9 COMMUNITY HEALTH RESOURCES 7 Ledgebrook Drive, Mansfield, CT Central Access & Crisis Triage Center for new appts Care Coordination- provides intensive coordination for children and youth age 0-17 with emotional/behavioral difficulties and complex service needs and their families. Care Coordinators assist the family through a child specific team that develops individual and family focused service plans. The coordinators also provide ongoing consultation with the child specific team to provide linkage, coordination, follow-up and advocacy for the child and family. Call ext 228 for more information Early Childhood Consultation- provides clinical consultation, training, clinical assessment, intervention, and linkage to services for daycare and preschool providers. The program focuses on children age 0-5 who are attending pre-school and who are in need of consultation to identify strengths and barriers to their care and early intervention to address mental health needs. Call ext. 348 for more information. Emergency Mobile Psychiatric Services (EMPS) - provides telephone triage and support as well as emergency mobile assessment in the community, at home or at school. EMPS supports children and families in times of emotional, behavioral and situational stress. EMPS focuses on prevention and early intervention, stabilization, diversion from hospitalization, and when necessary access to hospitalization. Call 211 for services. Functional Family Therapy (FFT) provides in home therapy to children age with a variety of mental health problems once a week. Family must commit to treatment. Targeted for families with limited resources, those who have experienced treatment failure, and who have multiple system involvement. Call ext. 336 for more information. Intensive In-Home Child and Adolescent Psychiatric Service (IICAPS) provides communitybased clinical, case management, and case coordination services for children and adolescents aged birth to 18, with serious emotional disorders. The goals of IICAPS include working with family in their home and community to stabilize and maintain children in their homes, develop strength-based comprehensive assessments and treatment plans, and to improve the child s relationships within his or her school, family, and community. 18 UNITED SERVICES VOCATIONAL REHABILITATION PROGRAM Offices in Dayville, Willimantic, Wauregan and Columbia The Vocational Services component helps adults recovering from severe and prolonged psychiatric illness deal with the following social/personal adjustment problems: Lack of successful employment experiences because of psychiatric disabilities and/or hospitalization Lack of basic vocational skills and work habits needed to accomplish career goals because of the effects of psychiatric disability or vocational development. The staff of Career Options works in partnership with local employers to help them find the employees they need. Staff work with clients to improve not only the vocational skills necessary for employment but also the social skills necessary for them to interview well and land and keep a job. Eligibility: Meets eligibility criteria of severe and prolonged mental illness as defined by the Department of Mental Health and Addiction Services. Funding is provided by the Department of Mental Health and Addiction Services. UNITED SERVICES TICKET TO WORK Program Offices in Dayville, Willimantic, Wauregan and Columbia Several million disabled and blind Americans receive monthly Social Security or Supplemental Security Income (SSI) payments. Some receive both. Many also have help from Medicare or Medicaid in paying medical bills. The impact of these programs in meeting people s needs is significant. However, many people with disabilities want to work. To help them, Social Security has a variety of work incentives, including the Ticket to Work Program. Some of the work incentives include keeping your cash benefits and medical coverage while you transition to the work place.
