Wenlock Hospital. 2. Urban Family Welfare Centre, Attavara, 3. Urban Family Welfare Centre, Jeppu, 4. Urban Family Welfare Centre, Kuluru. Each of the Urban Family Welfare Centers is headed by a medical officer. One Senior Health Assistant and 3 Junior Health Assistants and 1 Second Division Assistant post are created to each of these Urban Family Welfare Centers. These Urban Family Welfare Centers carryout the national programmes such as immunization, maternal care, sterilization etc. 3. Arogya Raksha Samithi: Arogya Raksha Samithi of Wenlock and Lady Goschen Hospitals is constituted under the societies act. Arogya Raksha Samithi is headed by the District Minister and consists of 20 other official members. 6 non official members including 1 lady member and 1 member from the SC/ST community has to be appointed by the government. Dean, KMC, Mangalore Special invitee 6 The Arogya Raksha Samithi receives donations from the general public, collects the prescribed fees from the patients for investigation and treatment. And also fees are collected from Kasturba Medical College, Mangalore and other Nursing and Para Medical Institutions availing clinical facilities in Wenlock and Lady Goschen Hospitals at the following rates. OT, ECG, EEG, Refractionist etc. 3, Sampoorna Suraksha Programme: This is a programme implemented by Sri Kshetra Dharmasthala Rural Development project, Dharmasthala for the complete protection against hospitalization expenses for the poor people by the poor people. This programme is aimed at protection of the families against unforeseen expenses like hospitalization, maternity, death and other hazards. Sampoorna Suraksha has been designed in such a way that the members on admission for treatment get cashless treatment in hospitals. Wenlock hospital has implemented this programme from July Facilities: 1. In a single member family, he can get cashless treatment to the maximum of Rs.5,000/-. Cover under this programme for more members in the family increases by Rs.5,000/- per member. The coverage is on floater basis in multiples of Rs.5,000/- per person. 2. Yearly contribution is collected along with the weekly savings of the members of the self help group so that it does not strain the finance of the family. 3. Cashless treatment facility in well equipped hospital with modern amenities recognized by SKDRDP. 4. In case of necessity any one member of the family can avail the benefit for any one ailment to the extent of maximum limit available to the family.

7 7 5. Zero rejection policy: In a unique methodology SKDRDP ensures that no person admitted to hospital will be denied of settlement if he makes claim under the scheme. 6. Rastriya Swasthya Bheema Yojane: This programme is implemented jointly by the Central and State government. In our state labour department has been appointed as the nodal agency. Wenlock District Hospital has implemented this programme from the year Under this scheme all BPL families are brought under the health insurance plan. In Karnataka 41,578 rural BPL families identified in the year 2002 by the survey conducted by rural development and Panchayathraj department are covered under this scheme. 2. Under this scheme the head of the family, wife and three dependents (including children) are entitled to avail free medical facility up to a maximum of Rs.30,000/- per year. 3. Under this scheme cashless treatments are provided for all diseases including maternity. 4. Whenever a member of the BPL family covered under this programme visits the hospital for treatment, he or she is entitled for the travelling allowance of Rs.100/- each time subject to a maximum of Rs.1,000/- per year per family. 7. Yashaswini Sahakari Raita Arogya Rakshana Yojane: This scheme has been implemented in Wenlock District Hospital during the year The scheme is aimed at protecting the health of the rural cooperative members. 1. This is not a insurance plan. This scheme is applicable only for the surgeries. The medical treatment not requiring surgery does not include in this programme. Eligibility criteria 1. The main applicant should be a member of any eligible cooperative society on the first day of June of the registration year. 2. The main applicant is entitled to enroll all eligible members of the undivided family. 3. Under the scheme everybody within the age group of 0-75 are eligible to be a member. But a person above the age of 75 is not entitled to be a member of the Yashaswini programme. 4. Under this programme the main applicant and his family members should pay the annual contribution prescribed by the trust at the time registration. 5. The registration is limited to a specific period every year. 6. If 5 or more members are enrolled in a family, they are entitled for 15% rebate in the contribution. 7. The members who have paid the specified contribution are entitled to undergo any surgery during that particular year without payment. 8. Duration of the scheme is one year, starting from 1 st of June and ending on 31 st May. After the expiry of this period the facility can be availed after renewal of the membership. 9. Under this scheme normal delivery, neonatal care, accident due to agricultural equipment, drowning, dog bite, snake bite, bull stab injury, electrical accidents are covered in addition to 1600 identified surgeries.

8 8 10. Cancer treatment such as chemotherapy, radiotherapy, joint replacement, burns, road accident, dental surgery, skin grafting, stunts, implants etc. are not covered under this scheme. 11. The Yashaswini trust will not be responsible for the treatments not covered under this scheme and the hospitals authorized by the trust. 12. The beneficiaries shall produce the original identity card and the initial contribution cash receipt in the recognized hospitals to get the treatment. 13. The medical officers will examine the patients if found necessary and if such surgery included in the scheme, requisition for surgery will be forwarded to the implementing agency. And the implementing agency shall examine the requisition and if eligible under the rules of the scheme shall permit for the surgery. 14. If the information in the identity card does not tally with computerized data or if wrong information were submitted and if complete documents are not provided the request of such beneficiary will be rejected. 15. It is mandatory to get the pre authorization to get the benefit of the scale from the implementing agency. 16. The trust is not responsible for the treatments availed in the hospitals which are not recognized by the trust. 17. The beneficiaries while visiting the hospital for treatment should provide all the information noted in the registration application. Incomplete application are liable to be rejected. 18. The identity card are not transferable. If any beneficiary secures the card by giving wrong information and in violation of Yashaswini group or misuses the card such cards are liable to be cancelled and such persons are liable to the punished. 8. Bala Sanjeevini Yojane: This is a centrally sponsored scheme under the child development programme. Under this scheme the anganvady children of age group of 0-6 years belonging to BPL category and suffering from illness and malnutrition requiring Tertiary Care treatment are entitled for free treatment in the selected 5 hospitals of the state up to a maximum of Rs.35,000/- and Rs.50,000/- for the treatment of the neonatal baby. The following hospitals are authorized to treat the children under this scheme. In special circumstances the child of any district can be treated any of the following hospitals. 1. Indira Gandhi child health institute, Bangalore covering the districts of Bangalore (Urban), Bangalore (Rural), Mysore, Mandya, Kodagu, Kolara, Chamaraja Nagara, Hassan, Chikmagalur, Tumakuru, Chikkaballapura & Ramanagar. 2. Bapuji child health institute, Davanagere covering Shimoga, Davanagere, Haveri, Koppala & Chitradurga District. 3. Dr. Prabhakar Kore Free hospital, Belgaum covering Darvad, Belgaum, Bellary, Bagalakote, Bijapura & Gadag District. 4. M.R. Medical College, Gulbarga covering Bidar, Gulbarga & Rayachur Districts. 5. Kastuba Medical College, Mangalore (Wenlock & Lady Goschen Hospitals) covering Uttara Kannada, Dakshina Kannad & Udupi districts. The anganavadi children between the age group of 0-6 can avail treatment for the following illnesses. 1. Severe pneumonia 2. Encephalitis-Meningitis.

9 3. Complicated malaria. 4. Anemia. 5. Diabetes. 6. Renal problems- Urinary Bladder/ Kidney problems. 7. Tertiary Care treatment of Neonatal baby. 8. Liver problems. 9. Neurological disorders. 10. Secondary malnutrition. 11. Complicated Diarrhea. 12. Surgery of neonatal baby. 13. Treatment of snake bite. 14. Poisoning. 9 Identification of the children coming for treatment under this scheme is the responsibility of anganavadi worker/ supervisor/cdpo and medical officer of PHC/PHU. The child so identified should be admitted to the hospital by the anganavadi worker/ supervisor along with the referral card. The anganavadi worker/ supervisor/cdpo and medical officer of PHC/PHU should sign in the referral card. The referral card should be accompanied by the BPL card and the photo of the child. Treatment cost: 1. Rs.35,000/- for the child requiring Tertiary care treatment and for the neonatal baby Rs.50,000/- (subject to actuals) excluding TA/DA of the mother/ father/ guardian. 2. The mother/ father/ guardian (only one person) is entitled for 2 nd class train fare/ bus fare and daily allowance at the rate of Rs.100/- per day till the patient is hospitalized. 3. The cost of the following items which are not available in the hospital and necessary for the treatment can be borne under this scheme. 1. Drugs. 2. Consumables such as syringe, glucose, oxygen, ventilator etc. 3. Investigation done outside if the facility is not available in the hospital. The following departments will cooperate in the implementation of this programme: 1. Women and children welfare department. 2. Health and family welfare department. 3. Medical hospitals and medical education institutions. 9. Handicap Certificates to the disabled persons: The specialists of the Eye/ medical/surgical/ Orthopedics/ ENT & Psychiatry departments of this hospital have been entrusted with the responsibility of examining and issue of medical certificates on all Mondays and Tuesdays to the mentally/ physically disabled persons. 10. Citizen Help Desk: The KHSDRP and the NRHM have been envisaged to launch various innovative schemes for the poor vulnerable groups and underserved areas of the state. The project aims at increasing utilization of qualitative and measureable public health services. As a part of this Citizen help desks are constituted in all the district hospitals of the state. Objectives.

10 10 1. To improve services at the Government hospitals by education of staff and public about facilities and services available: monitoring the quality of health care. Ex: time taken by doctor to attend a patient, availability of free drugs etc.: timely feedback to the concerned authorities regarding staff and drug shortages etc. 2. To enhance transparency and accounting in the delivery of services through the help desks which will give regular feedback and other support. 3. To empower the public with information from help desk and provide guidance through posters, leaflets and boards etc. 4. Enhance the efficiency of the service provision by staff by a strategy of counseling, confidence building and motivation. 5. Establish a link between service providers and users through regular feedback mechanism. 6. Conduct of regular interaction camps in the areas inhabited by poor to build confidence amongst the poor to access services at Public hospitals. Working Schedule 24/7: On all days of the year. The service provide: 1. The required information & guidens to the patients regarding the facilities available in the hospital and the charges levied etc. 2. The citizen help desk staff help in attending the operate console of the PRI. 3. Attend the handicapped certificate issue camp on every Monday and Tuesday organized in the hospital. 4. Collect feedback on the quality of services from patients (in-patients and out patients) and identify bottlenecks in effective delivery of services. 5. Register grievances with regard to health service through help line, , letter in drop box. 6. Redress grievances by referring them to the concerned officials and follow up. 7. Acts as channel of communication to the Arogya Raksha Samithi 11. Role of KMC in Wenlock: Kasturba Medical college is providing clinical services to the patients of this hospital. In addition they provide following services. 1. Laboratory services 2. Appointed 50 staff nurses to work in this hospital. 3. Cleaning and Security Services. 4. Equipment maintenance.
